General characteristics. Structure:

Active agent: hydrochloride methyloxadiazol color butylaminohydroxypropoxyphenoxymethyl (проксодолол) 10 g.

Excipients: citric acid — 0,048 g; sodium citrate for injections — 0,526 g; water for injections — to 1 l.

Pharmacological properties:

Pharmacodynamics. Non-selective alpha and beta adrenoblocker; has hypotensive, anti-anginal and antiarrhytmic effect (treats antiaritmika of the II class); reduces ChSS, expands vessels, reduces OPSS and reduces the minute volume of blood.

Due to the blockade of beta adrenoceptors the tone of bronchial tubes raises.

Indications to use:

Hypertensive crisis (stopping).

Route of administration and doses:

In/in struyno or kapelno.

In/in struyno — 10–20 mg (1–2 ml of 1% of solution (10 mg/ml) within 1 min. If necessary repeat introduction at an interval of 5 min. before emergence of effect. The maximum dose — 50–100 mg (5–10 ml of 1% of solution (10 mg/ml).

In/in kapelno — 50 mg (5 ml of 1% of solution (10 mg/ml) in 200 ml isotonic (0,9%) solution of sodium of chloride or in 5% glucose solution, with a speed of 0,5 mg/min. (2 ml of infusion solution) before receiving positive reaction.

Features of use:

At in administration of drug strict control of the ABP, ChSS, ECG is necessary.

Side effects:

At in introduction dizziness, weakness, nausea, a headache, bradycardia, excessive decrease in the ABP, a cold snap of extremities, blockade of legs of a ventriculonector, AV blockade are possible; bronchospasm; dryness in a mouth, a gastralgia, allergic reactions (a local or generalized dieback, a small tortoiseshell), increase in contractility of a myometrium.

Interaction with other medicines:

Simultaneous use with cardiac glycosides, metildopy, Reserpinum and guanfatsiny, blockers of "slow" calcium channels (verapamil, diltiazem), Amiodaronum and other antiarrhytmic means increases risk of development or aggravation of bradycardia and AV of blockade, can cause a cardiac standstill.

Nifedipine, diuretics, clonidine, sympatholytics, гидралазин and other antihypertensives can lead to excessive decrease in the ABP. Extends action of not depolarizing muscle relaxants and anticoagulating effect of coumarins.

At co-administration about three - and tetracyclic antidepressants, antipsychotic means (neuroleptics), ethanol, sedative and hypnagogues — strengthening of oppression of TsNS.

Simultaneous use with MAO inhibitors is followed by considerable strengthening of hypotensive action.

Contraindications:

- hypersensitivity to drug;
- the expressed bradycardia;
- AV blockade of the II-III degree;
- chronic heart failure (in a decompensation stage);
- cardiogenic shock;
- arterial hypotension;
- HOBL;
- bronchial asthma;
- concomitant use of MAO inhibitors;
- pregnancy;
- breastfeeding period;
- age up to 18 years (efficiency and safety are not established).

With care:
- AV blockade of the I degree;
- chronic heart failure;
- diabetes mellitus of type 1 (insulin-dependent);
- peptic ulcer of a stomach and duodenum.

Overdose:

Symptoms: the expressed decrease in the ABP, bradycardia (pass in 10–20 min. after the introduction termination), ventricular premature ventricular contraction, strengthening of symptoms of heart failure, a bronchospasm.

Treatment: to give to the patient situation "with raised head over heels"; oxygenotherapy; at bradycardia — Atropini sulfas or Dobutaminum in/in, it is possible in/in introduction of a glucagon or Epinephrinum, carrying out transvenous electrocardiostimulation.

At the expressed decrease in the ABP — in/in administration of plasma substituting solutions (in the absence of a fluid lungs) and under control of the ABP — vasoconstrictors (Epinephrinum, Norepinephrinum, a dopamine or Dobutaminum); at a bronchospasm — изопреналин or theophylline; at heart failure — cardiac glycosides and/or a glucagon and diuretics; at ventricular premature ventricular contraction — lidocaine or Phenytoinum.

Storage conditions:

In the place protected from light, at a temperature of 5-25 °C.
