General characteristics. Structure:

Active ingredient: 25 g or 50 g of a dimethyl sulfoxide in 100 g of gel.

Antiinflammatory drug for external use.

Pharmacological properties:

Pharmacodynamics. Inactivates hydroxylic radicals, improves the course of metabolic processes in the inflammation center. Has the local anesthetizing and analgeziruyushchy effect. Has moderate antiseptic and fibrinolitic effects.

At topical administration quickly and well gets through biological membranes, including through skin, mucous, a cover of microbic cells (increases their sensitivity to antibiotics). Performs also transport function, providing deep penetration of other medicines through skin and mucous.

Pharmacokinetics. At application on skin and mucous membranes quickly and almost it is completely soaked up. Bioavailability makes 60-70%, and degree of bioavailability and penetration depth in direct ratio depend on concentration of a dimethyl sulfoxide.

At local drawing on skin it is found in blood serum in 10 minutes after application, the peak of concentration is observed in 4-8 hours. Well gets into cells of fabrics of all bodies, except for hair, nails and an adamantine substance of tooth. It is metabolized by oxidation in метилсульфонилметан, with the subsequent recovery to methyl thiomethane. The dimethyl sulfoxide and its metabolites are removed with urine and a stake. Indications to use:

Complex therapy of a pseudorheumatism, Bekhterev's disease, osteoarthrosis (in the presence of defeat of periartikulyarny fabrics), radiculitis, a scleroderma, a knotty erythema, an erysipelatous inflammation; bruises, sprains, inflammatory hypostases; infiltrates (including traumatic); purulent wounds, burns, trophic ulcers, thrombophlebitis, eczema, acne, complex therapy of a pyoderma, furunculosis; in skin and plastic surgery – replaced skin auto-and gomotransplantata, conservation of skin gomotransplantat; in stomatology – inflammatory diseases of maxillofacial area, sialadens, a periodontal disease, a pulpitis, periodontitis, arthritis and an osteoarthrosis of a temporal and mandibular joint.

Route of administration and doses:

Drug is used nakozhno, in the form of applications. On affected areas of skin apply with a thin layer 50% gel Dimexidum (on face skin and other especially sensitive body parts), and also on mucous membranes – 25% gel диметксид and easily pound.

Applications carry out 2-3 times a day within 10-15 days.

At eczema, diffusion streptodermas, and also for a local anesthesia at pain syndromes applications of 50% of gel in the form of compresses on affected areas 2-3 times a day are possible.

At thrombophlebitis – in combination with Unguentum Heparini of 100 PIECES or gel of heparin of 1000 PIECES in the form of simultaneous applications 2-3 times a day within 10-15 days.

In skin and plastic surgery use bandages from 25% gel Dimexidum on replaced skin auto-and gomotransplantata directly after operation and in the next days of the postoperative period before permanent engraftment of a transplant.

Features of use:

Considering a possibility of individual intolerance of drug, it is recommended to carry out medicinal test on portability. For this purpose gel Dimexidum is applied on the small site of skin of a dorsum of a wrist. Emergence of a sharp hyperemia and an itch testifies to hypersensitivity to drug.

Side effects:

Usually drug is well transferred, however at a part of patients emergence of pruritic dermatitis, an erythema, feeling of a xeroderma is possible. In isolated cases the bronchospasm is possible. Some patients badly perceive a drug smell that can be followed by nausea, desires on vomiting.

Interaction with other medicines:

Increases absorption and strengthens effect of ethanol, insulin. Increases absorption and increases effect of others jointly of applitsiruyemy medicines. Let's combine with heparin, antibacterial drugs, non-steroidal anti-inflammatory drugs. Increases sensitivity of microorganisms to aminoglycosides and beta лактамным to antibiotics, chloramphenicol, rifampicin, griseofulvin. Sensibilizes an organism to effect of medicines for the general and local anesthesia.

Contraindications:

Use of drug is contraindicated at hypersensitivity, a heavy liver and/or renal failure, stenocardia, the expressed atherosclerosis, glaucoma, a cataract, a myocardial infarction, a stroke, a coma, pregnancy and a lactation (during treatment it is necessary to stop breastfeeding).

Overdose:

Cases of overdose of drug at topical administration in the recommended doses are not described.
