Diclofenac

General characteristics. Structure:

Active agent: diclofenac of diethyl amine of 11.6 mg that corresponds to the content of diclofenac of sodium of 10 mg

Excipients: isopropanol, carbomer 940, polysorbate 80, benzyl alcohol, sodium disulphite, sorbitol, троламин, the water purified.

Pharmacological properties:

NPVP derivative of phenylacetic acid, has antiinflammatory and analgeziruyushchy effect. Not selectively oppressing cyclooxygenase-1 (TsOG-1) and cyclooxygenase - 2 (TsOG-2), influences metabolism of arachidonic acid, reduces amount of prostaglandins in the inflammation center.

Pharmacokinetics. At topical administration it is partially soaked up through skin, bioavailability - 6%. Communication with proteins of plasma - 99.7%. It is removed by kidneys. At patients with polyarthritis who receive local therapy (in the inflamed joint) concentration in synovial fluid and fabric is higher, than concentration in plasma.

Indications to use:

Inflammatory and degenerative diseases of a musculoskeletal system: a pseudorheumatism, the psoriasis, juvenile chronic arthritis ankylosing a spondylitis; rheumatic defeats of soft tissues; gouty arthritis); traumatic the caused inflammations of sinews, sheaves, muscles and joints (as a result of stretchings, at loading and bruises).

Route of administration and doses:

Outwardly.

To adults and children 12 years drug are more senior put with a thin layer over the center of an inflammation of 3-4 times/days and slightly rub. The necessary amount of drug depends on the size of a painful zone. Single dose of drug: 2-4 g (that on volume is comparable to the size of large cherry).
After putting drug it is necessary to wash up hands.
Duration of treatment depends on indications and medical effect. After 2 weeks of use of drug it is necessary to consult with the doctor.

Features of use:

Gel is applied only on the unimpaired sites of skin. After imposing it is not necessary to apply an occlusive bandage.
When drawing on the big surfaces of skin the risk of development of system side effects increases for a long time.
It is necessary to avoid hit of drug in eyes, a pas mucous membranes of a pla open brines. To apply only as outside means.

Side effects:

From integuments: eczema, a photosensitization, contact dermatitis (an itch, a hyperemia, puffiness of the processed site of skin, papular and vesicular rashes, a peeling).

Allergic reactions: an itch and burning of skin, erythematic skin rash, a small tortoiseshell, a Quincke's disease, system anaphylactic reactions (including shock).

Interaction with other medicines:

Drug can strengthen effect of the drugs causing a photosensitization.
Clinically significant interaction with other medicines is not described.

Contraindications:

— a full or incomplete combination of bronchial asthma, the nose recuring a polypose and okolonosovy bosoms, and intolerance of acetylsalicylic acid or other NPVP (in the anamnesis);
— disturbance of integrity of integuments;
— pregnancy (III trimester);
— lactation period;
— children's age (up to 12 years);
— hypersensitivity to diclofenac or other components of drug to acetylsalicylic acid or other NPVP.

With care
Hepatic porphyria (aggravation), GIT erosive cankers (in an aggravation phase), heavy abnormal liver functions and kidneys, chronic heart failure, disturbance of coagulability of blood (including hemophilia, lengthening of a bleeding time, tendency to bleedings); bronchial asthma, advanced age, pregnancy I and II trimesters.

Use at pregnancy and feeding by a breast
Use at pregnancy (the III trimester) and in the period of a lactation contraindicated.
With care: pregnancy I and II trimesters.

Use at abnormal liver functions
It is contraindicated at heavy abnormal liver functions.

Use at renal failures
It is contraindicated at heavy renal failures.

Use for elderly patients
With care.

Use for children
It is contraindicated to children up to 12 years.

Overdose:

Symptoms: in view of low system absorption at gel application, the overdose is improbable. At accidental intake development of system side reactions is possible.

Treatment: gastric lavage, induction of vomiting, absorbent carbon, artificial diuresis, symptomatic therapy. Dialysis is not effective in view of high extent of linkng with proteins of diclofenac (about 99%).

Storage conditions:

To store in the place, unavailable to children, at a temperature from 15 °C to 25 °C.
