General characteristics. Structure:

Active ingredient: 10 mg of diclofenac of sodium in the form of diethyl amine diclofenac in 1 g of gel.

Excipients: propylene glycol, chlorocresol, carbomer 940, EDTA disodium salt, light liquid paraffin, isopropyl alcohol, benzyl alcohol, sodium sulfite, polysorbate 80, a wild lavender, trietanolamine, the water purified.

Pharmacological properties:

Pharmacodynamics. Diclofenac – non-steroidal anti-inflammatory drug (NPVS) from group of derivatives of phenylacetic acid. Has the expressed anesthetic and antiinflammatory action. The mechanism of action is connected with oppression of synthesis of prostaglandins, kinin and other mediators of an inflammation and pain, reduction of permeability of capillaries and the stabilizing influence on lizosomalny membranes. Diclofenac considerably reduces pain (both at rest, and at the movement), reduces morning constraint, a swelling of joints and by that improves a condition of the patient.

At the nonspecific inflammation arising after injuries or operations, diclofenac reduces feeling of pain and inflammatory hypostasis. At topical administration of drug active agent gets through skin and collects directly in the inflammation center.

Pharmacokinetics. About 3,3% of a dose at topical administration are soaked up through skin and it appears in a blood plasma. Diclofenac well gets into fabrics and synovial fluid where its concentration exceeds concentration in a blood plasma. It is removed by kidneys.

Indications to use:

• posttraumatic inflammation of sinews, sheaves, muscles, joints,
• musculoskeletal damages: traumatic stretchings, dislocations, bruises, contusions,
• local inflammatory and degenerative diseases of joints: pseudorheumatism, osteoarthrosis of joints and backbone, periartropatiya,
• local inflammatory and degenerative diseases of soft tissues and circumarticular soft tissues: tendovaginitis, syndrome "shoulder hand", bursitis.

Route of administration and doses:

Drug is intended only for external (local) use at adults and children by age of 6 years.

The amount of drug which is applied on skin depends on the size of an affected area and its pain. Usually use about 2 - 4 g (a strip of gel of 1 - 2 cm) of drug on one use and slightly rub in skin. Such procedure is repeated on average by 2 - 3 times a day or are more often, depending on existence of pain and reaction to treatment. Gel should be rubbed in skin until it is not absorbed in skin completely. It is not necessary to apply a bandage.

The course of treatment without consultation of the doctor should not exceed 10 days.

Features of use:

Drug is intended only for external (local) use. Gel should be applied only on the unimpaired sites of skin. It is necessary to avoid hit of drug in eyes, on mucous membranes of an oral cavity and other body parts.

With care use drug at patients with stomach ulcer or a duodenum in the anamnesis, at the expressed abnormal liver functions and/or kidneys, hemopoiesis disturbances.

Gel can be applied along with other dosage forms of diclofenac.

Side effects:

Drug is usually well transferred by patients. There can sometimes be local allergic reactions in the form of erubescence, a burning sensation and an itch or emergence of skin rashes on site of drug use.

In case of prolonged use of drug on big sites of skin possibility of some system side effects is not excluded: a headache, dizziness, allergic rashes, it is very rare – a bronchospasm, system anaphylactic reactions. In isolated cases manifestations of a photosensitization can be noted.

Interaction with other medicines:

At external use of drug interaction with other medicines is not revealed.

Contraindications:

Hypersensitivity to diclofenac or other components of drug. Also patients cannot use drug with hypersensitivity to acetylsalicylic acid and other non-steroidal anti-inflammatory drugs which is shown in the form of rhinitis, a Quincke's disease, asthmatic states, an urtikariya. Drug is contraindicated to pregnant women (the last trimester of pregnancy), mothers who nurse, and to children up to 6 years.

Overdose:

Data on cases of overdose of gel are absent.

Storage conditions:

To store in unavailable to children, the dark, dry place at a temperature not above 25 °C. A period of validity – 3 years.
