Doxycycline hydrochloride

General characteristics. Structure:

Active ingredient: 100 mg of doxycycline of a hydrochloride in 1 capsule.

Doxycycline the hydrochloride possesses a wide range of antimicrobic action.

Pharmacological properties:

Pharmacodynamics. Inhibits synthesis of proteins in a microbic cell, breaking communication transport aminoacyl-RNA with 30S in subunit of a ribosomalny membrane. Has bacteriostatic action.

Drug is effective concerning the majority of gram-positive and gram-negative microorganisms, including mycoplasmas, chlamydias, rickettsiae, iyersiniya, brucellas, vibrioes, bacteroids, clostridiums, fuzobakteriya, the elementary (a malarial plasmodium, an amoeba, etc.). Gets in a cell, suppressing reproduction of intracellularly located activators.

Pharmacokinetics. At doxycycline intake the hydrochloride is quickly soaked up; bioavailability reaches 95%. Effective concentration of an antibiotic at this way of introduction remain within 18 — 24 hours. Gets into the majority of fabrics and liquids of an organism, into bile, exudate of Highmore's and frontal sinuses, pleural, synovial, astsitny liquids. Collects in bones, teeth, a liver, a spleen, a prostate gland. Creates therapeutic concentration in eye tissues. Out of an organism it is brought generally with bile (90%), and also with urine. Indications to use:

Doxycycline is appointed to adults and children 8 years at the diseases caused by microorganisms, sensitive to his action, are more senior. Apply at rickettsioses, a borellioza, a brucellosis, an iyersinioza, bacillar and amoebic dysentery, a tularemia, cholera, Lyme's disease, malaria, a granulotsitarny erlikhioz; at diseases of ENT organs and lower parts of respiratory tracts (sinusitis, otitis, tonsillitis, bronchitis, pneumonia, pleurisy), infections zhelche-and urinary tract, at urogenital mycoplasmosis, gynecologic infectious diseases (an endometritis, an endocervicitis), acute prostatitis, an epididymite, gonorrhea, purulent infections of skin and soft tissues (phlegmons, abscesses, a furunculosis, an acne, etc.), at an infectious ulcer keratitis, for prevention of a surgical infection after medical abortions, a large intestine operations, for treatment of postoperative purulent complications; complications after viral infections, for prevention of malaria, etc. It is applied in treatment of syphilis as alternative drug in case of lack of effect of benzylpenicillin or an allergy to drugs of penicillinic group.

Route of administration and doses:

Apply inside in time or after food. The daily dose for adults in the first day of treatment makes 0,2 g. It is entered in one step or on 0,1 g with an interval of 12 hours. In the next days apply 0,1 g in a daily dose. At the heavy course of an infection the daily dose in the first day and all next days of treatment makes 0,2 g. To children from 8 to 12 years of doxycycline the hydrochloride is appointed in the first day of treatment at the rate of 4 mg/kg (in the form of the single dose or two equal doses accepted at an interval of 12 hours), in the next days on 2 mg/kg, at the heavy course of an infection — 4 mg/kg daily. To children 12 years are more senior appoint in a dose for adults.

Drug is used usually within 7 — 10 days.

At treatment of gonorrhea appoint by a sick acute uncomplicated urethritis in a course dose 0,5 g (the first reception — 0,3 g, the subsequent two receptions — on 0,1 g with an interval of 6 hours); at the complicated gonorrhea forms the course dose of an antibiotic (0,8 — 0,9 g) is distributed on 6 — 7 introductions (0,3 g on the first reception, on 0,1 g with an interval of 6 hours — on 5 — 6 of the subsequent).

At an acne disease appoint 0,05 g a day for 6 — 12 weeks.

At primary or secondary syphilis apply 0,3 g a day in stages within 10 (not less) days.

Features of use:

For prevention of ulcerations of a gullet it is necessary to accept with a large amount of liquid. During treatment and within 4 — 5 days after it contraindicated direct insolation or UV rays.

Side effects:

Gastrointestinal frustration, irritation of mucous membranes, allergic reactions, photosensitization, deterioration in a current of a lupus erythematosus, steady discoloration of an adamantine substance of tooth, hemolitic anemia, thrombocytopenia, neutropenia, tranzitorny increase in content in blood of hepatic transaminases, alkaline phosphatase, bilirubin, residual nitrogen are possible; at chronic ingestion — dysbacteriosis, superinfection drozhzhepodobny mushrooms.

Interaction with other medicines:

It is not necessary to combine doxycycline with the means containing ions of metals (the antacids containing salts of aluminum, calcium and magnesium; the drugs containing ions of manganese, zinc, iron preparations) in connection with formation of inactive chelates.

Absorption of doxycycline oppresses also холестирамин.

Barbiturates, carbamazepine, Phenytoinum reduce concentration of doxycycline in serum.

Doxycycline reduces efficiency of penicillin, oral contraceptives; at joint appointment with oral contraceptives the probability of bleeding in a cycle increases.

At simultaneous therapy careful control of a blood coagulation (against the background of reception of tetracyclines vitamin K synthesis decreases) is required by anticoagulants.

Contraindications:

Use of doxycycline at hypersensitivity to tetracyclines is contraindicated, to a heavy renal failure, during pregnancy, to a lactation (for treatment it is necessary to refuse breastfeeding), children up to 8 years (the period of development of teeth).
