Vinpocetine Forte

General characteristics. Structure:

Active ingredient: 10 mg of a Vinpocetine.

Excipients: magnesium stearate, silicon dioxide colloid anhydrous, potato starch, lactose monohydrate.

Medicine with выраженнм vascular and metabolic action.

Pharmacological properties:

Pharmacodynamics. Active ingredient of the Vinpocetine is the Vinpocetine - semi-synthetic derivative alkaloid of Devincanum which contains in a periwinkle small. Combines vascular and metabolic action. Drug inhibits activity of enzyme of phosphodiesterase and increases the maintenance of tsAMF in cells of unstriated muscles of vessels of a brain, reduces the content of calcium in cytoplasm of smooth muscle cells of vessels.

Thanks to it, under the influence of drug brain vessels extend, cerebral circulation improves, it is preferential in ischemic sites, supply of a brain with oxygen improves.

The Vinpocetine promotes utilization of glucose and increases the level of catecholamines in the central nervous system, stimulates metabolism of noradrenaline and serotonin in brain tissues. Reduces aggregation of thrombocytes, viscosity of blood, contributes to normalization of venous outflow against the background of decrease in vascular resistance of a brain. Under the influence of drug system arterial pressure decreases in insignificant degree. In the acute period of an ischemic stroke drug accelerates regress of all-brain and focal symptomatology, improves memory, attention, intellectual productivity.

In old and senile age sensitivity of vessels of a brain to action of a Vinpocetine increases that is caused by a sensitization when aging system of adenylatecyclase-tsAMF.

Pharmacokinetics. The Vinpocetine is quickly soaked up in a digestive tract. Bioavailability makes about 60%. Optimum concentration in plasma makes 10-20 ng/ml. The maximum concentration is reached in 1 hour. Drug easily gets through gistogematichesky barriers, including a blood-brain barrier, and gets into fabrics. Indications to use:

· Neurology: a symptomatic treatment of the cognitive disturbances connected with cerebrovascular diseases (the recovery period after an ischemic stroke, the tranzitorny ischemic attack, an effect of an ischemic stroke, distsirkulyatorny encephalopathy). Before an initiation of treatment it is necessary to exclude the degenerative diseases of a nervous system which are not connected with vascular pathology (for example, Alzheimer's disease).

· Ophthalmology: chronic vascular diseases of a choroid and retina of an eye.

· Otology: Decrease in auditory acuity of perceptual character, Menyer's disease, a ring in ears.

Route of administration and doses:

The Vinpocetine is accepted Forte inside on 5-10 mg by 3 times a day within no more than 30 days, further on 5 mg 3 times within 2 months. Possible advantages of longer reception of medicine are not known.

Pill should be taken during food, without chewing.  The tablet Vinpocetine Forte is not intended for division into parts. For achievement of a necessary dose we recommend to use Vinpocetine medicine, tablets of 5 mg of production of JV LLC Pharmlend.

To patients with diseases of kidneys appoint the above-stated doses.

To patients with a liver failure appoint with care.

Features of use:

Use for persons with cardiovascular pathology. It is necessary to appoint carefully to patients with cardiac arrhythmia. It is recommended to avoid as far as possible administration of drug to patients with the increased QT interval since the risk of arrhythmia increases. To appoint with care to the patients accepting the antihypertensives or medicines increasing QT interval (лоратадин, терфенадин, erythromycin, probucol, etc.), and also to patients with the hypersensitivity, known from the anamnesis, to the drugs containing active ingredient similar to a Vinpocetine (alkaloids of the Periwinkle of small (Vincaminor)).

It is not recommended to appoint a Vinpocetine with a labile arterial pressure and a low vascular tone.

Use in pediatrics. Safety and efficiency of use of a Vinpocetine for children is not established. Experience of use of a Vinpocetine for patients aged up to 18 years is limited. Use for children up to 18 years is not recommended due to the lack of adequate data.

Use for persons with an abnormal liver function and kidneys. It is not necessary to appoint medicine the Vinpocetine to Florta to patients with liver diseases with the expressed dysfunction.

Due to the content of lactose medicine the Vinpocetine is Forte not recommended to patients with rare inborn intolerance of a galactose, deficit of lactase of Lapp or malabsorption of glucose galactose.

Use during pregnancy and feeding by a breast. Medicine the VINPOCETINE FORTE should not be accepted during pregnancy and during feeding by a breast.
 
Influence on ability to driving of motor transport and control of mechanisms. Reception of medicine can break reaction that it is capable to reflect adversely on management of transport and work on the machine equipment.

The Vinpocetine Forte should not be applied during control of the vehicle or other operator activity.

Side effects:

Possible side reactions are listed below with the indication of frequency of occurrence: infrequently ≥1/1000 to <1/100, it is rare ≥1/10000 to <1/1000, is very rare <1/10000).

From a nervous system. Infrequently: headache; seldom: concern, paresthesias.

Mental disorders. Seldom: sleep disorder, excitement, concern.

From an acoustic organ and vestibular mechanism. Infrequently: dizziness.

From cardiovascular system. Seldom: tachycardia, premature ventricular contraction, ST segment depression, increase in excitability of a myocardium, lengthening of an interval of QT. Infrequently: lowering of arterial pressure.

From digestive tract. Seldom: abdominal pains, nausea; very seldom: gastrointestinal frustration, dyspepsia.

From skin and hypodermic cellulose. Infrequently: skin rash or itch. Allergic reactions are possible.

Interaction with other medicines:

Interaction with anti-hypertensive, antiarrhytmic and anticoagulating medicines is possible. In case of need co-administration of a Vinpocetine with the specified medicines it is necessary to watch carefully a condition of the patient, control of arterial pressure, an ECG, indicators of system of a blood coagulation is recommended.

Contraindications:

- hypersensitivity to a Vinpocetine or to any of drug components.
- the postponed intracerebral bleeding;
- the increased intracranial pressure;
- heavy disturbances of a cordial rhythm;
- severe forms of coronary heart disease;
- period of pregnancy and lactation.

Overdose:

Information on cases of overdose is absent. In case of overdose general measures on removal of medicine from an organism, the symptomatic and supporting treatment directed generally are recommended for maintenance of functions of heart and lungs.

Storage conditions:

To store in the place protected from moisture and light at a temperature is not from +15 °C to +25 °C higher. To store in the place, unavailable to children. Period of validity 3 years.
