General characteristics. Structure:
Active ingredients: 0,5 g of paracetamol.
Excipients: cellulose microcrystallic, starch, gelatin, methylparaben, propylparaben, magnesium the stearate, talc purified of silicon dioxide colloid, sodium lauryl sulfate, sodium krakhmalglikolit, a gidroksimetilpropiltsellyuloza, ethyl cellulose, titanium dioxide, propylene glycol.
The drug possessing anesthetic, febrifugal, antiinflammatory action.
Pharmacological properties:
Pharmacodynamics. Paracetamol has anesthetics, febrifugal, antiinflammatory properties. The mechanism of analgesic effect of paracetamol connected with suppression of biosynthesis of prostaglandins. Inhibiting a tsiklogenaza, it interferes with formation of E2 and F2 prostaglandins which play an important role at perception of pain stimulations nociceptors and transfers of excitement to the central nervous system.
Paracetamol effectively inhibits cyclooxygenase in cells of TsNS and is much weaker - in peripheral fabrics. The antipyretic effect of paracetamol is explained by inhibition of biosynthesis of prostaglandins directly in a hypothalamus, including E1 and E2 which are mediators of the center of thermal control.
Pharmacokinetics. The pharmacokinetics of paracetamol is characterized by bystry absorption from a digestive tract, hypodispersion in fabrics, rather short elimination half-life. The maximum concentration in plasma is defined ordinary in 30 - 60 minutes after intake. It is shown within 5 hours after a single dose. Bioavailability of drug very high - to 90%. Extent of linkng with proteins makes 25-50%. Paracetamol is metabolized (to 80%) microsomal enzymes of a liver, it is removed by a glyukurirovaniye and sulphation, only 3% are removed by kidneys in not changed state.
Indications to use:
Pain syndrome of small and medium intensity of different genesis (headache, migraine, dentagra, neuralgia, mialgiya, menalgiya; pain at injuries, burns and other states), a diarrhea at infectious and inflammatory diseases.
Route of administration and doses:
To adults and teenagers with body weight more than 60 kg - on 0, 5 g each 6 hours (the maximum daily dose - 4 g). To children from 6 to 12 years – on 0, 25–0, 5 g each 6 hours (the maximum daily dose - 2 g). The maximum duration of treatment – 5 – 7 days.
Features of use:
At prolonged use of drug it is necessary to monitor (1 time for 10 days) function of a liver, kidneys, behind a condition of the hemopoietic system.
Side effects:
At prolonged use of Milistan, especially in large doses, an opportunity nephrotoxic and a hepatotoxic action, metgemoglobino-education is not excluded. Allergic reactions are possible.
Interaction with other medicines:
Caffeine can exponentiate analgeziruyushchy action of Milistan. Drug can strengthen effect of indirect anticoagulants, derivative coumarin, an indole. Rifampicin, phenobarbital and alcohol strengthen a hepatotoxic action of Milistan. To apply carefully with other drugs which contain paracetamol.
Contraindications:
Hypersensitivity to paracetamol. Abnormal liver function and kidneys, diabetes mellitus, blood diseases, first trimester of pregnancy. Children's age up to 6 years.
Overdose:
Symptoms: sleep disorder, itch, rashes, urination delay, fatigue, tremor, tachycardia.
Treatment: carry out symptomatic therapy. The hemodialysis is inefficient.
Storage conditions:
To store in dry, protected from light and the place, unavailable to children, at a temperature not above 25 °C. Period of storage 3 years.
