General characteristics. Structure:
Active ingredient: 5 g of an acyclovir.
Excipients: propylene glycol, vaseline, a liquid paraffin, wax emulsion, a macrogoal (poly(ethylene oxide) 1500), the water purified.
Pharmacological properties:
Pharmacodynamics. Antiviral drug — a synthetic analog of a nucleoside of thymidine. In the infected cells containing a virus thymidinekinase there is a phosphorylation and transformation in an acyclovir monophosphate. Under the influence of acyclovir guanylate cyclase monophosphate will be transformed to diphosphate and under action several cellular enzymes — to triphosphate. Sharp selectivity of action and a hypotoxicity for the person are caused by lack of necessary enzyme for formation of an acyclovir of triphosphate in intact cells of a macroorganism.
Acyclovir triphosphate, "being built in" in synthesized by the DNA virus, blocks reproduction of a virus. Specificity and very high selectivity of action are also caused by its preferential accumulation in the cells affected with a herpes virus. It is highly active concerning the type Herpes simplex 1 and 2 virus; the virus causing chicken pox and shingles (Varicella zoster); Epstein's virus - Barrel. It is moderately active concerning cytomegaloviruses.
Pharmacokinetics. At use on intact skin: absorption is minimum; is not defined in blood and urine. On the affected skin: absorption is moderate; at patients with normal function of kidneys concentration in blood serum makes up to 0,28 mkg/ml, at patients with the chronic renal failure (CRF) — to 0,78 mkg/ml. It is removed by kidneys (to 9,4% of a daily dose).
Indications to use:
The skin infections caused by the Herpes simplex virus of types 1 and 2, genital herpes, shingles, chicken pox.
Route of administration and doses:
Outwardly. Drug is applied with clean hands or a cotton plug of 5 times a day (each 4 hours) a thin layer on the sites of skin struck and adjoining on them. Therapy should be continued till that time while on bubbles the crust is not formed or until they completely begin to live.
Duration of therapy averages 5 days, at most — 10 days. In the absence of effect it is necessary to see a doctor.
Features of use:
For achievement of the maximum therapeutic effect it is necessary to use drug as soon as possible (at the first symptoms of a disease: burning, itch, pricking, feeling of tension and reddening).
Ointment is not recommended to be applied on mucous membranes of an oral cavity and eyes as development of the expressed local inflammation is possible. At treatment of genital herpes it is necessary to avoid sexual contacts or to use condoms as use of an acyclovir does not prevent transfer of a virus to partners.
Pregnancy and period of a lactation. Use of drug is shown only when the estimated advantage for mother exceeds potential risk for a fruit. During treatment it is necessary to resolve an issue of the breastfeeding termination.
Side effects:
Hyperemia, dryness, skin peeling; burning, an inflammation at hit on mucous membranes.
Development of allergic dermatitis is possible.
Interaction with other medicines:
At external use interaction with other medicines is not revealed.
Strengthening of effect is noted at co-administration of immunostimulators.
Contraindications:
Hypersensitivity to an acyclovir and other components of drug.
With care — pregnancy, the lactation period, dehydration, a renal failure.
Storage conditions:
In the dry, protected from light place, at a temperature not above 25 °C. To store in the place, unavailable to children. Not to use drug after a period of validity. A period of validity - 3 years.
