Garazon

General characteristics. Structure:

Active agent: dinatrium betamethasone phosphate of 1.316 mg (1.0 mg of betamethasone are equivalent), Gentamycini sulfas - in an equivalent of 3.0 mg (3000 ME) to gentamycin to the basis.

Excipients: sodium dihydrophosphate a dihydrate, sodium hydrophosphate a dihydrate, sodium borate, citrate sodium a dihydrate, sodium chloride, эдетат dinatrium, a benzalkoniya chloride, the water purified.

Pharmacological properties:

The combined drug which effect is caused by the components which are its part.
Gentamycini sulfas - an antibiotic of a broad spectrum of activity from group of aminoglycosides, works bakteritsidno. It is effective concerning microorganisms: koagulazopozitivny and koagulazonegativny stafilokokk (including Staphylococcus aureus), Escherichia coli, indolpositive and indolnegative types of Proteus, Pseudomonas aeruginosa, different types of the Klebsiella group - Enterobacter - Serratia, types of Citrobacter, Salmonella, Shigella, Moraxella (including Moraxella lacunata) and Neisseria, in particular Neisseria gonorrhoeae.

Dinatrium betamethasone phosphate - the glucocorticosteroid (GKS), has local antiinflammatory effect, suppressing cellular and fibrinous exudation and normalizing a hyperpermeability of capillaries that is shown by reduction of a local hyperemia, hypostasis and an exudate. At allergic or traumatic defeat of a cornea of GKS suppress proliferation of fibroblasts and post-inflammatory neovascularization of a cornea, thus, keeping its transparency.
 

Indications to use:

- staphylococcal blefarokonjyunktivit, the keratoconjunctivites for the second time infected conjunctivitis; a blepharitis, a keratitis, an episcleritis, a dacryocystitis, barley (meibomitis), injuries of front department of an eye got as a result of hit of foreign bodys, impact of radiation, thermal and corrosive burns and also during the postoperative period;
- acute and chronic outside otitis, diseases of outside acoustical pass for the second time infected: eczema, seborrheal and contact dermatitis.

Route of administration and doses:

It is applied at adults and children 8 years in a conjunctival sac or outside acoustical pass are more senior.
- At diseases of eyes: The dosage has to be defined for each patient individually. Usually dig in 1-2 drops in a conjunctival sac of the affected eye 3-4 times a day. In an acute stage the frequency of use of drug can be increased to 2 drops an hour or each 2 hours; further, when disease becomes controlled, the frequency of use decreases.
The general duration of topical treatment depends on character and disease severity. In case of lack of therapeutic effect within 14 days after an initiation of treatment further use of drug is represented inexpedient.

At ear diseases: Before drug use outside acoustical pass has to be cleared. The recommended initial dosage - 3-4 drops from 2 to 4 times a day. The patient has to lie on one side so that the affected ear was turned up; after an instillation of solution it is necessary to remain in this situation some more minutes to provide penetration of drug in acoustical pass.
At reduction of the inflammatory phenomena it is necessary to reduce gradually a dosage and to stop use of drug after disappearance of symptoms of a disease.
If necessary, in outside acoustical pass it is possible to enter the cotton plug impregnated with the drug Garazon®. The tampon needs to be supported wet, moistening it with drug every four hours. Each 24 hours a tampon should be replaced.
At chronic diseases of eyes and ears the treatment termination of the drug Garazon® has to be made gradually, by reduction of frequency of use.

Features of use:

