Vinpocetine

General characteristics. Structure:

Acting substance: 5 mg of a Vinpocetine.

Excipients: lactose, starch, magnesium stearate, aerosil.

Drug with the expressed vascular and metabolic action.

Pharmacological properties:

Pharmacodynamics. Active ingredient of the Vinpocetine is the Vinpocetine - semi-synthetic derivative alkaloid of Devincanum which contains in a periwinkle small. Combines vascular and metabolic action. Drug inhibits activity of enzyme of phosphodiesterase and increases the maintenance of tsAMF in cells of unstriated muscles of vessels of a brain, reduces the content of calcium in cytoplasm of smooth muscle cells of vessels. Thanks to it, under the influence of drug brain vessels extend, cerebral circulation improves, it is preferential in ischemic sites, supply of a brain with oxygen improves.

The Vinpocetine promotes utilization of glucose and increases the level of catecholamines in the central nervous system, stimulates metabolism of noradrenaline and serotonin in brain tissues. Reduces aggregation of thrombocytes, viscosity of blood, contributes to normalization of venous outflow against the background of decrease in vascular resistance of a brain.

Under the influence of drug system arterial pressure decreases in insignificant degree. In the acute period of an ischemic stroke drug accelerates regress of all-brain and focal symptomatology, improves memory, attention, intellectual productivity. In old and senile age sensitivity of vessels of a brain to action of a Vinpocetine increases that is caused by a sensitization when aging system of adenylatecyclase-tsAMF.

Pharmacokinetics. The Vinpocetine is quickly soaked up in a digestive tract. Bioavailability makes about 60%. Optimum concentration in plasma makes 10-20 ng/ml. The maximum concentration is reached in 1 hour. Drug easily gets through gistogematichesky barriers, including a blood-brain barrier, and gets into fabrics. Indications to use:

• Insufficiency of cerebral circulation (tranzitorny ischemia, the progressing stroke, a complete stroke, states after a stroke).
• Neurologic and mental disturbances at patients with cerebrovascular insufficiency (memory disturbance, dizziness, aphasia, apraxia, motive frustration, a headache).
• Encephalopathy (hypertensive, posttraumatic).
• Vascular diseases of eyes (degenerative diseases of a choroid, retina or macula lutea, partial occlusions of arteries, secondary glaucoma).
• Decrease in auditory acuity of vascular or toxic genesis, senile relative deafness, Menyer's disease, kokhleovestibulyarny neuritis, sonitus, dizzinesses of a labyrinth origin.

Route of administration and doses:

The Vinpocetine is accepted inside on 1-2 tablets by 3 times a day, a maintenance dose – 3 times a day on 1 tablet. Apply it is long. Improvement is observed usually in 1-2 weeks; course of treatment about 2 months and more.

Features of use:

With care appoint to patients with severe forms of arrhythmias, with stenocardia.

Side effects:

The lowering of arterial pressure, is more rare – tachycardia, premature ventricular contraction. Increase in time of excitement of ventricles is possible.

Interaction with other medicines:

At simultaneous use of a Vinpocetine and heparin increase in risk of development of bleeding is possible.

Contraindications:

Heavy ischemic heart diseases, heavy arrhythmias, pregnancy, lactation, hypersensitivity to drug.

It is not recommended to appoint drug at the labile ABP and a low vascular tone.

Overdose:

Treatment: gastric lavage, purpose of absorbent carbon, symptomatic therapy.
