General characteristics. Structure:
Active ingredient: 5 g of an acyclovir.
Excipients: poly(ethylene oxide) 400, poly(ethylene oxide) 1500.
Antiviral means for topical administration.
Pharmacological properties:
Pharmacodynamics. The acyclovir is antiviral means from group of synthetic analogs of an acyclic purine nucleoside — the dezoksiguanidin which is the DNA component. The acyclovir has high specificity concerning the Herpes simplex viruses, 1 and 2 type, the virus causing chicken pox and shingles (Varicella zoster), Epstein-Burra's virus. It is moderately active concerning cytomegaloviruses.
The acyclovir gets directly into the cells infected with a virus. The cells infected with a virus produce a virus thymidinekinase which in turn phosphorizes an acyclovir to the atsiklovirtrifosfat which is active and selection inhibitor of a DNA polymerase of viruses.
Possibly the inhibition mechanism atsiklovirtrifosfaty synthesis of DNA consists that it is the substrate allowing to carry out the communication 3-5 necessary for extension of a chain of DNA for this enzyme.
Thus the premature rupture of a chain of DNA is carried out that leads to suppression of replication of a virus.
Pharmacokinetics. At use of ointment on intact skin: absorption is minimum; is not defined in blood and urine. The affected skin: absorption is moderate.
Indications to use:
The infections of skin and mucous membranes caused by the Herpes simplex virus, genital herpes, shingles, chicken pox.
Route of administration and doses:
Outwardly. Ointment is applied on affected areas of 4-6 times a day (as soon as possible after the beginning of an infection). Ointment is applied either a cotton plug, or clean hands to avoid additional infection of affected areas. Therapy should be continued till that time while on bubbles the crust is not formed or until they completely begin to live. Duration of treatment is not less than 5 days, at most 10 days.
Features of use:
Ointment should not be applied on mucous membranes of an oral cavity, a nose, eyes and a vagina. Efficiency of treatment by that will be higher, than earlier it is begun (at the first symptoms of an infection).
For implementation of medical effect of an acyclovir the condition of immune system of an organism matters. Patients with reduced immunity (at HIV/AIDS or after transplantation of marrow) against the background of topical administration of ointment of an acyclovir should appoint system administration of drug, just as in case of a heavy and recurrent current of a herpes infection.
The acyclovir does not prevent transfer of herpes sexually therefore during treatment it is necessary to abstain from sexual contacts, even in the absence of clinical manifestations.
Side effects:
Hyperemia, dryness, skin peeling; burning, an inflammation at hit on mucous membranes. Development of allergic dermatitis is possible.
Interaction with other medicines:
Strengthening of effect is noted at co-administration of an immunostimulator.
Contraindications:
Hypersensitivity to an acyclovir or to any other component of drug.
With care: pregnancy, lactation period, dehydration, renal failure.
Storage conditions:
In the dry, protected from light place at a temperature not above 25 °C. To protect from children. Period of validity 2 years. Not to use after the expiry date specified on packaging.
