Description:
Balkan local nephropathy (Greek endzmos local; Greek nephros a kidney + pathos suffering, a disease) — the chronic disease of kidneys of noninflammatory character which is found in some regions of the Balkan Peninsula, endemic for it. It is slightly more often observed at women; at children and teenagers, and also does not occur among urban population.
Symptoms of the Balkan local nephropathy:
At the beginning of a disease weakness, fatigue, a loss of appetite, weight loss, sometimes pains in lumbar area and a hamaturia are observed. Skin is pale, a copper shade, on palms and soles of a ksantokhromn. Hypostases and arterial hypertension are absent. The insignificant proteinuria is noted. Further concentration ability of kidneys is sharply broken, the renal failure develops. The diagnosis is made taking into account duration of accommodation of the patient in the endemic area, a clinical picture and results of laboratory researches. The differential diagnosis is foreseen with chronic pyelonephritis, a glomerulonephritis and a medicinal nephropathy.
Reasons of the Balkan local nephropathy:
Attempts to prove an infectious etiology of a disease did not yield results. The hypothesis of slow chronic intoxication heavy metals is more plausible. Influence of deficit of some microelements is possible. The probability of genetic conditionality of a disease is studied. Consider that the etiological factor possesses nephrotoxic and oncogenous action on the uric ways. The role of immune mechanisms is not excluded.
Treatment of the Balkan local nephropathy:
Treatment is generally symptomatic. In the period of a chronic renal failure recommend treatment by a low-proteinaceous caloric diet without salt restriction, folic acid, anabolic steroids, transfusion of eritrotsitny weight. In an end-stage of a renal failure resort to a hemodialysis and transplantation of a kidney.
