Лоперамид-Акри®

General characteristics. Structure:

Active ingredient: 2 mg of loperamide of a hydrochloride.

Excipients: lactose (sugar milk), starch corn, silicon dioxide colloid (aerosil), talc, magnesium stearate. Structure of the capsule: titanium dioxide, dye quinolinic yellow, dye sunset yellow, gelatin.

Pharmacological properties:

Pharmacodynamics. Loperamide, contacting opioid receptors of an intestinal wall (stimulation sincaline - and adrenergic neurons through a guanine nucleotides), reduces a tone and motility of smooth muscles of intestines (at the expense of inhibition of release of acetylcholine and prostaglandins). Slows down a peristaltics and increases time of passing of intestinal contents. Raises a tone of a proctal sphincter, promotes deduction of fecal masses and an urezheniye of desires to defecation. Action develops quickly and 4-6 hours proceed.

Pharmacokinetics. At intake absorption makes 40%. The maximum concentration in plasma is reached in 2,5 h after reception of capsules. Communication with proteins of plasma (it is preferential with albumine) — 97%. Does not get through a blood-brain barrier. It is almost completely metabolized by a liver by conjugation. An elimination half-life — 9-14 hours (on average 9,8 h). It is removed preferential with bile, an insignificant part is removed by kidneys (in the form of the conjugated metabolites).

Indications to use:

Symptomatic treatment of acute and chronic diarrhea of various genesis (allergic, emotional, medicinal, beam; at change of a diet and qualitative structure of food, at disturbance of metabolism and absorption; as supportive application at diarrhea of infectious genesis). Regulation of a chair at patients from ileostomy

Route of administration and doses:

Inside, without chewing, to wash down with water. Adult at acute diarrhea originally appoint 2 capsules (4 mg) ^{Loperamid-Akri®}, then on 1 capsule (2 mg) after each act of defecation in case of a liquid chair. The highest daily dose — 8 capsules (16 mg).

At chronic diarrhea the adult appoint 4 mg/days. The maximum daily dose — 16 mg.

At acute diarrhea to children 6 years are more senior appoint in an initial dose 2 mg, then on 2 mg after each act of defecation in case of a liquid chair. The maximum daily dose — 4 capsules (8 mg).

At chronic diarrhea to children 6 years are more senior ^{Loperamid-Akri®} appoint 2 mg in a daily dose. The maximum daily dose of 6 mg on 20 kg. After normalization of a chair or in the absence of a chair more than 12 hours reception of drug should be stopped.

Features of use:

In the absence of effect after 2 days of use ^{of Loperamid-Akri®} it is necessary to specify the diagnosis and to exclude infectious genesis of diarrhea.

To children 6 years are younger it is not recommended to appoint in capsules.

If at treatment locks or abdominal distention develop, ^{Loperamid-Akri®} should be cancelled. At patients with abnormal liver functions careful control of signs of toxic damage of the central nervous system is necessary. During treatment of diarrhea it is necessary to make up for loss of liquid and electrolytes.

During treatment it is necessary to be careful during the driving of motor transport and occupation other potentially dangerous types of activity demanding the increased concentration of attention and speed of psychomotor reactions.

Side effects:

It is observed, as a rule, only at long administration of drug. Allergic reactions (skin rash), drowsiness, dizziness, a hypovolemia, electrolytic disturbances, dryness in a mouth, intestinal colic, a gastralgia, pain or discomfort in a stomach, nausea, vomiting, a meteorism are possible.

Seldom — an ischuria, it is extremely rare — intestinal impassability.

Contraindications:

Hypersensitivity, intestinal impassability, acute ulcer colitis, divertuculosis, diarrhea against the background of an acute pseudomembranous coloenteritis, dysentery and other infections of digestive tract. Pregnancy (1 trimester), the lactation period, children's age up to 2 years (^{Loperamid-Akri®} in capsules do not appoint to children up to 6 years).

With care. Liver failure.

Overdose:

Symptoms: signs of oppression of function of the central nervous system (a stupor, lacks of coordination of movements, drowsiness, narrowing of pupils (miosis), the raised tone of skeletal muscles, respiratory depression), intestinal impassability.

Treatment: As a specific antidote use Naloxonum. Considering that duration of action ^{of Loperamida-Akri®} is more, than at Naloxonum, perhaps repeated introduction of the last. Symptomatic treatment: a gastric lavage, reception of absorbent carbon (in the first 3 hours after administration of drug), artificial ventilation of the lungs.

Storage conditions:

In the dry, protected from light place, at a temperature not above 25 °C. To store in the place, unavailable to children. Not to use drug after a period of validity. A period of validity - 4 years.
