Diclofenac

General characteristics. Structure:

Active agent: sodium diclofenac (sodium diclofenac) - 1 mg Excipients: propylene glycol - 17 mg, dinatrium эдетат (disodium salt of ethylene diamine tetraacetic acid, Trilonum of B) - 0,1 mg, sodium chloride - 0,5 mg, dihydrophosphate sodium a dihydrate (sodium phosphate monosubstituted 2-water) - 2,5 mg, hydrophosphate sodium dodecahydrate (sodium phosphate disubstituted 12-water) - 5,06 mg, hydroxide sodium solution of 1 M - to рН 6,5-7,5, water for injections to 1 ml.

Pharmacological properties:

Pharmacodynamics. The non-steroidal anti-inflammatory drug derivative of phenylacetic acid, possesses the antiinflammatory and anesthetizing action and moderate febrifugal action. Not selectively oppressing tsiklooksigena-the storage 1 (TsOG1) and cyclooxygenase 2 (TsOG2), breaks metabolism of arachidonic acid, reduces amount of prostaglandins (Pg) in the inflammation center. At topical administration reduces swelled also pain at inflammatory processes of a noninfectious etiology, blocks development of a miosis. Besides, diclofenac inhibits aggregation of thrombocytes. At prolonged use has the desensibilizing effect.

Pharmacokinetics. Drug well gets into various tissues of an eye, except for a crystalline lens. The maximum therapeutic concentration (TCmax) in moisture of an anterior chamber is reached in 30 min. after instillation. In therapeutic significant concentration does not get into system blood circulation.

Indications to use:

Miosis inhibition during operation for a cataract; 
treatment and prevention of inflammatory processes after surgical interventions on an eyeglobe;

prevention of cystous hypostasis of a makula after operations for a cataract;

treatment of noninfectious conjunctivitis;

treatment and prevention of posttraumatic inflammatory process (as addition to local antibacterial therapy).

Route of administration and doses:

Locally. For inhibition of an intraoperative miosis drug is dug in in a conjunctival sac within 2 hours with an interval of 30 minutes (4 times) before operation.
For prevention of cystous hypostasis of a makula drug    3 times a day within 2-3 weeks dig in on 1 drop the field of operation.
With the medical purpose назначаютпо to 1 drop 3 - 4 times a day depending on weight of a state. The course of treatment can proceed from 1 to 2 weeks.

Features of use:

Contact lenses delete before instillation of drug and establish not earlier than in 30 min. after an instillation again.

Side effects:

Burning in an eye, an illegibility of visual perception (right after an instillation); opacification of a cornea (cataract), iritis; allergic reactions: an itch in eyes, a hyperemia, a Quincke's disease, fever, a fever, a photosensitization, skin rash (preferential erythematic, a small tortoiseshell), a multiformny exudative erythema; nausea, vomiting. 
Use with other medicines If necessary can be applied along with other eye drops including containing glucocorticosteroids. At the same time the break between instillations has to be not less than 5 minutes for prevention of washing away of active ingredients the subsequent doses.

Interaction with other medicines:

The interval after use of eye drops Diclofenac and other eye drops has to make not less than 5 min. In the presence of clinical indications it is possible to combine with the eye drops containing glucocorticosteroid means.

Contraindications:

Hypersensitivity to drug components; disturbance of a hemopoiesis of not clear genesis; erosive and ulcer processes in a GIT in an aggravation stage. 
Use at pregnancy Is possible use of drug for treatment of pregnant women and nursing mothers to destination of the attending physician if the expected medical effect exceeds risk of development of possible side effects.

Overdose:

At use of eye drops Diclofenac in the recommended doses the overdose is improbable.

Storage conditions:

In the place protected from light, at a temperature from 15 to 25 °C. Not to freeze. To store in the places unavailable to children. 
Period of validity 2 years. After opening of a bottle to use within 1 month. Not to use after the period of validity specified on packaging.
