General characteristics. Structure:

Active agent: trimetazidina dihydrochloride of 20 mg;
excipients: cellulose of microcrystallic 20,0 mg, Mannitolum of 44,5 mg, кополивидон 4,0 mg, silicon dioxide of colloid 0,5 mg, magnesium stearate of 1,0 mg, a gipromelloza of 2,3 mg, a macrogoal 6000 (polyethyleneglycols) of 0,23 mg, dye - ferrous oxide red (Е 172) 0,1 mg, titanium dioxide (Е 171)0,49 mg.

Description: round, biconvex tablets, film coated red-brown color.

Pharmacological properties:

Pharmacodynamics. Trimetazidin has anti-anginal, anti-hypoxemic effect.
Directly influencing cardiomyocytes and neurons of a brain, optimizes their metabolism and function. The cytoprotective effect is caused by increase in energy potential, activation of oxidizing decarboxylation and rationalization of consumption of oxygen (strengthening of aerobic glycolysis and blockade of oxidation of fatty acids). Maintains contractility of a myocardium, prevents intracellular exhaustion of ATP and phosphocreatinine. In the conditions of acidosis normalizes functioning of ion channels, interferes with accumulation of calcium and sodium in cardiomyocytes, normalizes the intracellular maintenance of potassium ions.
Reduces the intracellular acidosis and content of phosphates caused by ischemia of a myocardium and reperfusion.
Interferes with the damaging action of free radicals, keeps integrity of cellular membranes, prevents activation of neutrophils in an ischemia zone, increases duration of electric potential, reduces a kreatinfosfokinaza exit from cells and expressiveness of ischemic injuries of a myocardium.
At stenocardia reduces the frequency of attacks (consumption of nitrates decreases), in 2 weeks of treatment tolerance to an exercise stress increases, differences of arterial pressure without any significant changes of cordial rhythms decrease. Improves hearing and results of vestibular tests at patients with pathology of ENT organs, reduces dizziness and a sonitus. At vascular pathology of eyes recovers functional activity of a retina of an eye.

Pharmacokinetics. After intake триметазидин it is quickly absorbed, Cmax is reached less than in 2 hours and makes 55 ng/ml after a single dose in a dose of 20 mg. At repeated use the equilibrium state is reached in 24-36 hours and remains stable until the end of a therapy course. Easily passes through gistogematichesky barriers. Linkng with proteins of plasma low, about 16% of in vitro.
It is brought out of an organism, generally by kidneys (60%) in not changed look. T1/2 - 4,5-5 hours.

Indications to use:

- long therapy of coronary heart disease: prevention of attacks of stenocardia (monotherapy or as a part of a combination therapy);
- treatment cochleovestibular narusheniyishemichesky nature (such, as: dizziness, sonitus, hearing disorder);
- chorioretinal vascular disorders with an ischemic component.

Route of administration and doses:

Drug is accepted during food.
Appoint 40-60 mg/days in a dose. Frequency rate of reception 2-3 times/days. Duration of therapy is established individually, depending on a clinical situation.

Features of use:

It is not applied to stopping of attacks of stenocardia!
Against the background of treatment by drug the patient with coronary heart disease has an essential reduction of daily need for nitrates.

Use of drug does not influence ability to driving and performance of work, the mental and physical reactions demanding high speed.

Side effects:

From the alimentary system: seldom - nausea, vomiting.
Allergic reactions: skin rash, an itch and other manifestations are possible.

Interaction with other medicines:

Now cases of medicinal interaction are not described.

Contraindications:

• hypersensitivity to drug;
• a renal failure (the clearance of creatinine is lower than 15 ml/min.);
• the expressed abnormal liver functions;
• pregnancy;
• breastfeeding;
• age up to 18 years (efficiency and safety are not established).

Use during pregnancy and during feeding by a breast
It is contraindicated at pregnancy due to the lack of clinical data on safety of its use.
In need of purpose of drug in the period of a lactation it is necessary to stop breastfeeding. It is unknown whether it is allocated триметазидин with breast milk.
In pilot studies teratogenic action of a trimetazidin is not established.

Overdose:

Now about overdose cases by drug it was not reported.

Storage conditions:

At a temperature not above 25 °C. To store in the place, unavailable to children. Period of validity 3 years. Not to apply after expiry date!
