General characteristics. Structure:

Active ingredient: 80 mg or 160 mg of a sotalol of a hydrochloride.

Excipients: corn starch, gelated starch, calcium hydrophosphate, magnesium stearate, talc.

Pharmacological properties:

Pharmacodynamics. Sotalola a hydrochloride represents non-selective beta adrenoblocker a beta 1-and a beta of 2 adrenoceptors. Reduces the heart rate and sokratitelny ability of a myocardium, slows down AV conductivity, reduces the absolute refractory period. Has the expressed antiarrhytmic effect which mechanism consists in increase in duration of action potential and lengthening of the absolute refractory period in all sites of the carrying-out system of heart (the III class of antiarrhytmic drugs). Blocking a beta 2 adrenoceptors, raises a tone of smooth muscles of bronchial tubes, vessels.

Indications to use:

Symptomatic disturbances of a cordial rhythm (tachyarrhythmia) of a supraventricular origin, for example, atrioventricular (nodal) Bouveret's diseases at Wolff-Parkinson-Whyte's syndrome or paroxysmal ciliary arrhythmias of auricles. Maintenance of a normal sinoatrial rate after stopping of blinking or an atrial flutter.

Heavy symptomatic ventrikulyarny disturbances of a cordial rhythm like tachyarrhythmia and their prevention at the proved efficiency.

The arrhythmias caused by excess circulation of catecholamines or hypersensitivity to catecholamines.

Route of administration and doses:

In the absence of other instructions it is possible to use the following scheme. For treatment of disturbances of a cordial rhythm like tachyarrhythmias about 40 mg 2 times a day are recommended to accept in the beginning. The usual daily dose makes 160-320 mg. At need the dose drug can be increased to 160 mg three times a day.

At patients with an impaired renal function the dose of drug depends on renal clearance. At decrease in renal clearance to 10-30 ml/min. (creatinine of serum of 2-5 mg/dl) the dose decline half is recommended; at clearance less than 10 ml/min. (creatinine of a syvortka more than 5 mg/dl is recommended a dose 1/4.

At coronary heart disease and disturbances of a cordial rhythm, and also at long use of drug its gradual cancellation is recommended. Cancellation or change of the mode of a drug dosing by the patient with life-threatening disturbances of a cordial rhythm are appointed only by the doctor. The patients who had a myocardial infarction or patients with the broken sokratitelny ability of a myocardium, have to be under observation of the cardiologist. A pill is recommended to be taken to food.

Features of use:

It is necessary to be careful at purpose of drug SOTALEKS to patients, recently had a myocardial infarction, at a diabetes mellitus, a pheochromocytoma, psoriasis, and also the patient with an impaired renal function.

Treatment by drug SOTALEKS is carried out under control of indicators of heart rate, arterial pressure, an ECG. In case of drug withdrawal the dose should be reduced gradually.

Pregnancy and feeding by a breast. It is necessary to use drug during pregnancy and feeding by a breast only in the presence of vital indications.

Side effects:

Bradycardia, AV blockade, strengthening of symptoms of heart failure, arterial hypotension, bronchospasm.

Are possible - dizziness, a headache, fatigue, drowsiness, parasthesias, a cold snap of extremities, muscular weakness. In rare instances - nausea, diarrhea, a lock, skin allergic reactions, a sleep disorder, dryness in a mouth, a hypoglycemia, sexual dysfunction, a depression. There are separate messages on cases of strengthening of attacks of stenocardia, disturbance of visual acuity, an inflammation of a cornea and conjunctiva, syncopal states.

Interaction with other medicines:

It is necessary to be careful at simultaneous use of drug with antagonists of calcium such as verpamil and diltiazem, in connection with the combined dystviye on sinus and atrioventricular nodes.

At use of drug SOTALEKS in a combination with other antiarrhytmic drugs of a class I, it is necessary to avoid purpose of the drugs extending QT interval (especially substances like quinidine) that increases risk of developing of ventricular arrhythmias.

It is also necessary to avoid simultaneous use of antiarrhytmic drugs of a class III, drugs: галопердол, antihistaminic drugs, астемизол and терфенадин, галофантрин (anti-malarial перапарат) because of danger of excess lengthening of an interval of QT.

Simultaneous use with antagonists of calcium and hypotensive drugs, some tranquilizers, somnolent drugs (tricyclic antidepressants, barbiturates, fenotiazina and narcotic drugs), diuretics and vasodilating drugs can lead to considerable falling of arterial pressure.

Negative chronotropic effect of drug, and also the delay of atrioventricular conductivity caused by it can amplify at simultaneous use with Reserpinum, a clonidine, alphamethyldopum, guanfatsiny or cardiac glycosides.

Simultaneous use of insulin or peroral anti-diabetic drugs, especially at the raised exercise stress, can cause decrease in level of sugar in blood or strengthen hypoglycemia symptoms.

Contraindications:

Chronic heart failure of IIB - the III stages, shock, arterial hypotension, an atrioventricular block 2 and 3 degrees, sinuatrial blockade, a sick sinus syndrome, bradycardia, lengthening of an interval of QT obliterating diseases of vessels, obstructive respiratory diseases, a metabolic acidosis, throat hypostasis, heavy allergic rhinitis, hypersensitivity to drug and sulfonamides, not treated pheochromocytoma.

Storage conditions:

To store at a temperature of 15-25 °C. To store in the place, unavailable to children! Not to use drug on the termination of a period of validity.
