Analginum

General characteristics. Structure:

Active ingredient: 500 mg of metamizole sodium.

Excipients: potato starch; sugar; calcium stearate; talc.

The non-narcotic analgetic drug rendering analgeziruyushchy, febrifugal and some spasmolytic (concerning smooth muscles of urinary and bilious tract) action.

Pharmacological properties:

Pharmacodynamics. Analgetic antipyretic. Is derivative pyrazyl ketone. Has analgetic, febrifugal and antiinflammatory effect which mechanism is connected with oppression of synthesis of prostaglandins.

Pharmacokinetics. After intake metamizole sodium is quickly hydrolyzed in a gastric juice with formation of an active metabolite of 4-methyl-amino-antipyrine which after absorption is metabolized in 4-formyl-amino-antipyrine and other metabolites. Later in/in introductions metamizole sodium quickly becomes unavailable to definition.

Metabolites of metamizole sodium do not contact proteins of plasma. The greatest part of a dose is removed with urine in the form of metabolites. Metabolites are allocated with breast milk.

Indications to use:

Pain syndrome of various genesis (renal and bilious colic, neuralgia, mialgiya; at injuries, burns, after operations; headache, dentagra, menalgiya). Fever at infectious and inflammatory diseases.

Route of administration and doses:

Inside or rektalno the adult appoint 250-500 mg of 2-3 times/days. The maximum single dose - 1 g, daily - 3 g. Single doses for children at the age of 2-3 years make 50-100 mg; 4-5 years - 100-200 mg; 6-7 years - 200 mg; 8-14 years - 250-300 mg; frequency rate of reception - 2-3 times/days.

In oil or in/in slowly adult - 250-500 mg of 2-3 times/days. The maximum single dose - 1 g, daily - 2 g. At children parenterally apply in a dose 50-100 mg on 10 kg of body weight.

Features of use:

At prolonged use it is necessary to control a pattern of peripheral blood.

Metamizole sodium is applied in a combination with pitofenony and a fenpiveriniya bromide as analgetic means with spasmolytic action.

With care to apply at children in the first 3 months of life.

Single doses for children at the age of 2-3 years make 50-100 mg; 4-5 years - 100-200 mg; 6-7 years - 200 mg; 8-14 years - 250-300 mg; frequency rate of reception - 2-3 times/days.

Side effects:

Allergic reactions: skin rash, Quincke's edema; seldom - an acute anaphylaxis.

From system of a hemopoiesis: seldom, at prolonged use - a leukopenia, an agranulocytosis.

Local reactions: at introduction in oil - infiltrates in an injection site.

Interaction with other medicines:

At simultaneous use with analgetics antipyretics, with NPVS perhaps mutual strengthening of toxic effects.

At simultaneous use with inductors of microsomal enzymes of a liver reduction of efficiency of metamizole sodium is possible.

At simultaneous use activity of indirect anticoagulants, peroral hypoglycemic drugs, GKS, indometacin owing to their replacement from communication with blood proteins under the influence of metamizole sodium amplifies.

At simultaneous use with derivatives of a fenotiazin the expressed hyperthermia is possible; with sedatives, anxiolytics - analgeziruyushchy effect of metamizole sodium amplifies; with tricyclic antidepressants, oral contraceptives, Allopyrinolum - metabolism of metamizole sodium is broken and its toxicity increases; with caffeine - effect of metamizole sodium amplifies; with cyclosporine - concentration of cyclosporine in a blood plasma decreases.

At use of metamizole sodium in a combination from a pitofenon a fenpiveriniya bromide (m-holinoblokator) mutual strengthening of their pharmacological action happens a hydrochloride (has direct myotropic effect on smooth muscles of internals and causes its relaxation) and with that is followed by reduction of a pain syndrome, relaxation of unstriated muscles and decrease in the increased body temperature.

Contraindications:

The expressed renal failures and/or a liver, deficit glyukozo-6-fosfatdegidrogenazy, blood diseases, hypersensitivity to pyrazyl ketone derivatives.

Overdose:

Symptoms: nausea, vomiting, pains in epigastriums, vestibular disturbances, a sonitus, clonic and tonic spasms, coma, an agranulocytosis, aplastic anemia, hemorrhagic diathesis, skin rash, manifestations of cardiotoxic action.

Treatment: a gastric lavage, standard detoksitsiruyushchy actions, if necessary - symptomatic therapy. 

Storage conditions:

To store in the unavailable to children, protected from light place at a temperature not above 30 °C.
