Lidocaine - Darnitsa, solution for and 2% on 2 ml No. 10

General characteristics. Structure:

International and chemical names: Lidocaine; 2-diethylamino-2 "6" - an atsetoksilidida a hydrochloride;
main physical and chemical properties: the transparent colourless or slightly painted liquid;
structure: 1 ml of solution contains hydrochloride lidocaine - 0,02 g;
excipients: sodium chloride, caustic natron 1M solution, water for injections.

Pharmacological properties:

Lidokain-Darnitsa is active mestnoanestiziruyushchy and antiarrhytmic (1B a class) drug, causes all types of local anesthesia: terminal, infiltration, conduction. Stabilizes cellular membranes, blocks natrium channels. In a myocardium automatism of the ectopic centers, mainly in ventricles oppresses; practically conductivity and a sokratitelnost of a myocardium does not oppress, accelerates process of repolarization of cellular membranes, reduces duration of action potential and the refractory period.

Pharmacokinetics. At intramuscular introduction the beginning of action - in 5-15 minutes, action duration - 60-90 minutes; at intravenous - the beginning in 45-90 sec., action duration - 10-20 min. After intramuscular introduction absorption almost full. Distribution is bystry, distribution volume - 1 l/kg. Linkng with proteins - 60-80%. It is actively metabolized in a liver with formation of two pharmacological active metabolites. Novocaine works longer, than. 10% - in not changed look are removed by kidneys in the form of metabolites. An elimination half-life - 1-3 hours. 

Indications to use:

Treatment and prevention of ventricular arrhythmias (premature ventricular contraction, tachycardia, trembling, fibrillation), including in the acute period of a myocardial infarction, at implantation of an artificial pacemaker, at glikozidny intoxication, an anesthesia. Terminal, infiltration, conduction, spinal (epidural) anesthesia in surgery, ophthalmology, otorhinolaryngology. Blockade of peripheral nerves and nerve knots.

Route of administration and doses:

In cardiological practice: Lidokain-Darnits's introduction begin with a bolus intravenously in a dose 1-2 mg/kg of body weight within 3-4 min.; an average single dose - 80 mg, the maximum single dose - 100 mg. Then at once pass to drop infusion with a speed of 20-55 mkg/kg/min. (at most 2 mg/min.) into 5% solution of glucose or in normal saline solution; drop infusions can be applied during 24-36 h.
If necessary against the background of drop infusion it is possible to repeat intravenously Lidokain-Darnits's introduction in a dose of 40 mg in 10 min. after the first bolus.
Intramusculary the drug is administered on 2-4 mg/kg (the maximum single dose - 200 mg) bucketed 4-6 hours. In some cases use higher doses - on 600 mg each 3-4 hours. 
At a myocardial infarction before hospitalization as a single preventive dose enter 4 mg/kg of Lidokain-Darnits (as much as possible - 200-300 mg).
For local anesthesia: apply 5-10 ml of 2% of solution to conduction anesthesia; for anesthesia of fingers - 2-3 ml; for anesthesia of humeral and sacral textures - 5-10 ml of 2% of solution.

Features of use:

With extra care to apply at patients with heart failure, arterial hypotension, a sinus bradycardia, incomplete AV with blockade, disturbances of intra ventricular conductivity, WPW syndrome, a renal failure and a liver, at a hypovolemia. During pregnancy and feeding by a breast drug can be used only according to vital indications.

Side effects:

Headaches, dizzinesses, sleeplessness, irritability. At very bystry intravenous administration and at patients with hypersensitivity spasms, a tremor, paresthesias, a disorientation, euphoria, a sonitus, a hypophrasia are possible. When using high doses of Lidokain-Darnits bradycardia, conductivity delay, arterial hypotension are possible.

Interaction with other medicines:

At simultaneous use with beta adrenoblockers strengthening of effects of Lidokain-Darnits is possible that causes the need to reduce a drug dose; with Ajmalinum, Amiodaronum, verapamil, Phenytoinum - strengthening of cardiodepressive action; with Phenytoinum and other anticonvulsant drugs derivative of hydantoin - reduction of efficiency of lidocaine; with novokainamidy - hallucinations, excitement are possible; with MAO inhibitors - strengthening of mestnoanesteziruyushchy effect of lidocaine; with drugs which block neuromuscular transmission - strengthening of their action; with somnolent and sedative drugs - strengthening of their oppressing action on TsNS.

Contraindications:

The expressed bradycardia, AV blockade of II and III degrees, the expressed arterial hypotension, cardiogenic shock, severe forms of chronic heart failure, hypersensitivity to drug, the expressed abnormal liver functions.

Overdose:

At overdose developing of bradycardia, delay of conductivity, arterial hypotension, spasms, a tremor, paresthesia, a disorientation, euphoria, a sonitus is possible.

Treatment: intravenous administration of barbiturates of short action, tranquilizers of a benzodiazepine row. In need of the patient intubate, hold artificial ventilation of the lungs and other resuscitation events.

Storage conditions:
