Pyrantelum

General characteristics. Structure:

Active ingredient: 720 mg of Pyrantelum of a pamoat (it is equivalent to the content of 250 mg of Pyrantelum).

Excipients: potato starch, povidone (коллидон 30), gelatin, talc, magnesium stearate.

Pharmacological properties:

Pharmacodynamics. Pyrantelum causes neuro and muscular blockade in helminths, sensitive to it, that facilitates their removal from a digestive tract even without additional use of purgatives.

Drug affects helminths in an early phase of development and on their mature forms, does not affect larvae during their migration in fabrics.

Aktivenvotnosheniya: Enterobius vermicularis, Ascaris lumbricoides, Ancylostoma doudenale, Necator americanus, Trichostrongylus orientalis itrichostrongylus colubriformis.
 
Pharmacokinetics. Pyrantelum is very badly soaked up from a digestive tract.

Concentration of drug in a blood plasma, after oral administration of a single dose of 10 mg/kg of body weight, makes 0,005 – 0,13 mkg/ml and is reached within 1-3 hours.

It metablizirutsya partially in a liver to N-methyl-1,3-propandiamina. About 50% of a peroral dose of drug are removed in an invariable view with a stake and about 7% with urine in an invariable form or in the form of a metabolite N-methyl-1,3-propandiamina.

Ascaridosis.

Route of administration and doses:

Drug is used inside in time or after food, carefully chewing a tablet, washing down with 1 glass of water.

Depending on age and the patient's weight drug is appointed in the following doses:

Children of 6 - 12 years	500 mg (2 tablets)
Children are more senior than 12 years and adults weighing up to 75 kg	750 mg (3 tablets)
Adults weighing more than 75 kg	1000 mg (4 tablets)

In case infection only with a human nematode is noted, Pyrantelum is appointed in a smaller dose, i.e. in one step by 5 mg of active ingredient to 1 kg of body weight.

At an ascaridosis and an enterobiosis, and also the mixed invasions these parasites, the dose intended on all course (10 mg/kg of body weight) is applied once. At an ankilostomidoza the daily dose (10 mg/kg of body weight) is applied within 3 days.

At massive invasions of Necator americanus the dose of 20 mg/kg of body weight within 2 days is recommended.

Features of use:

With care patients should appoint drug with abnormal liver functions.

During treatment of a myasthenia it is not necessary to appoint drug.

Use of drug does not demand co-administration of purgatives.

At an enterobiosis it is necessary to carry out simultaneous treatment jointly of the living persons. After completion of treatment it is necessary to conduct a control research a calla on existence of eggs of helminths.

Pyrantelum is more effective at infection with a duodenal ankylostoma (Ancylostoma duodenale), than the American ankylostoma (Necator americanus).

Use in pediatrics. With care it is necessary to use drug at children under 2 years.

Influence on ability to driving of motor transport and to control of mechanisms. In some cases against the background of administration of drug there can be dizziness therefore it is necessary to be careful when performing the work demanding speed of psychomotor reactions.

Pregnancy and period of a lactation. At pregnancy drug is appointed only if the expected effect of therapy exceeds potential risk for a fruit. In the period of a lactation use of drug is possible only under strict observation of the doctor.

Side effects:

Side effects are shown seldom, they are short-term and disappear after the treatment termination.

From a digestive tract: nausea, vomiting, lack of appetite, diarrhea, pains and spasms of a stomach. Seldom – increase in activity of hepatic transaminases.
 
From a nervous system and sense bodys: seldom – a headache, dizziness, drowsiness, sleeplessness, weakness.

In some cases – a hearing disorder, hallucinations with confusion of consciousness and paresthesia.

Interaction with other medicines:

Drug should not be used together with piperazin. These means work antagonistic.

Pyrantelum increases concentration of theophylline in a blood plasma.

Contraindications:

Hypersensitivity to Pyrantelum and other components of drug.
With care appoint at a liver failure.

To children aged up to 6 months drug has to be appointed only in case of unconditional need.

Overdose:

Symptoms: lack of appetite, nausea, vomiting, spastic abdominal pains, diarrhea, headaches, drowsiness or sleeplessness, excitement, skin rash, abnormal liver function.

Treatment: gastric lavage, use of laxatives, symptomatic therapy.
