Fluksen

General characteristics. Structure:

1 capsule contains hydrochloride fluoxetine in terms of fluoxetine 0,02 g (20 mg);
excipients: cellulose microcrystallic, aerosil, calcium stearate.

Pharmacological properties:

Pharmacodynamics. Antidepressant for oral administration, selectively and reversibly slows down the return neyronalny serotonin reuptake. Influences capture of noradrenaline and dopamine a little. Poorly also H1 receptors work on cholinergic. Promotes improvement of mood, reduces sensation of fear and tension, eliminates a dysphoria. Does not cause sedation. At reception in average therapeutic doses also other systems practically do not influence functions cardiovascular.

Pharmacokinetics. It is well soaked up from digestive tract. Meal does not influence extent of absorption though can slow down its speed. At intake the maximum concentration in a blood plasma is reached in 4 - 8 hours. The lasting clinical effect occurs after continuous administration of drug within 1 - 2 weeks. Linkng with blood proteins - 94,5%. Easily gets through a blood-brain barrier. It is metabolized in a liver by demethylation with formation of the main active metabolite of a norfluoksetin. The elimination half-life of fluoxetine makes 2 - 3 day, a norfluoksetina - 7 - 9 days. At patients with cirrhosis the elimination half-life increases by 3 - 4 times. Fluoxetine by kidneys of 80% and through digestive tract - about 15% is removed.

Indications to use:

The different types of depressions (especially at the depressions which are followed by fear) including which are not giving in to treatment by other antidepressants; obsessivno-compulsive states; bulimic neurosis (for reduction of appetite and decrease in body weight); alcoholism.

Route of administration and doses:

Fluksen appoint only the adult. An initial dose at a depression - 20 mg of 1 times a day in the first half of day. If necessary in 3 - 4 weeks the dose is increased by 20 mg a day. The maximum daily dose - 80 mg. To elderly people appoint in the dose which is not exceeding 60 mg a day. The expressed medical effect is observed in 1 - 4 weeks after an initiation of treatment. Course of treatment rather long (2 - 3 months). At single application (20 mg) drug is accepted in the morning, at two times - in the morning
and in the evening. At bulimic neurosis Fluksen appoint in a daily dose 60 mg in 3 receptions; at obsessivno-compulsive states - 20 - 60 mg/days depending on disease severity.

Features of use:

To patients with abnormal liver functions and kidneys, at warmly - vascular diseases it is necessary to accept drug with extra care. At patients with a diabetes mellitus change of level of glucose in blood is possible that demands correction of the mode of dosing of gipoglikemiziruyushchy drugs. At simultaneous use of Fluksen and electroconvulsive therapy development of long epileptic seizures is possible. Patients of advanced age need correction of the mode of dosing. During treatment by Fluksen it is necessary to abstain from potentially dangerous types of activity requiring special attention and bystry psychomotor reactions. Safety of use of Fluksen for children is not established. During treatment alcohol intake is not allowed.

Side effects:

As a rule, Fluksen is well had. However are sometimes possible:
- from the central nervous system: condition of alarm, tremor, drowsiness, headache, sleep disorder;
- from the alimentary system: diarrhea, nausea;
- from a metabolism: sweating strengthening, a hypoglycemia, a hyponatremia (especially at patients of advanced age and at a hypovolemia). Allergic reactions: skin rash, itch.
Others: a joint pain and muscles, the complicated breath, fervescence.

Interaction with other medicines:

Fluksen it is possible to apply not earlier than in 14 days after cancellation of inhibitors of a monoaminooxidase. The period after Fluksen's cancellation prior to therapy by inhibitors of a monoaminooxidase has to make
not less than 5 weeks. At simultaneous use with the drugs possessing high extent of linkng with proteins, especially with anticoagulants or digitoxin increase in risk of development of adverse effects is possible; with drugs of lithium increase in concentration of lithium and development of toxic effects is possible; with tryptophane perhaps motive concern, disturbances from digestive tract; with Phenytoinum - increase in its concentration in a blood plasma and development of toxic effects. At simultaneous use of Fluksen with the drugs exerting the oppressing impact on the central nervous system and also with ethanol perhaps considerable strengthening of the oppressing action on TsNS, and also increase in probability of development of spasms.

Contraindications:

Hypersensitivity to drug, pregnancy and the period of feeding by a breast, children's age, epilepsy, the expressed renal failures (a glomerular filtration rate less than 10 ml/min.), a benign hyperplasia of a prostate, glaucoma, a bladder atony, a convulsive syndrome of various genesis, co-administration of inhibitors of a monoaminooxidase.

Overdose:

At overdose nausea, vomiting, excitement, a hypomania, spasms are possible.
Treatment: the gastric lavage, absorbent carbon with sorbitol, at spasms enter diazepam.

Storage conditions:

To store in the unavailable to children, dry, protected from light place at a temperature not above 25 °C. A period of validity - 3 years.
