Inosine in ampoules

General characteristics. Structure:

Acting veshchestvo:inosine;

1 ml of solution contains inosine 20 mg;

excipients: hexamethylenetetramine, 1 M hydroxide sodium solution, water for injections.

Pharmacological properties:

Pharmacodynamics. Inosine – anabolic drug, possesses anti-hypoxemic and antiarrhytmic action. He is a predecessor of ATP, directly participates in exchange of glucose and promotes activation of metabolism in the conditions of a hypoxia and in the absence of ATP. Drug activates metabolism of pyruvic acid for ensuring normal process of tissue respiration and promotes activation of a xanthinedehydrogenase. Inosine positively influences a metabolism in a myocardium, in particular increases power balance of cells, stimulates synthesis of nucleotides, increases activity of a number of enzymes of a tricarbonic acid cycle. Drug normalizes sokratitelny activity of a myocardium and promotes full relaxation of a myocardium in a diastole due to ability to connect calcium ions which got to cells during their excitement activates an angenesis (especially a myocardium and a mucous membrane of the digestive channel).

Pharmacokinetics. Privnutrivenny introduction inosine is quickly distributed in fabrics, metabolized in a liver where it is completely utilized in biochemical reactions of an organism. It is excreted preferential with urine.

Pharmaceutical characteristics.

Main physical and chemical properties: transparent colourless liquid.

Incompatibility. Drug should not be mixed with other medicines in one syringe or in one infusional system. To use only the recommended solvents.

Indications to use:

Complex treatment of coronary heart disease (a state after a myocardial infarction, stenocardia), disturbances of a heart rhythm, intoxication drugs of cardiac glycosides, treatment of cardiomyopathies of various genesis, myocardial dystrophies (against the background of heavy exercise stresses, infectious and endocrine genesis), myocardites; liver diseases (hepatitises, cirrhosis, fatty dystrophy of a liver); prevention of a leukopenia at radiation. For improvement of visual functions at otkrytouglovy glaucoma with the normalized intraocular pressure.

Route of administration and doses:

Apply intravenously kapelno or struyno. At first enter 200 mg (10 ml of 2% of solution)  once a day, further at good tolerance – to 400 mg (20 ml of 2% of solution) 1-2 times a day. The course of treatment is defined individually (on average 10-15 days).

At drop introduction to a vein of 2% solution of drug is dissolved in 5% solution of glucose or 0,9% of solution of sodium of chloride (to 250 ml). The drug is kapelno administered slowly, with a speed of 40-60 thaws of 1 minute. At acute disorders of a heart rhythm perhaps jet introduction in a single dose of 200-400 mg (10-20 ml 2%rastvora).

Side reactions. Patients with hypersensitivity to drug can have allergic reactions (an itch, a dermahemia, rash, urticaria). From warmly vascular system are possible: arterial hypotension, tachycardia. Manifestation of various local reactions, the general weakness is possible.

In rare instances at treatment can occur повышениеуровня uric acid in blood, at long treatment – an exacerbation of gout.

At manifestation of side reactions drug should be cancelled.

Features of use:

At a renal failure use of medicine is possible only when, according to the doctor, the expected positive effect exceeds possible risk at use.

During treatment it is regularly necessary to control the level of uric acid in blood.

Ability to influence speed of response at control of motor transport or work with other mechanisms. There are no data.

Side effects:

At manifestation of side reactions drug should be cancelled.

Interaction with other medicines:

At simultaneous use of Inosine with beta-blockers the effect of Inosine does not decrease. In combination with cardiac glycosides drug can prevent developing of arrhythmias and strengthen inotropic action. Inosine can strengthen effects of heparin, increasing duration of its action. Perhaps simultaneous use with nitroglycerine, nifedipine, furosemide, Spironolactonum. It is incompatible in one capacity from an alkaloidami:pra interaction there is department of a basis of alkaloid and formation of insoluble compounds. With a tannin forms a deposit. It is incompatible with acids and alcohols, salts of heavy metals. It is incompatible with B6 vitamin (a pyridoxine a hydrochloride) because of deactivation of both connections.

Contraindications:

The increased individual sensitivity to medicine. Gout, hyperuricemia. Restriction to use of Inosine is the renal failure.

Use during pregnancy or feeding by a breast. The research of efficiency and safety of use for this group of patients was not conducted.

Children. It is not necessary to apply to children due to the lack of data on safety of use.

Overdose:

The drug individualnayaneperenosimost in the form of an itch, dermahemias is possible (drug is cancelled and carry out the desensibilizing therapy).

Storage conditions:

Period of validity 3 years. In the place protected from light at a temperature not above 25 °C. To store in the place, unavailable to children.
