Bicillinum-3

General characteristics. Structure:

Active ingredient: Bicillinum-3, powder of 600 000 PIECES (mix of a benzatin of benzylpenicillin of sterile (Bicillinum-1) of 200 000 PIECES, benzylpenicillin of sodium salt of sterile 200 000 PIECES, benzylpenicillin of novocainic salt of sterile 200 000 PIECES).

Pharmacological properties:

At simultaneous use diuretics, Allopyrinolum, phenylbutazone, NPVS reduce canalicular secretion and increase concentration of penicillin.

Pharmacodynamics. Drug has bactericidal effect by inhibition of synthesis of mukopeptid of a cellular cover of microorganisms. Gram-positive microorganisms are sensitive to Bicillinum: Staphylococcus spp. (except those which produce a penicillinase), Streptococcus spp. (including Streptococcus pneumoniae), Corynebacterium diphtheriae, Bacillus anthracis; gram-negative: Neisseria
gonorrhoeae, Neisseria meningitidis, Actinomyces israelii, and also Treponema spp., anaerobic spore-forming sticks. To effect of drug of a rack the majority of gram-negative bacteria, mycobacteria, mushrooms, viruses, the elementary.

Pharmacokinetics. Bicillinum – a form of benzylpenicillin of the prolonged action. At intramuscular introduction drug forms depot in muscular tissue. During the first hours after an injection the high level of benzylpenicillin in blood is formed. Bicillinum is well distributed in fabrics and liquids of an organism. Drug appears in high concentration in a liver, kidneys, lungs, mucous membranes, has ability to get into fibrinous fabrics. Bicillinum is preferential removed with urine in biologically active form (50-70%), in insignificant quantities – with saliva,
then, milk, bile.
Pharmaceutical characteristics.
Main physical and chemical properties: powder of color, white or white with slightly yellowish shade.
Incompatibility. It is not necessary to mix Bitsillina®-3 suspension with other injection solutions.

Indications to use:

The infections caused by microorganisms, sensitive to drug, in cases when long support of constant concentration of medicine in blood is necessary:
– respiratory infection and ENT organs: streptococcal pharyngitises and tonsillitis;
– infections of urinogenital system: syphilis;
– erysipelatous inflammation;
– exacerbation of rheumatism.

Route of administration and doses:

Drug is appointed only the adult! Before introduction it is necessary to carry out a preliminary intracutaneous test on portability of drug! Бициллин®-3 enter only intramusculary! Intravenous administration of drug is forbidden!
Suspension is prepared sterilely, just before use. Add to a bottle 5-6 ml of 0,9% of solution of sodium of chloride or sterile water for injections, or 0,25-0,5% of solution of novocaine, mix before formation of uniform suspension and enter deeply intramusculary into an upper external quadrant of a gluteus (2 injections do in different buttocks). Drug the adult in a dose of 600 000 PIECES is entered 1 time in 6 days. Usually duration of treatment makes from 3 to 12 months depending on disease severity.
Before administration of Bitsillina®-3 suspension it is necessary to be convinced that the needle did not get to a blood vessel. If there is blood, the needle needs to be extended and to carry out an injection to other place.

Features of use:

Before an initiation of treatment of Bitsillinom®-3 patients have to be checked for portability of drug. It is necessary to find out whether reactions to drugs of group of penicillin at their previous use were noted. To the patients inclined to allergic reactions, drug is appointed with care. At
emergence of signs of allergic reactions of Bitsillin®-3 it is necessary to cancel and appoint the corresponding therapy. Ability to influence speed of response at control of motor transport or
to work with other mechanisms. It is recommended to refrain from activity which demands speed of psychomotor reactions (management of transport, work with potentially dangerous mechanisms), including possible undesirable reactions from a nervous system (dizziness, a headache, a sonitus).

Side effects:

Hypersensitivity reactions: skin rashes, itch, fever, fever, arthralgia, hypostases, urticaria, mnogoformny exudative erythema, exfoliative dermatitis, Quincke's disease of Quincke, acute anaphylaxis.
From a respiratory organs: bronchospasm.
From the central and peripheral nervous system: dizziness, headache, sonitus.
From a digestive tract: nausea, stomatitis, glossitis, diarrhea pseudomembranous colitis, abnormal liver function.
From urinogenital system: intersticial nephrite.
From cardiovascular system: fluctuations of arterial pressure, disturbance of pumping function of a myocardium.
From blood and lymphatic system: eosinophilia, positive takes of the test of Koombs, hemolitic anemia, leukopenia, thrombocytopenia, agranulocytosis.
Others: the weakened patients, newborns, elderly people at prolonged treatment can have a superinfection caused by microflora, steady against drug (drozhzhepodobny mushrooms, gram-negative microorganisms). Patients who receive medical treatment concerning syphilis can also have Yarisha-Gerksgeymer's reaction for the second time to a bacteriolysis.

Interaction with other medicines:

At simultaneous use with bactericidal antibiotics (including cephalosporins, Cycloserinum, Vancomycinum, rifampicin, aminoglycosides) an action synergism is noted; with bacteriostatic antibiotics (including macroleads, chloramphenicol, linkozamida, tetracycline) – antagonism.
Бициллин®-3 reduces efficiency of oral contraceptives and ethinylestradiol (risk of development of breakthrough bleedings). It is not recommended to combine with non-steroidal anti-inflammatory drugs. Benzylpenicillin lowers clearance and increases toxicity of a methotrexate. At simultaneous use of Bitsillina®-3 with Allopyrinolum the risk of development of allergic reactions increases.

Contraindications:

Hypersensitivity to β-laktamny antibiotics (penicillin, cephalosporins, karbapenema) and novocaine; hay fever; bronchial asthma; allergic diseases.

Use during pregnancy or feeding by a breast. Contraindicated.
Children. Efficiency and safety of use of medicine to children were not investigated.

Overdose:

Perhaps toxic action on the central nervous system, namely: reflex excitement, headache, nausea, vomiting, spasms, mialgiya, arthralgia, symptoms of a meningism, lump. In such cases administration of drug should be stopped.
Treatment: symptomatic which includes a hemodialysis it is necessary to pay peritoneal dialysis, special attention to recovery of water and electrolytic balance.

Storage conditions:

Period of validity 4 years. To store in original packaging, at a temperature not above 25 °C. To store in the place, unavailable to children.
