Bee tolite

General characteristics. Structure:

Active ingredients: 5 ml of suspension oral contain sulfamethoxazole 200 mg, Trimethoprimum of 40 mg.
Excipients: sugar confectionery, saccharin of sodium, sodium carboxymethylcellulose, xanthane gum, menthol racemic, methylparahydroxybenzoate (Е 216), пропилпарагидроксибензоат (Е 218), Tropic 566 fragrance (contains 1,2 propylene glycol), citric acid, sodium citrate, the water purified.

Main physical and chemical properties: Suspension of white or almost white color with a characteristic fragrant smell, sweet taste. At storage stratification which is eliminated when stirring is possible.

Pharmacological properties:

Pharmacodynamics. Bee tolite — the combined drug which contains sulfamethoxazole — streptocide which inhibits synthesis of folic acid, and derivative a diaminopyridimine Trimethoprimum — inhibitor of bacterial reductase of degidrofoliyevy acid in the ratio 5:1.

Drug shows bacteriostatic and bactericidal actions. Drug of a broad spectrum of activity, active in relation to  E. сoli. ( including to enteropathogenic strains), indolpositive strains of Proteus spp. ( including Proteus vulgaris), Morganella morganii, Klebsiella spp., Proteus mirabilis, Enterobacter spp., H. Influenzae, Pneumocystis carinii, S. рneumoniae, Shigella flexneri, Shigella sonnei.

Pharmacokinetics. Drug is quickly soaked up in digestive tract. The maximum concentration of both components in blood serum is registered in 1–4 h which remains about 12 h. Gets into fabrics and liquids of an organism: in tissues of lungs, palatine tonsils, a prostate, kidneys, liquor of an inner ear, allocation of bronchial tubes and a vagina, and also is removed with breast milk and gets through a placental barrier. The volume of distribution of sulfamethoxazole is equal to 360 ml/kg of body weight, and Trimethoprimum — 2000 ml/kg. Drug in a liver is metabolized. Both components mainly are removed by kidneys. The biological period of semi-elimination of sulfamethoxazole makes 9–11 h, Trimethoprimum of 10-2 h. The semi-conclusion period at children depends on age: by first year of life — 7–8 hours, of 1–10 years of life — 5–6 h.

Indications to use:

Bee tolite is applied to treatment of the diseases caused by microorganisms, sensitive to drug:

- respiratory infections (bronchitis, pneumonia, pneumocystic pneumonia);

- inflammation of side bosoms of a nose, middle ear;

- infections of uric ways and kidneys (pyelonephritis, pyelitis, cystitis, urethritis);

- bacterial infections of generative organs;

- the digestive tract infections caused by strains of salmonellas, shigellas, Escherichia coli;

- purulent dermatitis.

Route of administration and doses:

Orally after meal. Before use shake up before receiving homogeneous suspension. The measured glass with divisions is added to a bottle.
Drug is appointed to children from 6 months to 2 years — on 2,5 ml (120 mg) in 12 h;
   from 3 to 5 years — on 5 ml (240 mg) in 12 h;    from 6 to 12 years — on 10 ml (480 mg) in 12 h.

To adults and children of 12 years — on 20 ml (960 mg) in 12 h.
The course of treatment makes 10–14 days (5 days at digestive tract infections).

At infection of Pneumocystis carinii of 100 mg of sulfamethoxazole and 20 mg of Trimethoprimum on 1 kg of body weight a day in equal doses each 6 h,  for 14–21 days.

Features of use:

With care appoint drug at patients with a liver and renal failure, deficit of folic acid, allergic diseases, bronchial asthma, disturbance of a krovoobrazovaniye; at an alcoholism. Patients have to accept enough liquid. At patients of old age the risk of an abnormal liver function and kidneys, emergence of heavy skin reactions, oppression of an erythrogenesis increases. To patients who manage vehicles or perform works with potentially dangerous mechanisms, appoint drug with care as it can cause disturbance from TsNS. It is necessary to avoid radiation UF during treatment. It is not necessary to violate rules of use of medicine, it can be injurious to health.

Side effects:

There can be frustration from digestive tract (nausea, vomiting, lack of appetite, it is rare — diarrhea, an abdominal pain, an inflammation of a mucous membrane of a stomach); skin allergic reactions (rash and urticaria are more often, the multitoformny erythema, a skin itch, exfoliative dermatitis, hypersensitivity to light is much more rare), fever, a fever. Changes in a blood picture (thrombocytopenia, a neutropenia, seldom an agranulocytosis and megaloblastny anemia) are possible, it is rare — parenchymatous hepatitis, reverse disorders of the central nervous system, a headache and dizziness, frustration of a dream, a hallucination, a renal failure (a crystalluria, a hamaturia, a hypercreatinemia).

Seldom chances of essential undesirable effects of use of streptocides: Stephen's syndrome — Johnson, a Lyell's disease. An acute necrosis of a liver, allergic myocarditis, pancreatitis, an inflammation of a mucous oral cavity, aseptic meningitis, a depression, apathy, a joint pain and muscles, it is very rare — infiltration of lungs, single cases of pseudomembranous colitis, pancreatitis, stomatitis, a glossitis, an arthralgia, a mialgiya, a photosensitization.

 In case of any undesirable phenomena it is necessary to see a doctor!

Interaction with other medicines:

Antidiabetic drugs, NPZZ, dipheninum, indirect anticoagulants, barbiturates increase risk of emergence of side reactions. Vitamin C increases risk of formation of a crystalluria.

Diuretics, especially tiazida, at patients of old age increase risk of development of thrombocytopenia. At such patients of Bee tolite can increase digoxin level in blood serum.

At simultaneous use with tricyclic antidepressants drug reduces their efficiency.

Bee tolite can strengthen effect of anticoagulants, a diphenyl, Phenytoinum and to reduce activity of peroral contraception, increases concentration of free fraction of a methotrexate in blood serum.

Contraindications:

- hypersensitivity to drug components;

- heavy liver failure;

- heavy renal failure;

- anemia which is caused by insufficiency of folic acid, an agranulocytosis;

- pregnancy and period of feeding by a breast;

- age up to 6 months.

 It is not recommended to apply at streptococcal tonsillitis and pharyngitis, a hyperbilirubinemia at children.

Overdose:

At overdose there are expressed symptoms of side effect, namely: nausea, vomiting, gripes, dizziness, headache, drowsiness, loss of consciousness, visual disturbances, fever, depression, disorientation, hamaturia, crystalluria; at long overdose: thrombocytopenia, leukopenia, megaloblastny anemia, jaundice.

Treatment: to stop drug use, to cause vomiting and to make a gastric lavage. To see a doctor. Depending on defeat carry out the corresponding therapy. The hemodialysis is inefficient. Symptomatic treatment.

Storage conditions:

To store at  a temperature not above 25 °C in the place, unavailable to children.
