Dimedrol

General characteristics. Structure:

Active agent – Dimedrol (a difengidramina a hydrochloride) 10 mg.

Excipient – water for injections to 1 ml.

Pharmacological properties:

Pharmacokinetics. It is quickly absorbed from digestive tract. The maximum concentration in a blood plasma is reached in 20-40 minutes (in the greatest concentration is defined in lungs, a spleen, kidneys, a liver, a brain and muscles). Gets through a blood-brain barrier. It is metabolized in a liver, partially – in lungs and kidneys. The elimination half-life is brought of 4-10 o'clock out of fabrics in 6 hours. During the day it is completely removed by kidneys in the form of the metabolites conjugated with glucuronic acid. Essential quantities are removed with milk and can cause sedation in children of chest age (the paradoxical reaction which is characterized by hyperexcitability can be observed).

Pharmacodynamics. Antihistaminic, sedative and hypnagogue. H1 receptors are a blocker histamine. Has antiallergic activity, renders the mestnoanesteziruyushchy, spasmolytic and moderate ganglio-blocking action. Causes sedative and somnolent effect, has moderate antiemetic effect, and also has the central cholinolytic activity. Drug reduces reaction of an organism to a histamine, removes the spasms of smooth muscles caused by a histamine, reduces permeability of capillaries, prevents development of the hypostasis of fabrics caused by a histamine, warns and facilitates development of allergic reactions.

Indications to use:

- urticaria, hay fever, Quincke's disease;

- allergic conjunctivitis, vasculomotor rhinitis, a hemorrhagic vasculitis, a serum disease, a pruritic dermatosis, sleep disorders (monotherapy or in a combination with hypnotic drugs);

- chorea, sea and aeroembolism;

- Menyer's syndrome;

- the allergic complications connected with use of various medicines (including antibiotics), fermental and other drugs, with hemotransfusion and blood-substituting liquids.

Route of administration and doses:

Intramusculary – in doses of 10-50 mg. Intravenously kapelno – 20-50 mg of Dimedrol in 75-100 ml of isotonic solution of sodium of chloride.

To children Dimedrol is appointed in smaller doses: till 1 year — on 2 - 5 mg, from 2 to 5 years — on 5 - 15 mg, from 6 to 12 years on 15 – 30 mg on reception.

The maximum doses for adults: intramusculary – one-time 50 mg, daily 150 mg. Course of treatment of 10-15 days.

Features of use:

During treatment it is not necessary to be affected by sunlight, it is necessary to avoid alcohol intake.

Because of sedative and somnolent action drivers of transport and other persons whose activity demands bystry mental and motor reaction cannot appoint Dimedrol in operating time.

Side effects:

- drowsiness, general weakness, reduction in the rate of psychomotor reactions;

- at children perhaps paradoxical development of sleeplessness, irritability and euphoria.

Seldom:

- dizziness, a headache, dryness in a mouth, nausea, an incoordination of movements, a tremor.

Interaction with other medicines:

At simultaneous use strengthens effect of ethanol, the drugs oppressing it is central a nervous system.
At simultaneous use monoamine oxidase inhibitors increase anticholinergic activity of drug.
Antagonistic interaction is noted at joint appointment with psychostimulants. Reduces efficiency of Apomorphinum as vomitive at treatment of poisoning. Strengthens anticholinergic effects of drugs with holinoblokiruyushchy activity.

Contraindications:

- closed-angle glaucoma;

- prostatauxe;

- the stenosing stomach ulcer and a duodenum;

- bladder neck stenosis;

- bronchial asthma;

- epilepsy;

- pregnancy and period of a lactation;

- hypersensitivity to drug.

Overdose:

Symptoms: dryness in a mouth, the complicated breath, a resistant mydriasis, face reddening, oppression or excitement (is more often at children) the central nervous system, confusion of consciousness; children have a development of spasms and a lethal outcome.

Treatment: induction of vomiting, gastric lavage, purpose of absorbent carbon; a symptomatic and maintenance therapy against the background of careful control of breath and level of arterial pressure.

Storage conditions:

To store in the dry, protected from light place at a temperature not above 25 °C.
