General characteristics. Structure:

1 tablet contains chloroquine 250 mg, excipients: starch corn, silicon acid colloid, polyacrylic acid, magnesium stearate talc, поливинилбутирол.

Description: flat tablets with slanted edge, white or almost white color, inodorous.

Pharmacological properties:

Antiprotozoan means, has also immunodepressive and antiinflammatory effect. Causes death of sexless erythrocyte forms of all types of plasmodiums. Has gametocidal effect, except for Plasmodium falciparum (shows anti-gametocidal action).

Pharmacokinetics. After intake quickly and almost it is completely soaked up from digestive tract. The maximum concentration in blood is reached in 2-6 h. For 55% contacts plasma albumine. Quickly extends on bodies and body tissues (a liver, kidneys, a spleen, lungs). Easily gets through a blood-brain barrier and a placenta. It is metabolized in a small amount (25%). Indications to use:

Malaria (prevention and treatment of all types), abenteric amebiasis, amoebic abscess of a liver, system "lupus erythematosus" (hard currency) (chronic and subacute forms), pseudorheumatism, scleroderma, photodermatosis.

Route of administration and doses:

Inside, for prevention of malaria - on 0,5 g in the first week 2 times, then 1 time always in the same day of the week.

Treatment of malaria is carried out according to the following scheme: 1 day once 1 g, in 6-8 h – 0,5 g, for the 2 and 3 day of treatment - on 0,75 g daily in one step. At an amebiasis - on 0,5 g 3 times a day within 7 days, then - on 0,25 g 3 times a day within 7 more days, further - on 0,75 g 2 times a week within 2-6 months.

At a pseudorheumatism - on 0,25 g 2 times a day within 7 days, then - on 0,25 g daily within 12 months.

At hard currency - daily, on 0,25-0,5 g.

At a photodermatosis - on 0,25 g daily within a week, then - on 0,5-0,75 g weekly.

Features of use:

During treatment carrying out systematic surveys of the oculist and the general blood tests is necessary.

Side effects:

From the alimentary system: nausea, vomiting, gastralgia.

From a nervous system: dizziness, headache, sleep disorder, psychoses, epileptic seizures.

From cardiovascular system: damage of a myocardium with changes on the electrocardiogram (ECG), a lowering of arterial pressure.

From sense bodys: at prolonged use – opacification of a cornea, damage of a retina, a vision disorder, a ring in ears.

Allergic reactions: dermatitis, photosensitization.

Others: mialgiya, leukopenia, discoloration of skin of hair.

Interaction with other medicines:

Joint appointment with phenylbutazone, gold drugs, Penicillaminum, cytostatics, levamisole increases probability of an aplasia of marrow and damage of skin.

Antacids break absorption, Cimetidinum increases concentration of drug in blood. The combination to other antimalarial drugs can be followed by antagonistic effect.

At a combination to glucocorticosteroids the risk of development of a myopathy and cardiomyopathy increases, with inhibitors of a monoaminooxidase (MAO) – the risk of development of a neurotoxicity, with ethanol – a hepatotoxic increases, with cardiac glycosides – glikozidny intoxication accrues.

Contraindications:

Hypersensitivity, liver and/or renal failure, oppression of a marrowy blood formation, heavy disturbances of a rhythm, psoriasis arthritis, neutropenia, purpurinuria, pregnancy, lactation.

With care – deficit glyukozo-6-fosfatdegidrogenazy, a retinopathy, epilepsy.

Overdose:

Symptoms: vomiting, consciousness disturbance, a visual disturbance, spasms, a collapse, oppression of a respiratory center, up to death (during 2 h from the moment of respiratory depression). Treatment: a vomiting call, a gastric lavage, intake of absorbent carbon in the dose, by 5 times exceeding an estimated dose of chloroquine; peritoneal dialysis, plasma exchange.
