General characteristics. Structure:

Active ingredient: 1 tablet contains 20 mg of a trimetazidin of dihydrochloride (in recalculation for 100% dry matter);

excipients: starch corn, lactoses monohydrate, cellulose microcrystallic, silicon dioxide colloid anhydrous, talc, calcium stearate, сепифилм 752 white.

Pharmacological properties:

Pharmacodynamics. Anti-ischemic, anti-anginal means. Trimetazidin provides normalization of power balance in cells at a hypoxia and ischemia, preventing decrease in intracellular content of ATP. Supports a cellular homeostasis, providing normal functioning of ion channels of a membrane and transmembrane transport of potassium ions. The mechanism of action is based on an ingibition dlinnotsepochny 3 SOA ketoacyl thiomanholes (3-KAT) that causes partial oppression of oxidation of fatty acids and leads to partial switching of energy balance from oxidation fat acid on glucose oxidation that is more favorable at ischemia. Trimetazidin increases exchange of phospholipids and their inclusion in membranes, than provides protection of membranes against damages.

At patients with a stenoyokardiya триметазидин improves energy balance in heart in the conditions of a hypoxia. From the 15th day of treatment increases a coronary reserve, increases tolerance to exercise stresses, without influencing the level of the arterial pressure (AP) and the heart rate (HR), considerably reduces the frequency of attacks of stenocardia, significantly reduces need of use of nitrates.

 In otorhinolaryngological practice use of a trimetazidin promotes vestibular compensation, improves portability of vestibular tests, increases auditory acuity, ustrayonyat a sonitus and dizziness at Menyer's disease and diseases of sosuyod of a brain.

In ophthalmology триметазидин increases functional activity of a retina that is shown by normalization of indicators of the electroretinogram, improves visual acuity and a field of vision.

Pharmacokinetics. Drug is completely and quickly absorbed from a digestive tract. Time of achievement of the maximum concentration in a blood plasma (Tmax)         less than 2 hours. The maximum concentration (Cmax) makes 55 ng/ml after one-time reception in a dose of 20 mg. Stable concentration is established approximately in 24-36 hours after repeated receptions and remains such throughout the entire period of treatment.

About 16% contact proteins of in vitro. It is well distributed in fabrics. The volume of distribution makes 4,8 l/kg.

It is allocated generally with kidneys, generally in not changed look. An elimination half-life (T1/2) – about 6 hours.

 

Pharmaceutical characteristics.

Main physical and chemical properties: tablets of white color, coated.

Indications to use:

Cardiology: prolonged treatment of coronary heart disease, prevention of attacks of stenocardia (monotherapy or in a combination with other medicines).

Otorhinolaryngology: treatment of cochleovestibular disturbances of an ischemic origin, such as dizziness, sonitus, decrease in hearing.

Ophthalmology: the chorioretinal frustration caused by ischemia.

Route of administration and doses:

Drug is appointed inside on 3 tablets a day in 3 inclusion in food time, washing down with a glass of water. Duration of treatment depends on weight and the course of a disease. Drug use to elderly people does not demand dosing change.

Features of use:

Use during pregnancy or feeding by a breast.

In view of lack of sufficient clinical data, it is not recommended to appoint drug during pregnancy.

In case of need uses of drug in the period of a lactation stop breastfeeding because ability of drug to get into breast milk is not studied.

Children.

Experience of use of drug in children's age is absent therefore it is not recommended to use drug at children.

Энерготон® it is applied to basic therapy of stenocardia, but not to stopping of attacks of stenocardia.

At a renal failure it is recommended to carry out monitoring of indicators of function of kidneys; if necessary the dose can be lowered.

Patients of old age and patients with a diabetes mellitus do not need correction of a dose.

Surgical intervention: does not demand special cautions before carrying out anesthesia.

Ability to influence speed of response at control of motor transport or work with other mechanisms.

Side effects:

At use of drug there can be following side reactions from such systems and bodies:

from the central and peripheral nervous system: a headache, dizziness, development of extrapyramidal symptoms (muscular weakness, muscle tension, a tremor, an akineziya), especially at patients with parkinsonism is very seldom possible.

From digestive tract: pain in the epigastric site, nausea, vomiting, diarrhea, dyspepsia.

From cardiovascular system: orthostatic hypotension, bradycardia, dermahemia of the person.

Allergic reactions: skin rash, itch, small tortoiseshell, Quincke's edema.

From an organism in general: adynamy.

Interaction with other medicines:

It is not revealed.

Contraindications:

The increased individual sensitivity to drug. It is not recommended to appoint drug during pregnancy and feeding a breast.

Overdose:

It happens very seldom. In cases of strong overdose considerable decrease in resistance of peripheral vessels can be observed, and, as a result, develop arterial hypotension and a dermahemia of the person.

Treatment. Symptomatic treatment. There is no specific antidote.

Storage conditions:

Period of validity. 2 years. Store in the place, unavailable to children, in original packaging, at a temperature not above 25 °C.
