General characteristics. Structure:

International and chemical names: vinpocetine; [(3α, 16α of)-eburnamenin-14-carboxylic acid ethyl ether]; main physical and chemical properties: colourless or with slightly greenish shade transparent liquid; structure: 1 ml of solution contains a Vinpocetine 5 mg; excipients: ascorbic acid, sodium metabisulphite, tartaric acid, benzilovy alcohol, sorbitol, water for injections.

Pharmacological properties:

Pharmacodynamics. The Vinpocetine has the complex mechanism of action, exerts positive impact at disbolism in TsNS, disturbance of cerebral blood circulation and disturbance of microcirculation. Drug has tserebroprotektorny action - improves metabolism in a brain, strengthens absorption of glucose and oxygen; increases portability of a cerebral hypoxia; increases concentration of tsAMF in brain tissue; strengthens metabolism of noradrenaline and serotonin; shows antioxidant activity.
The Vinpocetine improves cerebral microcirculation, inhibits aggregation of thrombocytes; reduces patholologically the increased viscosity of blood; strengthens deformation of erythrocytes and slows down absorption of adenosine by them, promotes transportation of oxygen to fabrics.
The Vinpocetine selectively strengthens cerebral circulation, reduces brain vascular resistance.

Pharmacokinetics. At parenteral administration of a Vinpocetine the volume of distribution makes 5,3 l/kg. An elimination half-life - about 5 hours. The Vinpocetine gets through a gistogemato-logical barrier.

Indications to use:

Acute and chronic insufficiency of cerebral circulation (transit ischemia, a state after the had ischemic stroke, cerebral atherosclerosis, posttraumatic and hypertensive encephalopathy); the mental and neurologic disturbances connected with cerebral insufficiency: memory disturbance, apraxia, aphasia, dizziness, headache, motive disturbances; diseases of a choroid and retina, macula lutea, arterial and venous fibrinferments and embolisms, secondary glaucoma; senile glukhovatost, Menyer's disease, kokhleovestibulyarny neuritis.

Route of administration and doses:

The Vinpocetine is entered intravenously kapelno the adult on 20 mg (2 ampoules on 5 mg/ml) dissolved in 500 ml of isotonic solution of sodium of chloride. The drug in the first two days is administered 1 time a day on 20 mg, further at satisfactory portability of drug it is possible to increase a dose within 2-3 days no more than to 1 mg/kg of body weight a day. Duration of use of drug averages 10 - 14 days.
The highest single dose of drug - 60 mg.
The highest daily dose of drug - 1,5 mg/kg.

Features of use:

With care use drug at arrhythmia, stenocardia, unstable arterial pressure and a reduced vascular tone. At a hemorrhagic cerebral stroke parenteral administration of a Vinpocetine is allowed only after reduction of the acute phenomena - usually in 5 - 7 days. Parenterally, as a rule, apply in acute cases to improvement of a clinical picture then pass to administration of drug in the form of tablets. Hypodermic administration of drug is not allowed.

Side effects:

Passing arterial hypertension, sleeplessness, dizziness, headache, weakness, dryness in a mouth, nausea, ST segment depression, lengthening of an interval of QT, tachycardia, phlebitis, a dermahemia is possible; seldom - premature ventricular contraction, increase in time of excitement of ventricles.

Interaction with other medicines:

At simultaneous parenteral use of a Vinpocetine and heparin the risk of development of bleedings increases. The Vinpocetine can strengthen hypotensive effect Methyldopum.

Contraindications:

Acute stage of a hemorrhagic stroke, the expressed coronary heart disease, severe forms of arrhythmias, syndrome of increase in an interval of QT, pregnancy, the feeding period a breast, hypersensitivity to drug.
There is no enough data on use of a Vinpocetine at children's age.

Overdose:

The overdose of a Vinpocetine can be followed by strengthening of side effects of drug. Treatment consists of use of hypotensive and antiarrhytmic means under control of the doctor.

Storage conditions:
