General characteristics. Structure:

Active ingredient: 50 mkg of liothyroninum of sodium.

Excipients: magnesium stearate, лудипрессо (lactose, povidone, кросповидон).

The drug compensating a lack of hormones of a thyroid gland.

Pharmacological properties:

Pharmacodynamics. Increases the need of fabrics for oxygen, stimulates growth and a differentiation of fabrics, increases the level of basal metabolism (proteins, fats and carbohydrates). In small doses renders anabolic, and in big – catabolic effect. Oppresses production of thyritropic hormone. Strengthens power processes, positively influences a condition of nervous and cardiovascular systems, a liver and kidneys.

Pharmacokinetics. After intake of 95% it is soaked up during 4 h. Stage of latency makes 4-8 h. In blood strongly contacts proteins. The maximum pharmacological effect occurs in 2–3 days. T1/2 – 2,5 days.

Indications to use:

Hypothyroidism of any origin.

Treatment of a hyperplasia of a thyroid gland: a benign craw with normal function, prevention of a recurrence of a craw after carrying out operation or after therapy by a radioiodine.

As a part of a combination therapy of a toxic craw with thyreostatic means (after achievement of medicamentous compensation of exchange processes).

Route of administration and doses:

Inside. At gipotireodny states appoint 25 mkg/days, increase in a dose by 25 mkg each 1–2 weeks is possible. The maintenance dose makes 25–75 mkg.

At a myxedema an initial dose — 5 mkg/days, increase each 1–2 weeks by 5–10 mkg, at achievement of a dose of 25 mkg/days further increase is made on 5–25 mkg/days by each 1–2 weeks. The maintenance dose makes 50–100 mkg/days.

At a nontoxic craw the initial dose (5 mkg/days) is increased by 5–10 mkg by each 1–2 weeks. The maintenance dose usually makes 75 mkg/days.

Features of use:

With care to appoint to patients of advanced age, at defeats of coronary vessels (ischemic heart disease), heart failure and at certain forms of disturbance of a cordial rhythm (tachyarrhythmia).

It is necessary to avoid even easy, caused by liothyroninum, hyperfunctions of a thyroid gland, in particular at insufficient blood supply of heart (coronary insufficiency), at heart failure or at tachyarrhythmias.

At hypofunction of the thyroid gland caused by damage of a hypophysis it is necessary to find out whether there is at the same time an insufficiency of bark of adrenal glands. In this case its treatment should be begun before purpose of therapy by hormones of a thyroid gland.

Side effects:

Headache, uneasiness, irritability, perspiration, tachycardia, arrhythmia, stenocardia, heart failure, dysmenorrhea, allergic reactions.

Interaction with other medicines:

Increases effect of peroral anticoagulants, vasoconstrictors, reduces — insulin and other hypoglycemic means. Activity decreases oral contraceptives and Colestyraminum. Phenytoinum, salicylates, дикумарол, furosemide (in high doses), Clofibratum, antidepressants, cardiac glycosides, ketamine increase concentration and risk of development of side effects of liothyroninum.

Contraindications:

Hypersensitivity, thyrotoxicosis, diabetes mellitus, Addison's disease, adrenal insufficiency, cachexia, ischemic heart disease, pregnancy and lactation.

Overdose:

Symptoms: loss of weight, the increased appetite, heartbeat, nervousness, a diarrhea, intestinal gripes, perspiration, tachycardia, arterial hypertension, heart arrhythmias, a shiver, sleeplessness, intolerance is warm, fever, irregular periods.

Treatment: The caused vomiting or a gastric lavage (except for a coma, spasms), absorbent carbon or холестирамин for a drug absorption delay, beta-blockers (propranolol intravenously in a dose of 1-3 mg within 10 min. either more or orally 80-160 mg a day), cardiac glycosides at congestive heart failure. Fight against fever and hypoglycemia, hypovolemia.

Dialysis is inefficient as drug is connected with proteins.

Storage conditions:

To store at a temperature of 15-25 °C, in dry, protected from light and the place, unavailable to children. Period of validity 3 years. Not to use after the expiry date specified on packaging.
