Pyrantelum suspension

General characteristics. Structure:

Active ingredient: pyrantel; 5 ml of suspension contain Pyrantelum of a pamoat is equivalent to Pyrantelum of 250 mg.

Excipients: xanthane gum, sucrose, citric acid, monohydrate; sodium citrate, glycerin, sodium methylparahydroxybenzoate (Е 219), sodium пропилпарагидроксибензоат (Е 217), polysorbate 80, flavoring additive pineapple, the water purified.

Pharmacological properties:

Pharmacodynamics. Pyrantelum is anthelmintic means which acts on Enterobius vermicularis, Ascaris lumbricoides, Ankylostoma duodenale and Necator americanus. Pyrantelum blocks neuromuscular system, paralyseing helminths owing to what they under the influence of a vermicular movement are brought from an organism with a stake. Pyrantelum active against sensitive puberal and immature forms. Larvae which migrate through fabrics are not surprised.

Pharmacokinetics. Intra intestinal absorption of drug very weak owing to insolubility of Pyrantelum of a pamoat. Plasmatic concentration insignificant: from 0,05 mg/ml to 0,13 mg/ml. The main absorption which is estimated by indicators of not changed substance and urine metabolites, is lower than 3,8%. The main metabolite is K-metil-1,3-propandiamin which concentration in urine makes 1,2-4,6%. Over 93% of drug are removed with excrements in not changed look.

Indications to use:

Enterobiosis, ascaridosis, анкилостомидоз.

Route of administration and doses:

Before use to stir up.
To accept inside.
At an enterobiosis, an ascaridosis the average dose makes 10–12 mg/kg of body weight on one reception, that is:
- to children age of 6 months – 2,5 ml of suspension on 10 kg of body weight on one reception;
- the adult with body weight to 75 kg – 15 ml on 1 reception;
- at body weight over 75 kg – 20 ml on 1 reception.
At treatment of an enterobiosis for the purpose of improvement of effect accurate observance of rules of personal hygiene (especially to children) and a repeated course of treatment in 3 weeks after previous is recommended.
At an ankilostomidoza in case of a heavy invasion or in local zones appoint in a dose 20 mg/kg a day in 1–2 receptions for 2–3 days, that is:
- to children age of 6 months – 5 ml of suspension on 10 kg of body weight a day;
- the adult with body weight to 75 kg – 30 ml a day;
- the adult with body weight over 75 kg – 40 ml a day.
In case of a soft invasion of Ankylostoma duodenale (which arises in not local zones) there can be enough dose of 10 mg/kg of body weight once.
It is more reasonable to adult to appoint drug in the form of tablets.

Features of use:

It is necessary to appoint Pyrantelum with care the patient with abnormal liver functions. Pyrantelum is always accepted once, irrespective of meal and time of day; there is no need to apply purgatives or to abstain from meal.
Drug should be accepted at the same time to all family members.
For prevention of repeated infection accurate observance of rules of personal hygiene is recommended.

Ability to influence speed of response at control of motor transport or work with other mechanisms
Dizziness, drowsiness and weakness which are occasionally observed at treatment by Pyrantelum can influence speed of response at control of motor transport or work with other mechanisms.

Use during pregnancy or feeding by a breast.
In pilot studies teratogenic influence of Pyrantelum on a fruit was not revealed. In clinical practice there were no cases of emergence of any side effects at treatment by drug of pregnant women. During pregnancy or feeding by a breast drug is used according to accurate indications after careful assessment of a ratio advantage for woman/risk for a fruit (child) which is defined by the doctor, and strictly adhere to the recommended doses.

Children.
Drug is not used to children age up to 6 months.

Side effects:

- From a nervous system: dizziness, drowsiness, sleeplessness, headache;
- from skin and hypodermic cellulose: skin rashes, urticaria;
- from a digestive tract: anorexia, vomiting, spasms in a stomach, diarrhea, tenesmus;
gepatobiliarny frustration: increase in level of liver enzymes;
- others: weakness, increased fatigue.

Interaction with other medicines:

Not to use drug together with levamisole as at simultaneous use potentiation of toxic effect of levamisole and increase in level of theophylline in a blood plasma is possible. Not to apply with piperazin which is an antagonist of protivogelmintny action of Pyrantelum.

Contraindications:

The increased individual sensitivity to Pyrantelum and other components of drug. Liver failure. Pregnancy. Children's age up to 6 months.

Overdose:

Symptoms: anorexia, nausea, vomiting, diarrhea, ataxy. However, in connection with the minimum adsorbed quantities of Pyrantelum, even at considerable overdose seldom there are symptoms of intoxication.
Treatment: a gastric lavage due to the lack of specific antidote.
Precautionary measures: control of respiratory and cardiovascular functions.

Storage conditions:

To store at a temperature not above 25 °C in protected from light and the place, unavailable to children.
After the first opening of a bottle to store drug no more than 4 weeks.
