General characteristics. Structure:

Active ingredient: 100 mg a meldoniya in 1 ml of solution.

The drug rendering the metabolic, cardioprotective, anti-anginal, anti-hypoxemic, adaptogenny, improving cerebral circulation action.

Pharmacological properties:

Pharmacodynamics. Is a structural synthetic analog of a gamma butyrobetaine — the predecessor of a carnitine. The gamma butyrobetaine hydroxylase inhibits enzyme, reduces synthesis of a carnitine and transport of long-chain fatty acids through cellular membranes, interferes with accumulation in cells of the activated forms of unoxidized fatty acids (including the acylcarnitine blocking delivery of ATP to cell organellas). Improves metabolic processes. At ischemia prevents disturbance of transport of ATP and activates glycolysis. As a result of decrease in synthesis of a carnitine the maintenance of the gamma butyrobetaine having vazodilatiruyushchy effect increases. At an acute myocardial infarction slows down a fabric necrosis, shortens the rehabilitation period. Against the background of heart failure improves contractility of a myocardium, increases tolerance to an exercise stress. At acute and chronic disorders of cerebral circulation promotes redistribution of a blood-groove to ischemic sites, thereby improves blood circulation in the ischemia center. Increases cellular and humoral immunity. Eliminates an abstinence syndrome at an alcoholism. Increases working capacity, reduces symptoms of a mental and physical overstrain, promotes increase in endurance.

Pharmacokinetics. At intake it is well soaked up from a GIT. Bioavailability — about 78%. Cmax is reached in 1–2 h by Biotransformiruyetsya in an organism with formation of two main metabolites which are removed by kidneys. T1/2 — 3–6 h.

Indications to use:

The lowered working capacity; a physical overstrain, including at athletes; the postoperative period for rehabilitation acceleration. As a part of complex therapy — an ischemic heart disease (stenocardia, a myocardial infarction), chronic heart failure, a cardialgia against the background of dishormonal dystrophy of a myocardium, acute and chronic disorders of cerebral circulation (a stroke, cerebrovascular insufficiency). An abstinence syndrome at an alcoholism (in a combination to specific therapy). In ophthalmology — a hemophthalmia and retinal apoplexies of various etiology, fibrinferments of the central vein of a retina and its branches, retinopathies of various etiology (diabetic, hypertensive) — only for parabulbar introduction.

Route of administration and doses:

Inside, in/in and in oil, parabulbarno. The way of introduction, a dose and duration of a course of treatment establish individually depending on indications, weight of a state, etc. Average doses for adults: at intake — on 250 mg 2–4 times a day, parenterally — 500 mg/days, parabulbarno — 50 mg/days.

Features of use:

It is necessary to be careful at prolonged use at patients with diseases of a liver and/or kidneys.

Use at pregnancy and feeding by a breast. It is not recommended at pregnancy (safety of use is not proved). For the period of treatment it is necessary to stop breastfeeding (it is unknown whether gets into breast milk).

Side effects:

Dyspepsia, excitement, tachycardia, arterial hypotension, skin itch.

Interaction with other medicines:

It is necessary to be careful at a combination to nitroglycerine, nifedipine, alpha adrenoblockers and other anti-hypertensive means and peripheral vazodilatator. Let's combine with anti-anginal means, anticoagulants and antiagregant, antiarrhytmic means, bronchial spasmolytics, diuretic means.

Contraindications:

Hypersensitivity, increase in intracranial pressure (at disturbance of venous outflow, intracranial tumors).

Restrictions to use. Pregnancy, feeding by a breast, children's age to 18 (capsules, solution for injections) or 12 years (syrup) as safety and efficiency of use are not defined.

Storage conditions:

To store at a temperature of 15-25 °C, in dry, protected from light and the place, unavailable to children. Period of validity 3 years. Not to use after the expiry date specified on packaging.
