General characteristics. Structure:

Active ingredients: albumine in terms of protein - 50 g, sorbitol - 60 g, solution of sodium of a lactate of 60% - 35 g (in terms of sodium a lactate - 21 g), sodium chloride - 8 g, Calcii chloridum in terms of dry matter - 0,1 g, potassium chloride - 0,075 g, Natrii hydrocarbonas - 0,1 g;

excipients: water for injections - to 1 l.

Ionic structure of 1 ml of drug: Na ⁺ - 7,898 mg, ^K+ - 0,0393 mg, Ca ⁺⁺ - 0,036 mg, Cl ^- - 4,953 mg, _HCO3 ^- - 0,073 mg, _CH3CH(OH)COO ^- - 16,692 mg.

Molar structure of 1 l of drug: Na ⁺ - 343,5 mmol, ^K+ - 1,0 mmol, Ca ⁺⁺ - 0,9 mmol, Cl ^- - 139,7 mmol, _HCO3 ^- - 1,2 mmol, _CH3CH(OH)COO ^- - 187,4 mmol.

Main properties: transparent viscous liquid of light yellow or yellow color; theoretical osmolarity of 1020 ¼«ß¼«½ý/l; рН 6,2-7,4.

Pharmacological properties:

^{Лактопротеин-С®} quickly increases arterial pressure and renders assistance to deduction of liquid in a blood channel. Possesses antishock and disintoxication action, promotes neutralization of a metabolic acidosis. At repeated transfusions leads to normalization of protein metabolism. The main pharmacological active agents of drug are albumine, sorbitol and sodium a lactate.

Albumine – plasma substituting means; fills shortage and raises reserves of proteinaceous food. Has visoky oncotic activity and supports the oncotic pressure of blood, quickly increases the arterial pressure of blood that defines its hemodynamic properties. Promotes deduction of liquid in a circulatory bed.

Sorbitol - hexatomic alcohol which at intravenous administration quickly joins in the general metabolism: 80-90 its % are utilized in a liver and collect in the form of a glycogen, 5% are laid off in brain tissues, a cardiac muscle and skeletal muscles, 6-12% - are allocated with urine. In a liver sorbitol at first turns into fructose which turns further into a glycogen. A part of sorbitol is used by an organism for urgent energy demands, other part is postponed as a stock in the form of a glycogen. Isotonic solution of sorbitol has dezagregatny action and, thus, improves microcirculation and perfusion of fabrics. The maximum speed of utilization of sorbitol makes 0,25 g/kg of weight body/hour.

Sodium the lactate belongs to alkalizing means of the slowed-down action. At introduction to a vascular bed of sodium the lactate releases sodium, _{CO2 and H2O} which form sodium bicarbonate and by that increase an alkaline reserve of blood. Correction of a metabolic acidosis by means of sodium of a lactate happens slowly (in process of lactate sodium inclusion in a metabolism), and does not cause sharp fluctuations рН. Effect of sodium of a lactate is shown in 20-30 minutes after introduction. Only one half of the entered lactate sodium is considered active (isomer L), the second half  (isomer D) is not metabolized and removed with urine.

Indications to use:

^{Лактопротеин-С®} apply to correction of an acid-base state and a hypoproteinemia, improvement of microcirculation and a hemodynamics, reduction of intoxication at:

- traumatic, operational, hemolitic and burn shocks, burn disease;

- heavy band operations, heart operations;

- hypoproteinemias and hypoalbuminemias developing at dystrophy, a nephrotic syndrome;

- cirrhosis;

- long suppurative processes;

- various infectious defeats, eksikoza caused by gastrointestinal diseases;

- sepsis;

- intestinal impassability;

- chronic hepatitises;

- in complex therapy of the oncohematological patients receiving program cytostatic treatment;

- in complex therapy of various groups of patients with hepatic pathology of an alcoholic etiology, including with chronic persistent hepatitis, chronic active hepatitises І - ІІ degrees of activity, cirrhoses of a liver And, In, From severity on Child-Pugh, when carrying out a paracentesis at ascites.

