Hydrocortisone

General characteristics. Structure:

Active agent: Hydrocortisoni acetas – 1,0 g. Excipients: vaseline medical, lanolin (lanolin anhydrous), pentaerythrityl dioleate (pental), stearic acid, methylparahydroxybenzoate (нипагин or methylparaben), пропилпарагидроксибензоат (нипазол or propylparaben), the water purified.

Pharmacological properties:

The hydrocortisone is synthetic glucocorticosteroid drug for external use. Has antiinflammatory, antiedematous, antipruritic effect. Slows down release of cytokines (interleykin and interferon) from lymphocytes and macrophages, release by eosinophils of mediators of an inflammation oppresses, breaks metabolism of arachidonic acid and synthesis of prostaglandins. Stimulating glucosteroid receptors, induces formation of a lipokortin. Reduces inflammatory cellular infiltrates, reduces migration of leukocytes, including lymphocytes to the area of an inflammation. At appointment in the recommended doses does not cause system side effects.

Pharmacokinetics. After application collects in epidermis (generally in a granular layer). It is metabolized directly in epidermis, and further in a liver. It is allocated with kidneys and intestines.

Indications to use:

Inflammatory and allergic diseases of skin of not microbic etiology (including followed by an itch): eczema, allergic and contact dermatitis, neurodermatitis, psoriasis, erythrosis, prurigo, stings of insects, seborrhea.

Route of administration and doses:

Outwardly. Ointment is applied a thin layer on affected areas of skin by 2-3 times a day. Duration of a course of treatment depends on the nature of a disease and efficiency of therapy, making, as a rule, 6-14 days; at persistent disease it can be prolonged up to 20 days. On the limited centers for strengthening of effect it is possible to use occlusive bandages. At use of ointment for children of 2 years is also more senior it is necessary to limit the general duration of treatment to term - no more than two weeks and to exclude the actions leading to strengthening of absorption of a steroid (warming, fixing and occlusive bandages).

Features of use:

In cases of use of drug for children, or under occlusive bandages duration of a course of treatment needs to be reduced by area of the person up to 2 weeks.

At children's age suppression of function of bark of adrenal glands can quicker develop. Besides, decrease in secretion of a growth hormone can be observed. At drug use it is necessary to watch the body weight, growth, concentration of cortisol of plasma for a long time. To children up to 12 years drug is appointed only under medical control.

It is necessary to avoid hit of ointment in eyes and mucous membranes. If after 7 days of use there does not occur improvement or the aggravation of symptoms is observed, use of drug it is necessary to stop and see a doctor. In case of resuming of symptoms in several days after drug withdrawal it is necessary to consult with the doctor.

At prolonged treatment and/or drawing it is recommended to appoint a diet with restriction of sodium and the increased content of potassium to big surfaces; to enter enough protein into an organism.

It is necessary to control the arterial pressure, content of glucose in blood, coagulability of blood, a diuresis, body weight of the patient, plasma cortisol level.

For the prevention of infectious damages of skin drug is recommended to appoint in combination with antibacterial and antifungal agents.

Side effects:

Hyperemia, puffiness and itch in ointment sites of application.

At prolonged use also development of secondary infectious damages of skin, atrophic changes in it, a hypertrichosis is possible.

At prolonged use of ointment or use of an occlusive bandage, especially on big sites of defeat, development of symptoms of a hypercorticoidism (a hyperglycemia, a glucosuria, reversible oppression of function of bark of adrenal glands, manifestation of a syndrome of Cushing) as manifestation of resorptive action of a hydrocortisone is possible.

Contraindications:

Hypersensitivity to a hydrocortisone or other components of drug, wound, skin cankers, bacterial, viral and fungus diseases of skin, a tuberculosis cutis and syphilitic damages of skin.
Rozatsea, vulgar eels, perioral dermatitis, skin tumors, children's age up to 2 years. Pregnancy. 
Use at pregnancy is not specified.

Overdose:

The acute overdose is improbable, however, at excessive or prolonged use of drug the chronic overdose which is followed by hypercorticoidism signs is possible: hyperglycemia, glucosuria, reversible oppression of function of bark of adrenal glands, manifestation of a syndrome of Cushing.

Treatment. The corresponding symptomatic treatment is shown. In case of chronic toxic action gradual drug withdrawal is recommended.

Storage conditions:

To store at a temperature from 8 °C to 15 °C. To store in the place, unavailable to children. A period of validity - 3 years. Not to apply after a period of validity.
