General characteristics. Structure:

Active agent: 50 mg or 100 mg of Nimesulide

Excipients: starch, microcrystallic cellulose, colloid silicon dioxide, gelated starch, monohydrate of citric acid, the purified talc, sodium salt of glycollate of starch, aspartame, banana fragrance (for tablets of 50 mg), fruit fragrance (for tablets of 100 mg) magnesium stearate, the water purified.

Pharmacological properties:

Nimika is non-steroidal anti-inflammatory drug (NPVS) from a class of sulfonanilid. Has the antiinflammatory, anesthetizing and febrifugal effect.
Nimesulide concerns to new generation of NPVS which mechanism of action is connected with the selection inhibition of cyclooxygenase II and impact on some other factors: suppression of a factor of activation of thrombocytes, factor of a necrosis of tumors alpha, suppression of proteinases and histamine.

Pharmacokinetics. At intake it is well soaked up from digestive tract. The maximum concentration of active agent in a blood plasma is reached in 1,5–2,5 hours after administration of drug. Use of drug along with food reduces absorption speed, but without influencing its degree. Linkng with proteins of plasma makes 99%, distribution volume — 0,19–0,35 l/kg. Nimesulide is actively metabolized in a liver with formation of several metabolites allocated generally with urine (70%) and a stake (30%).
At patients with a renal failure (clearance of creatinine of 30-80 ml/min.), and also at children and elderly people the pharmacokinetic profile of Nimesulide significantly did not change.

Indications to use:

The fever symptoms connected with upper respiratory tract infections (the virus or bacterial nature) and for simplification of pains of various origin during the postoperative period at damages of a musculoskeletal system, at wounds of soft tissues.

Route of administration and doses:

Adult: Usually appoint inside in a dose 100 mg (2 tablets on 50 mg) 2 times after food.
Children: 1,5 mg/kg of body weight 2–3 times a day. The maximum dose for children should not exceed 5мг/кг/сутки, divided into 2 or 3 receptions. Tablets before the use should be dissolved in 5 ml (1 teaspoon) of water.

Features of use:

Because at use of drug emergence of such by-effects as dizziness and drowsiness is possible, it is necessary to be careful at its appointment to the patients who are engaged in potentially dangerous types of activity demanding the increased concentration of attention and bystry mental and motor reactions.

Side effects:

Usually Nimik's drug does not cause side effects. In rare instances ulcerations of a mucous membrane of a GIT, a liquid delay, nausea, vomiting, a stomach ache, diarrhea, dizziness, drowsiness, an itch, allergic reactions (skin rash, an acute anaphylaxis) can be observed; thrombocytopenia, leukopenia, anemia, agranulocytosis; increase in activity of "hepatic transaminases"; lengthening of a bleeding time; hamaturia.
At manifestation of any side effects it is necessary to stop administration of drug and to consult with the doctor.

Interaction with other medicines:

It is necessary to be careful at simultaneous use of Nimesulide with:
— digoxin, Phenytoinum, and lithium drugs;
— diuretics and antihypertensives;
— other NPVS;
— anticoagulants;
— cyclosporine;
— methotrexate;
— peroral antidiabetic means.
since at the level of linkng with proteins of plasma Nimesulide increases concentration of the medicines competing for communication with protein.

Contraindications:

— Hypersensitivity to Nimesulide and components of drug, acetylsalicylic acid or other NPVS,
— "aspirinovy" asthma,
— a peptic ulcer of a stomach and duodenum in a phase of an aggravation, bleeding from a GIT,
— the expressed abnormal liver functions and kidneys,
— pregnancy and feeding by a breast,
— children's age up to 12 years.

Cautions
It is necessary to be careful at purpose of Nimesulide to patients with renal failures, arterial hypertension, heart failure, vision disorders, a diabetes mellitus 2 types.

Overdose:

Cases of overdose of drug are not described. The specific antidote does not exist.
