Carnitine chloride

General characteristics. Structure:

Active ingredient: 100 mg of a carnitine of chloride in 1 ml of solution.

Excipients: water for injections.

Drug which renders the anabolic, anti-hypoxemic, anti-thyroid, stimulating a lipometabolism, regenerating action.

Pharmacological properties:

Pharmacodynamics. A carnitine chloride belongs to the drugs stimulating metabolic processes. Treats group of vitamins B (vitamin of W — growth vitamin).

Carnitine chloride participates in various links of energy balance, influences lipidic exchange. Drug recovers an alkaline reserve of blood, does not influence coagulant system of blood, reduces formation of ketonic acids, increases resistance of fabrics to influence of toxic decomposition products, activates aerobic processes and anaerobic glycolysis oppresses, stimulates and accelerates reparative processes.

Has zhiromobilizuyushchy effect. Competitively forcing out glucose, turns on the zhirnokislotny metabolic shunt which activity is not limited by oxygen (unlike aerobic glycolysis) therefore it is effective at an acute hypoxia of a brain and at other critical states. Possesses neurotrophic action. Slows down development of apoptosis. Limits a zone of defeat and recovers structure of nervous tissue.

Increases secretion and enzymatic activity of digestive juices (gastric and intestinal), improves digestion of food. Reduces excess body weight and reduces the content of fat in muscles.

Raises a resistance threshold to an exercise stress, leads to elimination of postload acidosis and, as a result, maintenance after the long exhausting loadings. Increases glycogen stocks in a liver and muscles, promotes its more economical use.

Pharmacokinetics. In the conditions of intravenous administration 3 hours later after use it is not found in blood. Easily gets into a liver, a myocardium, more slowly — into muscles.

It is removed by kidneys preferential in the form of acylic ethers.

Indications to use:

Carnitine chloride is applied at acute disorders of cerebral circulation — an ischemic stroke, the tranzitorny ischemic attack. Appoint drug in the acute, subacute and recovery periods. Apply at distsirkulyatorny encephalopathy and various traumatic and toxic damages of a brain.

Carnitine chloride is effective at primary (genetic) and secondary deficit of a carnitine, including at the patients with a chronic renal failure who are on a hemodialysis; at a cardiomyopathy, myocarditis, an ischemic heart disease (stenocardia, an acute myocardial infarction, postinfarction states), hypoperfusion owing to cardiogenic shock. Purpose of drug as a cardioprotector at treatment is shown by cytostatics, especially anthracycline row.

Carnitine chloride is applied also at anorexia, a hypotrophy, a growth inhibition at children.

Medicine is appointed independently or in complex therapy.

Route of administration and doses:

Medicine is entered intravenously kapelno slowly (no more than 60 drops a minute!). Before introduction each 100 mg of drug (1 ml of 10% of solution) dissolve in 50 ml of isotonic solution of sodium of chloride or 5% of solution of glucose.

In the acute period of a disease in the first 3 days enter 10 — 14 mg/kg of body weight of the patient, in the next days — on 7 mg/kg of body weight. General course of treatment of 7 — 10 days. If necessary in 10 — 12 days conduct a repeated course on 7 mg/kg of weight within 3 — 5 days.

At purpose of a carnitine in the subacute and recovery periods of distsirkulyatorny encephalopathy and various damages of a brain by the patient enter 0,5 — 1,0 g of a carnitine of chloride within 3 — 5 days once a day. If necessary in 12 — 14 days appoint a repeated course.

Features of use:

Medicine is applied with care at patients with burdened allergoanamnezy. As the carnitine improves digestion of glucose, it is necessary to control its level in blood serum at patients with a diabetes mellitus.

Pregnancy and lactation. Special researches on studying of a possibility of use at pregnancy and feeding by a breast were not conducted. At appointment during these periods it is necessary to compare carefully advantage for mother and potential risk for a fruit or the child. It is considered that deficit of a carnitine during pregnancy represents significantly bigger risk for mother, than theoretical risk for a fruit. The carnitine is a natural component of breast milk, however was not conducted researches of efficiency and safety of use of a carnitine in the period of a lactation.

Features of influence of medicine on ability to manage the vehicle or potentially dangerous mechanisms. Persons who note muscular weakness after use of a carnitine have to refrain from driving and potentially dangerous mechanisms.

Side effects:

At purpose of a carnitine allergic reactions are possible. In case of bystry introduction (80 drops more in a minute) emergence of pains on the course of the veins passing at reduction in the rate of introduction is possible.

Interaction with other medicines:

Glucocorticoids promote accumulation of a carnitine in fabrics (except a liver), other anabolic steroids strengthen effect of drug.

Contraindications:

The increased individual sensitivity to medicine.

With care: pregnancy and the period of a lactation (due to the lack of data).

Overdose:

Symptoms: strengthening of expressiveness of manifestations of side effect.

Treatment: drug withdrawal, symptomatic therapy.

Storage conditions:

In the place protected from light at a temperature not above 25 °C. To store in the place, unavailable to children. Period of validity 2 years. Not to use after the period of validity specified on packaging.
