General characteristics. Structure:

Pharmacological properties:

Antiepileptic means. Practically does not drowse.

Strengthens brake GABA ергические influences on TsNS, especially in a thalamus, the ascending activating reticular formation of a brainstem at the level of internuncial neurons. Reducing permeability of membranes of nerve fibrils for sodium ions, reduces distribution of impulses from the center of epileptic activity. The effect occurs in 20-60 min. after oral administration.

Pharmacokinetics. It is metabolized with formation of phenobarbital which renders antiepileptic effect. Linkng with proteins of a blood plasma weak. High concentration are found in a brain, a liver and kidneys. Gets through gistogematichesky barriers, it is allocated with breast milk. T1/2 makes 3-4 Is removed by kidneys both in not changed look, and in the form of metabolites.

Indications to use:

Epilepsy of various genesis, generalized and partial attacks.

Route of administration and doses:

Establish individually. Inside - on 100 mg 3 Maximum doses: one-time - 300 mg, daily - 800 mg. Treatment is begun with a single dose of a single dose. In 2-3 days gradually increase a dose before achievement of clinical effect (decrease in frequency or complete cessation of attacks). Prolonged treatment.

To children at the age of 3-6 years - on 25-50 mg on reception (100-150 mg/), 7-10 years - on 50-100 mg on reception (150-300 mg/days), 11-14 years - on 100 mg on reception (300-400 mg/). The maximum doses for children (advanced age): one-time - 150 mg, daily - 450 mg

Features of use:

At the patients who were earlier accepting barbiturates upon transition to treatment with petrolbarbital sleep disorders are possible. In these cases appoint phenobarbital (50-100 mg) or other hypnagogues to night.

Side effects:

From TsNS: seldom - drowsiness, slackness, block, a headache, an ataxy, a nystagmus, difficulties of the speech (in these cases dose adjustment or purpose of caffeine is required).

From system of a hemopoiesis: thrombocytopenia, anemia.

From cardiovascular system: decrease in the ABP, thrombophlebitis.

Others: deterioration in appetite, allergic reactions, bronchospasm.

Interaction with other medicines:

Strengthening of effects of opioid analgetics, anesthetics, antipsychotic drugs, anxiolytics, tricyclic antidepressants, ethanol, somnolent drugs.

At simultaneous use decrease in efficiency of paracetamol, anticoagulants, tetracyclines, griseofulvin, GKS, mineralokortikoid, cardiac glycosides, quinidine, vitamin D, xanthines is observed.

At simultaneous use with the drugs having miyelodepressivny effect strengthening of a gematotoksichnost is observed.

Contraindications:

Heavy renal failures and/or a liver, chronic heart failure of the II-III stage, a porphyria, anemia, bronchial asthma, respiratory insufficiency, a diabetes mellitus, adrenal insufficiency, hyperkinesias, depressions (with suicide attempts), hypersensitivity to petrolbarbital.
