Fedin-20

General characteristics. Structure:

Active ingredient: piroxicam of 20 mg

Excipients: lactose, starch corn, methylhydroxybenzoate, propyl-hydroxybenzoate, silicon dioxide colloid.

Drug of anesthetic, antiinflammatory, febrifugal and anti-aggregation action.

Pharmacological properties:

Pharmacodynamics. Fedin-20 treats drugs of anesthetic, antiinflammatory, febrifugal and anti-aggregation action. It reduces prostoglandinovy synthesis (including synthesis PGE1, PGE2 and the PGE2-alpha), formation of thromboxanes at the expense of a supressirovaniye of cyclooxygenase activity. Also drug causes decrease in aggregation of thrombocytes, reduces activity of phagocytosis.

Fedin-20 reduces intensity of the pain and symptoms accompanying inflammatory processes. Drug perfectly stops a joint pain, both at rest, and at the movement, and removes a swelling of joints at a joint syndrome.

Pharmacokinetics. At oral administration of Fedin-20 it is actively soaked up in digestive tract. The maximum concentration of drug in an organism is observed in 3-5 hours. Equilibrium concentration in a blood plasma is reached on an extent from one to one and a half weeks. Drug well contacts plasma proteins (to 97-99%). The elimination half-life makes more than two days (the period can increase at patients, the suffering various disturbances of filtrational function of a liver).

Metabolism is carried out generally in a liver. The main metabolites - N-methyl-benzosulfonamid and 5 hydroxypiroxicam – do not belong to biologically active agents. Products of final metabolism are removed by kidneys and digestive tract. At the same time with urine twice more metabolites are allocated, than with a fecal masses – it is preferential in the form of glyukurnid. To 5% of metabolites it is removed in an invariable look.

Fedin-20 has ability to get into breast milk and through a placental barrier. At the same time drug has no property of cumulation.

Indications to use:

Fedin-20 it is recommended to use at degenerative and inflammatory diseases of a backbone and joints of extremities which are followed by emergence of a pain syndrome. Rheumatism, a pseudorheumatism, Bekhterev's disease, juvenile chronic arthritis, osteoarthroses, tendinites and a tendovaginitis, a tunnel and carpal syndrome, exacerbations of gout concern to them.

Drug at acute pain syndromes of other genesis is also effective, such as: mialgiya, neuralgia, effects of sports injuries in the form of stretchings, bruises and dislocations. Apply it also at the puffiness and a joint pain and muscles caused by an overstrain and also acute respiratory diseases of an infectious etiology.

Route of administration and doses:

Drug is recommended to be accepted orally during meal or right after it.

At purpose of course treatment of inflammatory or dystrophic diseases of joints it is recommended to accept Fedin-20 on 20 mg once in days. At an aggravation of a pseudorheumatism or rheumatism the dosage in 40 mg within one-two days, and then transition to a daily dosage to 20 mg is recommended.

At a bad attack of gout reception of 40 mg a day once within two days, then transition to a standard dosage to 20 mg twice a day within 4-6 days is recommended.

In case of stopping of a posttraumatic or post-operational pain syndrome it is necessary to accept 20 mg of drug once in days (in case of strong intensity of pains, it is possible to increase a dose to 40 mg a day in one or two receptions).

It is necessary to remember that anyway the maximum daily dosage makes 40 mg for adult patients.

Features of use:

With extra care it is necessary to appoint Fedin-20 in the presence of various cankers of digestive tract, bronchial asthma, chronic obstruction of respiratory tracts, an allergy to active components of drug, various forms of heart failure, and also other pathologies among which symptoms hypostases and arterial hypotension are registered.

During passing of a course of treatment it is necessary to carry out by drug regular control of cellular composition of blood, hepatic and renal function. Also during reception of Fedina-20 it is necessary to exclude completely alcohol intake in any form.

Effect of drug on patients of the nursery and a teenage age group was not investigated. It is not necessary to appoint Fedin-20 the patient of these age categories.

Side effects:

Nervous system: headaches, dizzinesses, an acrimony, a sonitus, sleep disorders (sleeplessness or drowsiness), the general low-expressed weakness, hallucinations, decrease in sight, irritation of eyes.

Cardiovascular system and blood: decrease or increase in the ABP, a heart consciousness, trombo-and a leukopenia, insignificant anemia, reduction of figures of a hematocrit and hemoglobin, emergence of hemorrhages is possible.

Digestive tract: dryness of mucous membranes of an oral cavity, irritation mucous a GIT, dispepsichesky frustration, an eructation, feeling of nausea, a meteorism, bleeding of gums, pains in a stomach, ulcers and erosion mucous a GIT, hepatic jaundice and disturbance of filtrational function of a liver (due to increase in activity of transaminases).

Urinogenital system: developing of intersticial nephrite, renal failure is in rare instances observed.

Other: allergic reaction to drug components (reddening of integuments, itch, skin rash, Stephens-Johnson's cm, Layell's cm, various anaphylactic reactions, photosensitization). Hypostases of the lower extremities - preferential at patients with heart failure, the increased perspiration, a hyperpotassemia, increase in concentration of urea, change of body weight are seldom observed.

Interaction with other medicines:

Fedin-20 forces out from a chemical bond with proteins of plasma other medicines. Also, drug lowers efficiency of action of various antihypertensives.

At use of Fedina-20 along with other drugs of the NPVS group the risk of formation of ulcerations mucous digestive tract considerably increases. In case of use along with kaliysberegayushchy diuretic drugs and other kaliysberegayushchy means increases chance of emergence of a condition of a hyperpotassemia.

Use of Fedina-20 increases the content of lithium and Phenytoinum in a blood plasma. As well as other anticoagulants, the risk of developing of bleedings strengthens. Reduces concentration of piroxicam in plasma to 80% of the concentration which was available before reception.

Contraindications:

Use of Fedina-20 is forbidden to the patients suffering from hypersensitivity to the non-steroidal anti-inflammatory drugs, persons having the expressed hepatic or renal dysfunction, hemorrhagic diathesis, inflammations or bleedings of a rectum or area of an anus, change of indicators of blood of not clear genesis.

Overdose:

The main symptoms of overdose of Fedina-20 are nausea, vomiting, drowsiness and unpleasant feelings in upper area of a stomach. Also gastric bleeding can take place. In rare instances the renal failure, difficulty of breath and anaphylactic reactions can be observed.

In this case it is necessary to start a gastric lavage and use of absorbent carbon or other sorbents immediately. It is important to carry out an artificial diuresis. In addition there can sometimes be necessary a carrying out a hemodialysis. When holding all listed actions it is necessary to monitor vital functions and if necessary to support them.

Storage conditions:

It is necessary to store Fedin-20 in the dry and protected from influence of solar ultraviolet place unavailable to access for children at temperature not higher than 30 grdus of Celsius.
