Description:
Gastrectomy — the surgical intervention meaning total (full) removal of a stomach with imposing of an esophageal and intestinal anastomosis (anastomosis).
The main indication to operation is the carcinoma of the stomach.
Technology of operation. On ligaments of a stomach the clips feeding it vessels are imposed alloyed. The body is cut from ligaments, a gullet and a duodenum. After removal of a stomach together with big and an omentulum, ligaments, regional lymph nodes between a gullet and a jejunum in 30 — 40 cm from a duodenal and jejunal bend the esophageal and enteric anastomosis on the "end sideways" type is imposed. The loop of a jejunum is carried out to an opening of a mesentery of a cross colon and connected to a gullet a two-row intestinal seam with additional imposing of an enteroenteroanastomoz between bringing and taking away jejunum loops.
Complications. The most frequent complication of a gastrectomy — insufficiency of the seams of an esophageal and intestinal anastomosis imposed on the gullet walls which are not covered with a peritoneum.
