Ibuprofen

General characteristics. Structure:

Active ingredient: 5 g of an ibuprofen in 100 g of ointment.

The ibuprofen has antiinflammatory and analgeziruyushchy effect.

Pharmacological properties:

Pharmacodynamics. Drug belongs to non-steroidal anti-inflammatory drugs for external use. Suppresses products of mediators of an inflammation. Inhibits cyclooxygenase and blocks biosynthesis of prostaglandins. Antiinflammatory action is caused by normalization of a hyperpermeability of vessels, microcirculation improvement, reduction of a liberation of a histamine, bradikinin and other mediators of an inflammation, braking of formation of ATP that causes decrease in power supply of inflammatory process.

Analgeziruyushchy action of an ibuprofen is connected with decrease in intensity of an inflammation, reduction of development and weakening of an algogennost of bradikinin. The dimethyl sulfoxide possesses local nonspecific antiinflammatory, analgeziruyushchy action, promotes the best and deeper penetration of an ibuprofen into fabrics.

Ointment ibuprofenovy has the bystry and expressed anesthetizing effect - weakens a pain syndrome, including an arthralgia at rest and at the movement. Reduces hypostasis and a hyperemia. Reduces morning constraint and restriction of mobility in joints.

Pharmacokinetics. At local use in the form of 5% of ointment the ibuprofen is soaked up slightly and has no the expressed systemic action on an organism. Slowly gets into a joint cavity, but is late in synovial fabric, creating in it big concentration, than in plasma. Is exposed to biotransformation with formation of three main metabolites.

Products of metabolic disintegration are brought out of an organism by kidneys. The dimethyl sulfoxide with a high speed gets through biological membranes, including through the unimpaired skin, mucous membranes. It is metabolized with formation of methyl sulphone and methyl thiomethane. Not changed dimethyl sulfoxide and methyl sulphone are removed with urine and excrements, methyl thiomethane is excreted through lungs and skin.

Indications to use:

Inflammatory and degenerative diseases of a musculoskeletal system: arthritis (rhematoid, psoriasis, gouty), tendivit the humeroscapular periarthritis ankylosing a spondylarthritis (Bekhterev's disease), an osteoarthrosis, osteochondrosis with a radicular syndrome, radiculitis, a tendovaginitis, a bursitis, a lumbago, a sciatica, a mialgiya, injuries without disturbance of integrity of soft tissues (dislocation, stretching or a rupture of muscles and sheaves, a bruise, posttraumatic hypostasis of soft tissues, etc.).

Route of administration and doses:

Drug is used outwardly. The strip of ointment 5-10 cm long is applied on skin in the field of the painful site and carefully rubbed the easy movements. Ointment is applied by 3-4 times a day daily. Duration of treatment is defined individually, depending on disease severity and the nature of damage.

Features of use:

Drug is intended only for external use.

In hard cases it is recommended to combine local therapy with reception of non-steroidal anti-inflammatory drugs inside.

It is necessary to avoid hit of ointment ibuprofenovy on mucous membranes or in eyes.

Emergence of sharp reddening and itch testifies to hypersensitivity to drug (use is not recommended).

Side effects:

As a rule, adverse effects at use of drug are not observed.

The dermahemia, burning sensation or prickings can be noted.

Allergic reactions (skin rash, itch, Quincke's edema) are in rare instances possible. At emergence of allergic reactions treatment by drug should be stopped, in case of need it is necessary to carry out the desensibilizing therapy.

Interaction with other medicines:

It is not established cases of negative medicinal interaction at purpose of ointment ibuprofenovy with other medicines included in the standard schemes of treatment. Caffeine strengthens analgeziruyushchy effect of an ibuprofen.

Contraindications:

The increased individual sensitivity to drug components, including - to a dimethyl sulfoxide, an ibuprofen, and also to other non-steroidal anti-inflammatory drugs. The becoming wet dermatosis, the eczema infected grazes and wounds, pregnancy, the lactation period.

Overdose:

Overdose cases at use of ointment ibuprofenovy are not described.

At prolonged use in the high doses (exceeding recommended) for extensive sites of defeat development of system side effects of an ibuprofen is possible.
