Inosine

General characteristics. Structure:

Active agent – inosine in terms of 100% substance of 100,0 mg.

Excipients – hexamethylenetetramine, 1M hydroxide sodium solution, water for injections.

Pharmacological properties:

Pharmacokinetics. After intravenous administration it is distributed in body tissues. It is metabolized in a liver with formation of glucuronic acid and the subsequent its oxidation. In insignificant quantity it is allocated with urine.

Pharmacodynamics. Inosine – derivative purine (nucleoside) – the predecessor of an adezintrifosfat (ATP). Treats group of the medicines stimulating metabolic processes. Has anti-hypoxemic and antiarrhytmic effect. Increases power balance of a myocardium, improves coronary circulation, prevents effects of intraoperative ischemia of kidneys. Is directly involved in exchange of glucose and promotes activation of exchange in the conditions of a hypoxia and in the absence of ATP. Stimulates synthesis of nucleotides, increases activity of some enzymes of a tricarbonic acid cycle. Activates metabolism of pyruvic acid. Reduces aggregation of thrombocytes, activates an angenesis.

Indications to use:

- coronary heart disease, including a myocardial infarction, stenocardia, coronary insufficiency (as a part of complex therapy)

- a cardiomyopathy of various genesis, the disturbances of a rhythm connected using cardiac glycosides, the inborn and acquired heart diseases, dystrophic changes of a myocardium after heavy exercise stresses and the postponed infectious diseases or owing to endocrine disturbances

- hepatitis, cirrhosis, including an alcoholic etiology (as a part of a combination therapy)

- ischemic kidney operations

Route of administration and doses:

Drug is used intravenously struyno slowly or kapelno (40-60 drops 1 minute). Treatment begin with introduction 200 mg (10 ml) once a day, then at good tolerance, the dose is increased to 400 mg (20 ml) by 1-2 times a day. Duration of treatment is 10-15 days.

Jet administration of drug is possible at acute disorders of a heart rhythm in a single dose of 200-400 mg (10-20 ml).

For pharmacological protection of the kidneys subjected to ischemia, inosine is entered intravenously struyno in a single dose of 1200 mg (60 ml) in 5-15 minutes prior to crossclamping of a renal artery, and then by 800 more mg (40 ml) immediately after blood circulation recovery.

At drop introduction to a vein, drug part in 5% solution of glucose or isotonic solution of sodium of chloride (up to 250 ml).

Use in pediatrics. To children appoint at the rate of 200-500 mg/days. Duration of a course of treatment is determined by the attending physician.

Features of use:

With care appoint drug at renal failures. At emergence of an itch and a dermahemia drug should be cancelled.

At pregnancy and in the period of a lactation inosine is appointed taking into account a ratio advantage/risk.

It is necessary to be careful at control of motor transport and potentially dangerous types of activity demanding the increased concentration of attention.

Side effects:

- tachycardia

- an exacerbation of gout (at prolonged use)

- hyperemia and itch of skin

- allergic reactions (small tortoiseshell, Quincke's disease, fever, asthma)

- lipodystrophy

Interaction with other medicines:

At use as a part of complex therapy Inosine promotes increase in efficiency of antiarrhytmic, anti-anginal and inotropic medicines.

At simultaneous use of Inosine with beta adrenoblockers the effect of Inosine does not decrease. In combination with cardiac glycosides drug can prevent developing of arrhythmias and strengthen inotropic action.

At simultaneous use with immunodepressants efficiency decreases.

Clinically significant interaction of Inosine with medicines of other groups is not described.

Inosine should not be mixed in one syringe with other medicines in order to avoid chemical incompatibility of drugs.

Contraindications:

- hypersensitivity to drug

- gout

- hyperuricemia

Overdose:

Symptoms - are possible allergic reactions.

Treatment - symptomatic, in hard cases it is necessary to see a doctor.

Storage conditions:

To store in the dry, protected from light place, at a temperature not above 30 °C.

To store in the place, unavailable to children!
