Mildrokard

General characteristics. Structure:

Active ingredient: 250 mg of meldoniya.

Pharmacological properties:

Pharmacodynamics. Mildrokard - an analog of a gamma butyrobetaine, the predecessor of a carnitine. The mechanism of action of a mildrokard defines variety of its pharmacological effects: increase in working capacity, reduction of symptoms of a mental and physical overstrain, activation of fabric and humoral immunity, cardiotyre-tread action. In case of acute ischemic injury of a myocardium slows down formation of a necrotic zone, shortens the rehabilitation period.

At heart failure increases contractility of a myocardium, increases tolerance to an exercise stress, reduces the frequency of attacks of stenocardia. At acute and chronic ischemic disorders of cerebral circulation improves blood circulation in the ischemia center, promotes redistribution of blood in favor of the ischemic site. Mildrokard is effective in case of vascular and dystrophic pathology of an eyeground. Has tonic effect on TsNS, eliminates functional disturbances of somatic and vegetative nervous systems at alcoholics in the period of abstinence.

Pharmacokinetics. At intake it is well soaked up. Bioavailability makes about 78%. The maximum concentration in a blood plasma is reached approximately in 1-2 hours after reception. It is metabolized in an organism with formation of 2 main metabolites which are removed by kidneys. The elimination half-life makes 3-6 h and depends on a dose.

Indications to use:

• the lowered working capacity, a physical overstrain (including at athletes);

• the postoperative period for rehabilitation acceleration;

• as a part of complex therapy: An ischemic heart disease (stenocardia, a myocardial infarction), chronic heart failure, a cardialgia against the background of dishormonal dystrophy of a myocardium; disturbances of blood supply of a brain (stroke, cerebrovascular insufficiency)

• an abstinence alcoholic syndrome (in a combination with specific therapy).

Route of administration and doses:

At intellectual and physical overworks (including at athletes) the recommended dose makes 1000 mg a day (on 250 mg (1 capsule) 4 times a day or on 500 mg (2 capsules) 2 times a day). A course of treatment - 10-14 days. If necessary treatment is repeated in 2-3 weeks.

To athletes - on 500-1000 mg (2-4 capsules) 2 times a day before trainings. Course duration during the preparatory period - 14-21 days, during competitions - 10-14 days.

At unstable stenocardia and a myocardial infarction (as a part of a combination therapy) on 500-1000 mg of 1 times the day divided into 2 receptions further pass to reception 250 mg (1 capsule) 3 times a day.

At a cardialgia against the background of hormonal dystrophy of a myocardium drug is accepted on 250 mg (1 capsule) by 2 times a day. A course of treatment - 12 days.

At an alcoholism (in a combination with specific therapy) - on 500 mg (2 capsules) 4 times a day. A course of treatment - 7-10 days.

At disturbances of cerebral circulation after the end of therapy parenteral dosage forms drug is appointed inside on 500 mg/days. A course of treatment - 2-3 weeks.

Features of use:

Long-term experience of treatment of an acute myocardial infarction and unstable stenocardia in cardiological departments shows what милдрокард is not row drug I at an acute coronary syndrome and its use is not sharply necessary.

As drug can cause moderate tachycardia and a lowering of arterial pressure, it is necessary to be careful at a combination of a mildrokard with nitroglycerine, nifedipine, alpha adrenoblockers, antihypertensives and peripheral vazodilatator. 

Side effects:

Mildrokard is usually well had at use in the recommended doses. Hypersensitivity reactions, a skin itch, dyspepsia, tachycardia, excitement, a lowering of arterial pressure are seldom possible.

Interaction with other medicines:

Strengthens effect of koronarodilatiruyushchy and some hypotensive medicines, cardiac glycosides. Mildrokard it is possible to combine with anti-anginal drugs, anticoagulants, antiagregant, antiarrhytmic medicines, diuretics, bronchial spasmolytics.

Contraindications:

Hypersensitivity, increase in intracranial pressure (at disturbance of venous outflow and intracranial tumors), pregnancy, the lactation period.

Overdose:

The overdose phenomena are not described so far.
