Novitropan

General characteristics. Structure:

Active ingredient: 5 mg of an okibutinin of chloride.

Novitropan has direct spasmolytic effect on smooth muscles.

Pharmacological properties:

Pharmacodynamics. Blocks m-holinoretseptory. Eliminates spasms and lowers a tone of unstriated muscles (a GIT, zhelche-and urinary tract, a uterus, especially smooth muscles of a bladder). At patients with a neurogenic bladder новитропан increases bladder capacity, reduces the frequency of reductions of a detruzor, constrains and reduces the frequency of urinations (any and involuntary).

Pharmacokinetics. The maximum concentration of active agent in plasma in 1 hour after oral administration of drug. Bioavailability makes 6%, thanks to presistemny metabolism. The elimination half-life makes about 2 hours. Gets through a blood-brain barrier. It is found in breast milk of mother.

Indications to use:

The neurogenic bladder, urine incontience (the idiopathic nature), hyperreflexia and dysfunction of a detruzor, night enuresis and neurogenic dysfunctions of a bladder at children is more senior than 5 years.

Route of administration and doses:

Drug is accepted inside, to food. The adult appoint drug in an initial dose of 5 mg (1 tablet), 2-3 times a day. The maximum daily dose of 20 mg (on 1 tablet 4 times a day) Elderly and to children is more senior than 5 years appoint 5 mg 2 times a day. The maximum daily dose of 15 mg (on 1 tablet 3 times a day). At night enuresis – the last reception before going to bed.

Features of use:

During treatment drug is not recommended to be managed motor transport and other potentially dangerous types of activity demanding increase in concentration of attention and bystry psychomotor reaction because of emergence of drowsiness and an ambiguity of sight.

Novitropan with care appoint at an abnormal liver function and kidneys. It is also necessary to be careful at patients with hernia of the esophageal opening connected with a gastroesophageal reflux. At appointment as the patient with ulcer colitis новитропан suppresses an intestines perilstatika that can result in paralytic impassability of intestines. Owing to use of a novitropan symptoms of a hyperthyroidism, neurocirculatory dystonia, a coronary disease of heart, congestive heart failure, cardiac arrhythmia, arterial hypertension, a prostatauxe can amplify. At high temperature of the environment новитропан can cause developing of fever and cordial weakness (the lowered sweating).

Side effects:

Dryness in a mouth, nausea, vomiting, decrease in motility of a stomach and intestines, frustration of an urination (ischuria), the general weakness, dizziness, tachycardia, drowsiness, a vision disorder (a mydriasis, an amblyopia, accommodation paralysis), increase in intraocular pressure, decrease in sweating, impotence, suppression of a lactation, allergic reactions (rashes).

Interaction with other medicines:

At simultaneous use of drug with sedatives and alcohol drowsiness and dizziness amplifies.

Contraindications:

Children up to 5 years. Pregnancy, feeding by a breast. Hypersensitivity to drug. It is closed - and an open angle glaucoma, obstruction of digestive tract, an intestines atony, expansion of a colon (toxic, caused by ulcer colitis), a myasthenia, bleedings. Safety of use of an oksibutinin during pregnancy is not established.

Overdose:

Symptoms: excitement of TsNS – concern, irritability, a tremor, spasms, nonsense, hallucinations, nausea, vomiting, face reddening, tachycardia, hyper - hypotension, respiratory insufficiency, a coma.

Treatment: breath maintenance to cause vomiting with the subsequent gastric lavage and use of absorbent carbon and purgatives. For regression of symptoms of anticholinergic intoxication appoint physostigmine (a cholinesterase blocker), at a hyper pyrexia – cold compresses (including ice, spirit rubdowns).
