Haemoferum

General characteristics. Structure:

Active ingredient: gland (II) chloride of 4-water 157 mg = 44 mg of iron (II) in 1 ml.

Excipients: Natrium benzoicum, citric acid monohydrate, sodium hydroxide, sorbitol of 70%, glucose monohydrate, aroma of "Cocoa", the water purified.

Pharmacological properties:

Pharmacodynamics. Haemoferum – the antianemic drug containing bivalent iron in the form of simple salt of ferric chloride (II) and intended for treatment of iron deficiency states.

Ions of iron play an essential role in transport of oxygen and tissue respiration. Are a compound important part of hemoglobin, a myoglobin and various enzymes, for example: cytochrome C, taking part in education and transmission of energy. Haemoferum in the form of drops contains bivalent iron and is intended for oral administration. Supplements the existing deficit of iron in an organism, and also counteracts emergence of its shortcoming in the period of the increased requirement and in case of insufficient intake of iron.

At deficit of iron, the first effects of effect of drug are observed 3-10 days of its use later. However iron reserves in an organism are recovered in 2-3 months from the beginning of reception.

Pharmacokinetics. Ions of iron are soaked up in upper parts of a small bowel, mainly, in a duodenum.

The soaked-up iron remains in the look connected with ferritin, generally in a liver.

In marrow it is included hemoglobin.

Absorption of iron depends on valency, degree of acidity of gastric contents and deficit of iron in an organism. Drugs of bivalent iron do not demand an introductory reduction and are easily soaked up.

In conditions of the expressed deficit, extent of absorption of this element is estimated for about 20%. In process of iron exchange normalization, the amount of assimilable iron in a digestive tract, gradually decreases.

Indications to use:

- treatment of zhelezodifetsitny anemia;

- treatment of latent deficit of iron;

- treatment of chronic posthemorrhagic anemia.

Prevention of deficit of iron during the periods of pregnancy and a lactation; at premature, newborn of polycarpous pregnancy, at children from mothers at whom during pregnancy the iron deficiency anemia was observed; during intensive growth at children and teenagers; at adults (vegetarians and elderly people).

Route of administration and doses:

1 ml of drug contains 44 mg of bivalent iron.

In 1 drop about 1,6 mg of bivalent iron.

The mode of dosing is set individually.

In the medical purposes:

Premature children - daily 1,5-3,0 mg of iron (II)/kg of body weight (1-2 drops) within 3 - 5 months.

Children till 1 year - daily 15-30 mg (9-19 drops) a day.

Children from 1 year to 12 years - daily 45 mg (28 drops) 1-2 times a day.

Children are more senior than 12 years - daily 45 mg (28 drops) 2 times a day.

Adults - daily 55 drops (2 ml) 2 times a day.

The medical dose for children should not exceed 3 mg/kg a day, for adults – 180-200 mg a day.

Preventive doses make 1/2-1/3 from therapeutic.

Duration of treatment is not less than 2 months. The dosage of drug and terms of treatment depend on degree of deficit of iron. In case of clinically expressed deficit of iron the level of hemoglobin is normalized only in 2-3 months after an initiation of treatment. After that, for recovery of reserves of iron in an organism, administration of drug Haemoferum in preventive doses has to be continued within several months.

Features of use:

Drug can cause dark coloring of enamel of teeth therefore it should be accepted through a straw or to rinse a mouth at once after administration of drug. As a result of the increased content of iron in intestines, during treatment by drug, the chair can be more dark.

Because of presence at composition of excipient of glucose – fluctuations of level of glucose of blood at patients with a diabetes mellitus, at persons with a glyukozo-galaktozovy syndrome of bad absorption can appear, fructose is intolerance or persons with deficit have sakharozo-isomaltases.

Side effects:

Hyperthymia, headache, dermahemia, allergic reactions (itch, rash), nausea, vomiting, locks or diarrhea, abdominal pains.

Interaction with other medicines:

It is incompatible with the medicines containing SH group: colloid bismuth citrate, calcium phosphate, chloramphenicol, холестирамин, колестипол, tetracyclines, Pancreatinum, salts of zinc can reduce efficiency of drug. Ascorbic acid increases absorption of ions of iron. Drug can reduce efficiency of hinolon, levodofa, left thyroxine, Methyldopa, tetracyclines, penicillin, Sulfasalazinum, and also zinc salts. It is necessary to avoid simultaneous use of salts of iron and Allopyrinolum. Food, mainly milk and its products, and also eggs reduce drug absorption.

Contraindications:

- hypersensitivity to drug components;

- excess content of iron in an organism (including a hemosiderosis and hemochromatosis);

- the anemias caused by iron utilization disturbance (for example, lead anemia, sideroakhrestichesky anemia);

- the anemias which are not connected with deficit of iron (including aplastic and hemolitic anemias, megaloblastny anemia);

- aplastic anemia;

- hemochromatosis;

- inflammatory diseases of digestive tract;

- deficit of sucrose/isomaltase, intolerance of fructose, glyukozo-galaktozny malabsorption.

With care: peptic ulcer of a stomach and duodenum, enteritis, ulcer colitis.

Overdose:

Symptoms: pain in epigastriums, nausea, vomiting, diarrhea or a lock, a lowering of arterial pressure, in more hard cases - drowsiness, pallor, a necrosis of a mucous membrane of digestive tract, hypovolemic shock up to a coma. Symptoms of a peripheral circular collapse are shown within 30 minutes after reception; a metabolic acidosis, spasms, heat, a leukocytosis, a coma - within 12-24 hours; an acute renal and hepatic necrosis - in 2-4 days.

Treatment: drug withdrawal, gastric lavage, purpose of laxative drugs, performing symptomatic therapy; in case of a serious poisoning - Deferoxaminum in/in slowly: to children - 15 mg/h, adult - 5мг/кг/ч (to 80 mg/kg/days); at slight poisoning in oil: to children on 1 g each 4-6 hours, the adult - on 50 mg/kg (to 4 g/days).

Drops for intake, 44 mg of iron (II)/ml, on 10 ml or 30 ml in bottles of dark glass with a polyethylene stopper dropper and the screwing-up polyethylene cover with a guarantee ring of "the first opening". 1 bottle together with the application instruction in a cardboard pack.
