General characteristics. Structure:

Active ingredient: пентоксифиллин; 1 tablet contains a pentoksifillin (in terms of 100% substance) 100 mg; excipients: lactose (sugar milk), potato starch, magnesium stearate, gipromelloz, povidone 25, колликоат MAY 30 DP, propylene glycol, talc, titanium E 171 dioxide, кармоисин E 122.

Pharmacological properties:

Drug improves microcirculation in vessels and a condition of blood due to expansion of blood vessels, reduction of aggregation of thrombocytes, increase in elasticity of erythrocytes and decrease in viscosity of blood. Drug improves providing fabrics with oxygen.
Pentoksifillin is quickly soaked up in a digestive tract. At the first passing through a liver a number pharmacological of active metabolites is formed. The maximum concentration in a blood plasma is reached in 1 h after intake and considerably decreases by the 8th o'clock – less than 10% of the entered dose. The most part (about 94%) is removed with urine in the form of non-toxic metabolites. Less than 4% of the accepted dose are removed with a stake.
At patients with heavy renal failures excretion of metabolites is slowed down. At patients with the broken function of a liver the elimination half-life increases.

Indications to use:

Atherosclerosis of brain vessels, ischemia, state after the postponed myocardial infarction; diabetic nefroangiopatiya, diabetic angiopatiya, disturbances of peripheric circulation (Raynaud's disease, endarteritis); acute and chronic insufficiency of blood supply of a retina and choroid; functional hearing disorder.

Route of administration and doses:

The initial dose for adults makes 200 mg of Pentoksifillin three times a day within the first week of treatment. In case of hypotension, negative reactions from digestive tract or the central nervous system, the initial dose can be lowered to 100 mg of Pentoksifillin three times a day. Patients with chronic diseases usually accept 100 mg of Pentoksifillin three times a day.
For subjects with renal disturbances the reduced doses (50-70% of a usual dose), creatinine level> 400 mmol/l are recommended.
It is not necessary to exceed the maximum daily dose of 1200 mg.
Pill should be taken whole with a small amount of liquid in time or after food, best of all at the same time.

Features of use:

With care to apply to patients with the atherosclerosis of coronary and brain vessels which is followed by arterial hypertension, disturbances of a cordial rhythm, stenocardia and also to patients with arterial hypotension.

Side effects:

Nausea, vomiting, pain and heavy feeling in epigastric area, diarrhea, a headache, dizziness, tachycardia, stenocardia attacks, arterial hypotension, a dermahemia of the person, occasionally – a skin itch, a small tortoiseshell, a Quincke's disease, recent bleedings, vision disorders, arrhythmias, a grippopodobny syndrome, a nose congestion, anorexia, an intestines atony, increase in activity of liver enzymes, a leukopenia, thrombocytopenia, an exacerbation of cholecystitis are possible.

Interaction with other medicines:

Can strengthen action of anti-hypertensive means and strengthen hypotensive effect at a combination with sympatholytics, ganglioblokator, peripheral vazodilatator. Pentoksifillin increases glikemiziruyushchy effect of insulin and peroral antidiabetic means, and also exponentiates effect of heparin and fibrinolitic drugs.

Contraindications:

Acute myocardial infarction, bleeding, the expressed atherosclerosis of coronary vessels and vessels of a brain, hemorrhagic stroke, eye retinal apoplexy, hypersensitivity to components of drug and derivatives of xanthines (caffeine, theophylline, etc.). Pregnancy, feeding period breast, children's age.

Overdose:

Symptoms of overdose appear in 4 - 5 hours after drug use. The dermahemia of the person, arterial hypotension, kloniko-tonic spasms, a loss of consciousness, vomiting of color of a coffee thick, fever are possible. Along with symptomatic therapy and a gastric lavage special attention is paid to control and maintenance of function of breath, arterial pressure and elimination of spasms.