Route of administration and doses:

^{Лактопротеин-С®} enter intravenously. At shock – struyno, in other cases - kapelno (60-80 drops a minute):

- at traumatic, burn, postoperative and hemolitic shocks - on 400-600 ml (6,5-10,0 ml/kg of body weight) disposable or twice a day, in the beginning struyno, then kapelno (at control of a diuresis);

- at chronic hepatitises - on 200 ml (3,5 ml/kg of body weight) kapelno; repeatedly - daily or every other day on an extent of 3-5 days (at control of a diuresis, indicators of function of a liver and an acid-base state);

- in the preoperative period and after surgical interventions - on 200-400 ml (3,5-7,0 ml/kg of body weight) kapelno, disposable or twice a day on an extent of 3-5 days.

- at complex therapy of the oncohematological patients receiving program cytostatic treatment from the first day of chemotherapy about 200 ml within 5 days are recommended to enter daily; taking into account results administration of drug can be continued up to 6-7 days. The course of introduction can be repeated in 1-2 weeks;

- at alcoholic diseases of a liver:

- chronic persistent hepatitis and chronic active hepatitis І - ІІ activity stage degrees – on 100 ml each 2-3 days to 4-5 times on a course; it is reasonable to conduct repeated courses to 3 times a year in a complex of the standard therapy;

- cirrhosis (And, In, From severity on Child-Pugh) and existence of uncomplicated ascites – on 200 ml each 2-3 days to 4-5 times on a course; repeated courses are conducted against the background of basic therapy by diuretics to 3 times a year;

- cirrhosis with ascites, resistant to medicamentous therapy, – on 200 ml right after a medical paracentesis, in the next days – on 200 ml of 4-5 times at an interval of 2-3 days and in the subsequent (by results) the same courses 3 times a year.

Features of use:

The first 10-15 ml of drug apply under control of the doctor, slowly (with a speed of 20-30 thaws a minute). In case of development of undesirable reactions, administration of drug should be stopped and if necessary to use antihistaminic, cardiotonic, glucocorticoid, other medicines.

Drug is used at control of indicators of osmolarity of blood and urine, and also an acid-base state. At ascites І, ІІ, ІІІ severity and forms, resistant to medicamentous therapy, careful monitoring of level of urea, creatinine, sodium and potassium is necessary. An indispensable condition of use of drug at ascites is the diet with restriction of salt and adequate diuretic therapy.

Treatment of patients with alcoholic diseases of a liver is available in all therapeutic, gastroenterological and surgical departments of hospitals, and in the presence of the corresponding conditions, can be carried out on an outpatient basis.

Use during pregnancy and feeding by a breast. It is not necessary to use drug during pregnancy and feeding by a breast.

Side effects:

At certain patients lumbar pains, allergic reactions - temperature increase, a small tortoiseshell, etc. are possible. At long introduction the irritation of peripheral veins in injection sites is possible.

Interaction with other medicines:

Drug is not mixed with other medicines and not used as solution carrier for other drugs.

Contraindications:

Giperosmolyarny coma, full anury, alkalosis, tendency to allergic reactions, hematencephalon, thromboembolism, cardiovascular decompensation, idiopathic hypertensia ІІ-ІІІ St. Use of drug at the cirrhosis and ascites complicated by a gepatorenalny syndrome and an anury is not shown. It is not necessary to apply in complex therapy of patients with alcoholic diseases of a liver at such laboratory indicators: bilirubin level in blood> 100 µmol/l; prothrombin ratio <40%; number of thrombocytes <40 * ^109/l; creatinine level in blood> 115 µmol/l; urea > 8,3 mmol/l; ALAT> 1,5 mmol / час*л; ASAT> 1,5 mmol / час*л; Na ⁺> 150 mmol/l; K ⁺> 5,4 mmol/l.

It is not necessary to administer the drug to children.

Overdose:

At considerable overdose there are phenomena of an alkalosis and dehydration. Nausea, intestines swelling, an abdominal pain can be observed. In this case administration of drug is immediately stopped.

Storage conditions:

Store in the dry, protected from light place at   a temperature from 2 °C to 8 °C. To store in the place, unavailable to children.