Гаразон® it is intended only for topical administration. The drug cannot be administered in the form of an injection subkonjyunktivalno or directly in an anterior chamber of an eye.
Lack of bystry clinical effect at use of the drug Garazon® can be the cause for additional inspection for the purpose of specification of the diagnosis.
At preservation of symptoms of a disease or in case of its recurrence, despite completely treated with drug, it is desirable to conduct a bacteriological research of smears from a conjunctiva for definition of sensitivity of the activator to antibiotics.
At purpose of the drug Garazon® for the term of 10 days it is also more necessary to measure intraocular pressure. Carrying out a tonometriya of eyes and a research using a slit lamp is recommended. Patients in whose family anamnesis there are patients with an open angle glaucoma, a myopia of high degree or a diabetes mellitus treat risk group of increase in intraocular pressure as a result of topical treatment of GKS.
It is known that at the diseases causing thinning of a cornea or sclera, topical administration of GKS can lead to perforation of an eyeglobe. In this regard, it is not recommended to begin with use of a combination "an antibiotic - anti-inflammatory drug" treatment of helcomas of a bacterial etiology which can be caused by Pseudomonas aeruginosa. It is reasonable to use only antibacterial agent in the beginning. If there is a response to antibacterial therapy, then it can be recommended to attach to treatment anti-inflammatory drug for minimization of fibrous reaction and the prevention of formation of a hem of a cornea.
At acute purulent processes in an eye of GKS are capable to mask already existing infection or to exponentiate it.
At treatment of the infection caused by a virus of a herpes simplex, GKS should be applied extremely carefully.
In case of local purpose of an antibiotic from group of aminoglycosides it must be kept in mind a possibility of its ototoxicity.
Researches on animals showed that gentamycin at topical administration in the field of outside acoustical pass can be soaked up in blood as after appointment as such way it was defined in blood serum and in urine. Prolonged topical administration of antibiotics or GKS can lead to increase in growth of insensitive microorganisms, including mushrooms. Between various aminoglycosides and GKS cross allergic reactions were noted.
To avoid pollution and cross infection, it is not necessary to use one bottle of drug for simultaneous treatment of an infection of an eye and an ear. In case of a touch solution pollution can happen the open end of a bottle dropper to any surface. Use of a bottle dropper more than one person can lead to spread of an infection.
At prolonged treatment by drug its cancellation is recommended to be carried out gradually.

Use in pediatrics. Safety and efficiency of the drug Garazon® at children aged up to 8 years is not established.

Side effects:

After use perhaps short-term emergence of a burning sensation, an itch or a xeroderma in the place of use.
The undesirable phenomena connected with impact of GKS on eyes include increase in intraocular pressure, glaucoma, damage of an optic nerve, decrease in sharpness and narrowing of fields of vision, development of a back subkapsulyarny cataract, the slowed-down healing of wounds, penetration of air bubbles after surgical treatment of a cataract, development of consecutive infection of eyes (for example, Herpes Zoster), an acute front uveitis, perforation of a cornea and a sclera, a mydriasis, disturbance of accommodation and a ptosis.
When using antibiotics for treatment of eyes allergic reactions can develop. It was reported about passing irritation of eyes in connection with use of Gentamycini sulfas.

Interaction with other medicines:

Any interaction of the drug GARAZON® with other medicines is not registered.

Contraindications:

- hypersensitivity to any of drug components;
- the keratitis caused by a herpes simplex virus (a treelike keratitis)
- viral diseases of a cornea and conjunctiva (for example, at chicken pox),
- mikobakterialny or fungal infections of an eye or ear,
- trachoma,
- a vaktsiniya (the dermatosis arising as a complication of antismallpox vaccination);
- the states which are followed by thinning of a cornea and sclera (for example, after removal from a cornea of a foreign body);
- absence or perforation of a tympanic membrane;
- children's age - up to 8 years;
- pregnancy;
- the lactation period (in connection with a lack of information);
- during treatment carrying soft contact lenses is not recommended (in connection with existence in composition of drug a chloride benzalkoniya).

With care. An open angle glaucoma, a myopia of high degree or a diabetes mellitus in the family anamnesis;
the infection caused by a herpes simplex virus.
Use during pregnancy and a lactation
Гаразон® it is not necessary to appoint during pregnancy if only the potential advantage of its use for mother does not justify potential risk for a fruit.
It is not known whether drug Garazon® components as a part of breast milk are allocated. Therefore during use of drug it is necessary to refuse breastfeeding.

Overdose:

Symptoms. At prolonged use in high doses of GKS - function suppression гипоталамо - pituitary and adrenal system, a hypercorticoidism, aminoglycosides - ototoksichesky action.
Treatment. Gradual drug withdrawal, if necessary - symptomatic therapy, including correction of a water and electrolytic imbalance.

Packaging:

Drops eye and ear. 5 ml of drug in the bottle dropper made of polyethylene of low density. The bottle dropper is closed by the screw-on polyethylene cap having a control ring of the first opening. 1 bottle dropper together with the application instruction in a cardboard pack.
