General characteristics. Structure:

Active ingredient: Left carnitine (Карнифит) of 300 mg; excipients: citric acid monohydrate of 1,2 mg, methylparahydroxybenzoate of 0,5 mg, пропилпарагидроксибензоат 0,2 mg, the water purified to 1 ml.

Description: the transparent colourless or slightly painted liquid. Existence of a specific smell is allowed.

Pharmacological properties:

Pharmacodynamics. A L-carnitine – natural substance, related to group B vitamins. Participates in processes of a metabolism as a carrier of long-chain fatty acids (palmitic, etc.) from cytoplasm in a mitochondrion where these acids are exposed to process of β-oxidation with formation of adenosine triphosphoric acid and atsetil-KOA. Improves proteinaceous and the lipometabolism, increases secretion and enzymatic activity of gastric and intestinal juice, improves digestion of food, reduces excess body weight and reduces the content of fat in muscles. Increases resistance to exercise stresses, formation of ketonic acids and anaerobic glycolysis oppresses, reduces lactacidemia degree, promotes an economical expenditure of a glycogen and increases its stocks in a liver and muscles. Has anabolic and lipolytic effect. Normalizes the raised standard metabolism at a hyperthyroidism: is a peripheral (indirect) antagonist of effect of thyroid hormones, but not direct inhibitor of function of a thyroid gland. Improves a metabolism and power supply of fabrics.

Pharmacokinetics. It is well soaked up in intestines, concentration in a blood plasma reaches a maximum in 3 hours and remains in the therapeutic range within 9 hours. Easily gets into a liver, a myocardium, more slowly – into muscles. It is removed by kidneys, it is preferential in the form of acylic ethers.

Indications to use:

Drug is used as a part of complex therapy.
• intensive exercise stresses and psychoemotional loadings: for increase in working capacity, endurance, decrease in fatigue, including, at elderly persons;
• during rehabilitation after the postponed diseases and surgical interventions, injuries, including, for acceleration of an angenesis;
• complex therapy of chronic gastritis and chronic pancreatitis with the lowered secretory function;
• complex therapy of skin diseases (psoriasis, seborrheal eczema, focal scleroderma and diskoidny lupus erythematosus);
• hyperthyroidism of easy degree;
• neurologic manifestations at vascular, toxic and traumatic damages of a brain;
• syndrome of nervous anorexia;
• the diseases which are followed by a lack of a carnitine or its increased loss (myopathies, a cardiomyopathy, mitochondrial diseases, hereditary diseases with the accompanying mitochondrial insufficiency) – for completion of its deficit as a part of complex therapy;
• in pediatric practice (treatment of children up to 3 years under observation of the doctor): when nursing premature babies, the newborns who transferred a birth trauma or asphyxia; to children with a sluggish sucking reflex and a low increase of body weight; a reduced muscle tone, at underdevelopment of motive and mental functions, and also for the purpose of prevention of these disturbances at children of "risk group"; at a growth inhibition and a lack of body weight at children and teenagers up to 16 years.
• at adults in sports medicine and at intensive trainings (впериод training process at working off of aerobic productivity): for improvement of high-speed and power indicators and coordination of movements, for increase in muscle bulk and a reduction of fatty body weight, for prevention of a post-training syndrome (acceleration of recovery processes after exercise stresses), at traumatic damages for the purpose of acceleration of regeneration of muscles.

Route of administration and doses:

Drug is used as a part of complex therapy. Inside in 30 min. prior to food, in addition diluting with liquid.

At long exercise and psychoemotional stresses: from 0,75 g (1/2 measured spoons or 2,5 ml) 3 times a day to 2,25 g (1,5 measured spoons or 7,5 ml) 2-3 times a day.

At nervous anorexia, and also during rehabilitation after the postponed diseases and surgical interventions and injuries: on 1,5 g (1 measured spoon or 5 ml) 2 times a day. A course of treatment - within 1-2 months.

