Naysulid

General characteristics. Structure:

Active ingredient: 10 mg of Nimesulide in 1 g of gel.

Excipients: alcohol, carbomer, macrogoal 400, propylene glycol, dimethyl sulfoxide.

The non-steroidal anti-inflammatory drug of local appointment having soothing and antiinflammatory properties.

Pharmacological properties:

Pharmacodynamics. Gel reduces concentration of the short-lived PGN2 which is substrate for kinistimulirovanny synthesis by PG-isomerase (with the participation of hemin and adrenaline) PGE2 in the center of an inflammation and in the ascending ways of carrying out painful impulses in a spinal cord. Decrease in concentration of PGE2 (a mediator of an inflammation and pain) reduces activation of prostanoid receptors of ER of type that is shown by analgetic and antiinflammatory effect.

Pharmacokinetics. Gel is quickly soaked up in the subject fabrics, comes to synovial fluid and has the expressed local antiinflammatory and analgeziruyushchy effect. System absorption at the same time insignificant; in 5 h after a gel appliktion concentration of the main active metabolite (4-hydroxy-Nimesulide) in a blood plasma does not exceed 100 mkg/l.

Indications to use:

- as supportive application at complex therapy of diseases of joints and soft tissues of the inflammatory nature (the osteoarthritis, a periarthritis, a tendinitis, tendosinovit);
- muscle strain and sheaves.

Route of administration and doses:

Apply outwardly. About 3 cm of gel apply on an affected area and slightly rub; the procedure is repeated by 3-4 times a day. It is not necessary to rub gel intensively or to use bandages. Duration of therapy is defined individually; efficiency of treatment is recommended to be estimated in 4 weeks.

Features of use:

Pregnancy and lactation. It is recommended to abstain from use of gel during pregnancy and feeding by a breast.

Efficiency of gel in pediatric practice is not established.

At development of local reactions (irritation of skin, etc.) gel is cancelled. It is impossible to apply gel on the mucous membranes or sites of skin struck with a dermatosis or infectious processes. The processed sites of skin cannot be closed occlusive bandages.

Influence on ability to management of transport and mechanisms. External use of gel does not exert impact on ability to management of transport and mechanisms.

With care it is necessary to appoint medicine at GIT erosive cankers (in an aggravation phase), a liver failure, a renal failure (KK <30 ml/min.), chronic heart failure, arterial hypertension, bleeding from a gastric-intestinal path, a diabetes mellitus of type 2.

Side effects:

The local irritation, erythema, rash, peeling and itch of skin are in rare instances possible. Perhaps minor change of skin in a gel application site of application which disappears in several days.

Interaction with other medicines:

At cutaneous use of drug interaction with other medicines is not established it. However it is necessary to consider that at possible receipt in a system blood stream Nimesulide can increase efficiency and toxicity of many medicines as a result of replacement from places of linkng with proteins.

Contraindications:

Hypersensitivity to Nimesulide, acetylsalicylic acid or other NPVP.

Overdose:

Due to the low system absorption at topical administration the overdose is improbable.

Storage conditions:

In the place protected from light at a temperature not above +25 °C. To store in the place, unavailable to children. Period of validity 2 years. A period of storage after opening of 28 days.
