Yohimbine hydrochloride of 5 mg No. 50

General characteristics. Structure:

International and chemical names: yohimbin, 17a-гидрокси-20a-йохимбан-16-b-карбоновой acids, methylether, hydrochloride; main physical and chemical properties: tablets of color, white or white with a yellowish shade, with a facet; structure: 1 tablet contains yohimbine of a hydrochloride 5 mg; excipients: potato starch, sugar milk (lactose), calcium stearate.

Pharmacological properties:

Pharmacodynamics. The selection blocker of a2-adrenoceptors. Increases a flow of a sympathetic impulsation from TsNS and release of noradrenaline from nerve terminations, having thus sympathomimetic effect. Increases physical activity. It is experimentally established that yohimbine normalizes the sexual function and ability to a coitus weakened as a result of a stress eliminates the braking of an erection caused by stimulation of a2-adrenoceptors exerts the stimulating impact on TsNS.

Pharmacokinetics. It is completely absorbed in a digestive tract in 45–60 min. The maximum concentration in blood is reached in 45–75 min. At oral administration of drug in the recommended doses of cumulation of drug does not occur. It is evenly distributed in fabrics. In plasma of 82% of yohimbine contacts protein. It is metabolized in a liver and out of it. The yohimbine elimination half-life after use of a single dose makes from 0,25 to 2,5 h. 

Indications to use:

Yohimbine is applied as the means strengthening a potentiality (aphrodisiac) to treatment of mental and functional forms of impotence.

Route of administration and doses:

Inside, without chewing, during food, washing down with a glass of water. The recommended dose – 1–2 tablets 1–3 times a day. It is reasonable to treat with duration 3–4 weeks. If necessary, the course can be repeated.

Features of use:

It is necessary to appoint with care drug to patients with arterial hypertension, heart diseases (coronary heart disease, tachyarrhythmias), a peptic ulcer of a stomach and duodenum, mental diseases, glaucoma.
Simultaneous use of drug with alcohol can negatively affect ability to drive the car and work with potentially dangerous mechanisms.

Side effects:

Increase in arterial pressure, increase in heart rate, sleep disorder, emergence of nervousness, tremor, perspiration, erubescence and headache is possible. There can sometimes be gastrointestinal symptoms, such as nausea and vomiting, appetite loss, diarrhea, in rare instances – disturbances of regulation on hypotonic type.

Interaction with other medicines:

Co-administration of a clonidine as effects of drugs are mutually neutralized is not shown. Yohimbine can change activity of anti-hypertensive means, potentsiirovat effect of antidepressants and increase risk of emergence of their side effects, reduce activity of an alprozalam. Klopiramin increases yohimbine level in a blood plasma.

Contraindications:

Arterial hypotension, hypersensitivity to drug. Existence of a serious illness of a liver and kidneys, treatment of an adrenomimetikama. It is not recommended to use drug in cases of danger of sudden decrease in the ABP with reflex tachycardia, dysfunctions of kidneys and a liver. 

Overdose:

Symptoms: intoxication is shown by the general weakness, generalized paresthesia, disturbances of memory and coordination, a severe headache, dizziness, a tremor, tachycardia, arterial hypertension and sensation of fear, substantial increase of level of noradrenaline in a blood plasma, nausea, vomiting, a mydriasis, salivary and dacryagogue, the increased perspiration.
Treatment: as an antidote at overdose it is possible to apply a clonidine which immediately eliminates clinical displays of intoxication. Appoint inside in a dose 0,1–0,2 mg, then repeatedly with hour intervals in a dose of 0,1 mg before full normalization of a state. Hypertensive crisis is stopped purpose of a combination of a blocker of β-adrenoceptors with a blocker of α-adrenoceptors or a peripheral vazodilatator. For elimination of sensation of fear or hallucinations appoint benzodiazepines. It is impossible to appoint neuroleptics of a fenotiazinovy row.

Storage conditions:

In dry, protected from light and the place, unavailable to children, at a temperature not above 25 °C. 
Period of validity - 3 years.