In complex therapy of chronic gastritis and chronic pancreatitis with the lowered secretory function: on 0,375 g (1/4 measured spoons or 1,25 ml) 2 times a day. A course of treatment - within 1-1,5 months.

For treatment of skin diseases: on 0,75 g (1/2 measured spoons or 2,5 ml). A course of treatment - within 2-4 weeks.
At a hyperthyroidism of easy degree: on 0,25 g (13 drops) 2-3 times a day. A course of treatment – 20 days. The course of treatment is repeated after 1-2 monthly breaks or appointed within 3 months without interruption.
At vascular, toxic and traumatic damages of a brain: on 0,75 g (1/2 measured spoons or 2,5 ml) in days. A course of treatment - within 3-5 days. If necessary in 12-14 days appoint a repeated course.
At the diseases which are followed by a lack of a carnitine (primary and secondary karnitinovy insufficiency): to 50-100 mg/kg (2-5 drops/kg) of body weight with frequency rate of reception 2-3 times a day. A course of treatment - within 3-4 months.
To children appoint in the form of additive to sweet dishes (kissel, compote, juice).
To children up to 3 years the dose is defined by the attending physician. From 3 to 6 years – in a single dose of 0,1 g (5 drops) 2-3 times a day, in a daily dose of 0,2-0,3 g (11-16 drops). A course of treatment – 1 month. To children from 6 to 12 years appoint in a single dose 0,2-0,3 g (11-16 drops) 2-3 times a day, in a daily dose of 0,4-0,9 g (22-48 drops). A course of treatment – not less than 1 month. 
At a growth inhibition: on 0,25 g (13 drops) 2-3 times a day. A course of treatment – 20 days. The course of treatment is repeated after 1-2 monthly breaks or appointed within 3 months without interruption.
In sports medicine and at intensive trainings: on 2,5 g 1-3 times a day (a daily dose of 2,5 - 7,5 g); in case of use with the medical purpose – 70-100 mg/kg/days (5-7,5 g/days). Reception courses: 3-4 weeks during the precompetitive period. During training process – up to 6-8 weeks.

Features of use:

Aged up to 3 years to perform treatment of children under observation of the doctor. Drug does not exert impact on performance of potentially dangerous types of activity requiring special attention and bystry reactions (driving and other vehicles, work with moving mechanisms, work of the dispatcher and operator, etc.).

Side effects:

Allergic reactions, gastralgia, dyspepsia, myasthenia are possible (at patients with uraemia).

Interaction with other medicines:

Glucocorticosteroids promote accumulation of drug in fabrics (except a liver), other anabolic steroids strengthen effect.

Contraindications:

Individual intolerance.

Use at pregnancy and in the period of a lactation

Due to the lack of researches use at pregnancy and in the period of a lactation is not recommended.

Overdose:

Symptoms: dispepsichesky frustration, myasthenic frustration (at patients with uraemia). Treatment: gastric lavage, reception of absorbent carbon.

Storage conditions:

In protected from light and the place, unavailable to children, at a temperature not above 25 °C. After opening – in the refrigerator within 2 months. Period of validity 3 years. Not to use after the termination of a period of validity.

Solution for intake of 300 mg/ml.

100 ml glass bottle. On 100 ml in the bottles of dark glass corked by covers with a control ring of the first opening. The measured glass of 20 ml is put on a bottle. Each bottle together with the application instruction is placed in a pack from a cardboard. 

50 ml glass bottle. On 50 ml in the bottles of dark glass corked by covers with droppers with a control ring of the first opening. Each bottle together with a measured spoon with a nominal volume of 5 ml with risks with marking "¼" and "½" (that there correspond 1,25 ml and 2,5 ml), with the application instruction is placed in a pack from a cardboard.     

25 ml glass bottle. On 25 ml in the bottles of dark glass corked by covers with droppers with a control ring of the first opening. Each bottle together with the application instruction is placed in a pack from a cardboard. 

25 ml plastic bottle. On 25 ml in bottles plastic, corked by covers with a control ring of the first opening. 10 or 20 plastic bottles together with the application instruction place in a pack with partitions from a cardboard.
