Description:
Hypogonadism (men's) — the morbid condition caused by insufficient secretion of androgens and/or insufficiency of a gametogenesis.
Hypogonadism symptoms:
Clinical manifestations of a hypogonadism depend on age at which there was a disease, and degrees of androgenic insufficiency. Distinguish dopubertatny and post-pubertal forms of a hypogonadism. At defeat of testicles before puberty the typical eunuchoid syndrome develops, lengthening of extremities, an underdevelopment of a thorax and a shoulder girdle are noted high disproportionate growth owing to delay of ossification of epiphyseal regions of growth. Skeletal muscles are developed poorly, the hypodermic fatty tissue is distributed on female type. The true gynecomastia is frequent. Integuments are pale. Poor development of secondary sexual characteristics:
lack of pilosis on a face and a body (on a pubis — on female type)
throat underdevelopment
high pitched voice
Generative organs are underdeveloped:
penis of the small sizes
the scrotum is created, but depigmented, without skladchatost
gipoplazirovana testicles
the prostate is underdeveloped, quite often is not defined palpatorno
At a secondary hypogonadism, except symptoms of androgenic insufficiency, obesity is often observed, symptoms of hypofunction of other hemadens — thyroid, bark of adrenal glands (result of loss of tropny hormones of a hypophysis) are frequent. The panhypopituitarism symptomatology can be observed. Sexual desire and a potentiality are absent.
If loss of function of testicles happened after puberty when sexual development and formation of kostnomyshechny system are already finished, symptoms of a disease are expressed less. Reduction of testicles, decrease in pilosis of the person and body, thinning of skin and loss of its elasticity, development of obesity in female type, disturbance of sexual functions, infertility, vegetovascular frustration are characteristic.
Hypogonadism reasons:
Inborn underdevelopment of gonads, toxic, infectious, their radiation injury, dysfunction of gipotalamo-pituitary system.
Decrease in secretion of sex hormones is observed by testicles. At primary hypogonadism directly testicular fabric is surprised, at secondary — hypofunction of gonads arises owing to defeat of gipotalamo-pituitary system with decrease in gonadotropic function of a hypophysis.
Treatment of the Hypogonadism:
Basis of treatment of a hypogonadism (age androgenodefitsit) is normalization of level of testosterone in blood serum. Performing replacement therapy by testosteronsoderzhashchy drugs and performing the stimulating therapy directed to stimulation of synthesis of own testosterone is possible.
The main drugs for performing androgenzamestitelny therapy at men are drugs of natural testosterone. The replacement therapy by testosterone (RTT) has several modifications in the form of oral, injection and transdermalny administration. The decision on a type of treatment, on its duration and dosages is accepted by the experienced doctor, taking into account features of the specific patient, his age, and also clinical symptomatology.
