General characteristics. Structure:

Active ingredient: meldoniya dihydrate of 100 mg, 500 mg

Excipients: water for and - to 1 ml.

Pharmacological properties:

The drug improving metabolism and power supply of fabrics. Meldony - a structural analog of a gamma butyrobetaine, suppresses gamma бутиробетаингидроксиназу, reduces synthesis of a carnitine that leads to reduction of transport of long-chain fatty acids through membranes of mitochondrions; interferes with accumulation in cells of the activated forms of unoxidized fatty acids - a derivative acylcarnitine and an acylcoenzyme And, preventing that their negative action.

In the conditions of ischemia optimizes processes of delivery of oxygen and its consumption in cells, prevents disturbance of transport of ATP; along with it activates glycolysis which proceeds without additional consumption of oxygen. As a result of decrease in concentration of a carnitine synthesis of a gamma butyrobetaine amplifies that leads to increase in content of nitrogen oxide II and causes an endothelium - a dependent vazodilatation.

The mechanism of action defines variety of pharmacological effects a meldoniya: increase in working capacity, reduction of symptoms of a mental and physical overstrain, cardiotyre-tread action. In case of acute ischemic injury of a myocardium slows down formation of a necrotic zone, shortens the rehabilitation period. At heart failure increases contractility of a myocardium, increases tolerance to an exercise stress, reduces the frequency of attacks of stenocardia. At acute and chronic ischemic disorders of cerebral circulation improves blood circulation in the ischemia center, promotes redistribution of blood in favor of the ischemic site. It is effective in case of vascular and dystrophic pathology of an eyeground. Also tonic action on TsNS, elimination of functional disturbances of somatic and vegetative nervous systems at an alcoholism in the period of abstinence is characteristic.

At in introduction bioavailability - 100%. Cmax in plasma is reached directly after introduction.

Metabolism and removal
It is metabolized in an organism with formation of two main metabolites which are removed by kidneys.

Indications to use:

— as a part of a combination therapy at an ischemic heart disease (unstable stenocardia, a myocardial infarction, stable stenocardia), chronic heart failure; a cardialgia against the background of dishormonal dystrophy of a myocardium;
— as a part of a combination therapy of disturbances of cerebral circulation (an ischemic stroke of a brain, chronic disturbances of blood circulation of a brain);
— as a part of a combination therapy of vascular pathology of an eyeground and a retina of various etiology (a hemophthalmia and a retinal apoplexy of various etiology, fibrinferments of the central vein of a retina and its branches, a diabetic and azotemic retinitis);
— physical overworks, the postoperative period for rehabilitation acceleration;
— an abstinence alcoholic syndrome (in a combination with specific therapy).

Route of administration and doses:

The drug is administered in/in, in oil, parabulbarno, retrobulbarno, subkonjyunktivalno.
In view of possible exciting effect it is recommended to use drug in the first half of day.
As a part of a combination therapy at an ischemic heart disease:

Unstable stenocardia, myocardial infarction - in/in struyno on 500-1000 mg (1-2 ampoules) of 1 times / during the first 3-4 days, further appoint inside 250 mg 2 first 3-4 days then 2 times a week on 250 mg 3 Course of treatment - 4-6 weeks.

Stable stenocardia: in/in struyno on 500-1000 mg of 1 times / within 3-4 days, after that appoint inside 2 times a week on 250 mg 3 Course of treatment - 4-6 weeks.

Chronic heart failure (as a part of a combination therapy) - in/in struyno on 500-1000 mg of 1 times / or in oil on 500 mg 1-2, a course of treatment - 10-14 days, appoint after that inside in a dose 500-1000 mg / the Course of treatment - 4-6 weeks.

Cardialgia against the background of dishormonal dystrophy of a myocardium (as a part of a combination therapy) - in/in struyno on 500-1000 mg of 1 times / or in oil on 500 mg 1-2 within 10-14 days, after that appoint inside on 250 mg 2 (in the morning and in the evening). A course of treatment - 12 days.

Disturbances of cerebral circulation (as a part of a combination therapy):

Ischemic stroke - in/in on 500 mg of 1 times / within 10 days, inside in a dose 500 mg / the Course of treatment - 2-3 weeks after that appoint.

Chronic disturbances of cerebral circulation - 1 time, in oil on 500 mg,/, it is desirable in the first half of day. A course of treatment - 2-3 weeks.

Vascular pathology of an eyeground and retina of various etiology (hemophthalmia and retinal apoplexy of various etiology, fibrinferments of the central vein of a retina and its branches): retrobulbarno or subkonjyunktivalno enter on 0.5 ml of solution for injections 100 mg/ml within 10 days, at a diabetic and azotemic retinitis - retrobulbarno.

There are no data on adverse influence of drug on ability to manage vehicles.

Side effects:

Seldom: allergic reactions (skin itch, rashes, reddening and face edema), dyspepsia, tachycardia, excitement, decrease in the ABP, sleeplessness.

Interaction with other medicines:

Not to apply along with other medicines containing meldoniya (risk of aggravation of side effects).
Strengthens action koronarodilatiruyushchy and antihypertensives, cardiac glycosides.
In view of possible development of moderate tachycardia and arterial hypotension, it is necessary to be careful at a combination with nitroglycerine, nifedipine, beta adrenoblockers, hypotensive medicines and peripheral vazodilatator.
It is possible to combine with anti-anginal medicines, anticoagulants, antiagregant, antiarrhytmic medicines, diuretics, bronchial spasmolytics.

Contraindications:

— increase in intracranial pressure (at disturbance of venous outflow and tumors of a brain);
— pregnancy;
— period of a lactation (breastfeeding);
— children's age up to 18 years;
— hypersensitivity to drug components.

With care it is necessary to use drug at patients with chronic diseases of a liver and kidneys.

Use of drug VAZOMAG at pregnancy and feeding by a breast
Use at pregnancy and in the period of a lactation (breastfeeding) is contraindicated.

Use at abnormal liver functions

Use for children
It is contraindicated to children up to 18 years.

Overdose:

Cases of overdose of drug of Vazomag are not established.
