Gentamycin

General characteristics. Structure:

Active ingredient: 1 mg of Gentamycini sulfas in 1 g of ointment.

Pharmacological properties:

Pharmacodynamics. Gentamycin is characterized by a wide range of antimicrobic action. It is active concerning the majority of gram-negative and gram-positive microorganisms: Pseudomonas aeruginosa, Escherichia coli, Proteus spp., Klebsiella spp., Serratia spp., Salmonella spp., Shigella spp., Staphylococcus spp., including concerning mnozhestvennoustoychivy strains. Drug does not affect anaerobic bacteria, mushrooms, viruses.

Has bactericidal effect. Actively getting through a cellular membrane of bacteria, communicates with 30S in subunit of bacterial ribosomes and activator protein synthesis oppresses.

Pharmacokinetics. Gentamycin badly gets through the unimpaired skin and mucous membranes thereof at topical administration of ointment drug practically does not possess resorptive action. Action duration after single use — 8 — 12 hours. When drawing on the injured skin easily it is soaked up. Gentamycin is removed by kidneys in not changed look, is preferential due to glomerular filtering.

Indications to use:

Ointment of gentamycin is applied in surgery to topical treatment of wounds (in the second phase of a wound process) for the purpose of prevention of purulent complications; for treatment of burns of II-IIIA of degree (in the second phase of a wound process) and frostbites; for treatment of pyodermas, superficial folliculites, the furunculosis infected seborrheal dermatitis, infected with an acne, eczemas, at secondary bacterial infection at the fungal and virus infections of skin infected varicose ulcers at household damages (for treatment of small wounds and cuts).

Route of administration and doses:

Ointment of gentamycin is applied outwardly.

Medicine is applied on an affected area of skin, after removal of pus and necrotic masses, with a thin layer by 2 — 3 times a day, at burns — 2 — 3 times a week. At extensive burn defeats the daily dose of ointment should not exceed 200 g (200 mg of an antibiotic). Duration of treatment depends on a form and disease severity and makes 7 — 14 days.

At treatment of dermatological diseases medicine is applied with a thin layer on an affected area by 2 — 3 times a day, or apply on a gauze bandage with the subsequent application on an affected area of skin. Treatment duration — 7 — 14 days.

In the presence of exudate of a wound and an erosion previously wash out solutions of Furacilin (1:5000), hydrogen peroxide (3%) or boric acid (1 — 2%).

Side effects:

At gentamycin ointment use perhaps local irritant action (reddening, itch) and allergic reactions (skin rash, itch, small tortoiseshell). At emergence of allergic reactions drug is cancelled and carry out the desensibilizing therapy.

Interaction with other medicines:

It is not necessary to appoint along with other antibiotics having from - and nephrotoxic action (streptomycin, Kanamycinum, флоримицин, Monomycinum, Ristomycinum), with furosemide. Activity of gentamycin decreases in the presence of ions of Na+, K+, Ca ++, Mg ++, and also a number of anions (sulfates, phosphates, nitrates, etc.). There are data on a synergism of combinations of gentamycin with a dioxidin and on increase in its therapeutic effectiveness at combined use with corticosteroids.

Contraindications:

Use of medicine is contraindicated at the increased individual sensitivity to gentamycin.

Overdose:

At prolonged use of medicine in the doses exceeding recommended perhaps nephrotoxic (a proteinuria, an azotemia, etc.) and ototoksichesky (vestibular disturbances, it is rare — decrease in hearing) action, a hyperbilirubinemia, increase in activity of hepatic transaminases, changes of cellular composition of peripheral blood.
