Acrodynia


Description:


Acrodynia (Feer's disease, Swift's syndrome, an erythrosis polineyropatichesky) - the disease of not clear etiology (generally at children up to 5 years) which is characterized by changes of behavior before the expressed mental disturbances, polymorphic changes of skin of extremities with a severe itch and pains, hypotonia of muscles, paresis, paralyzes, sometimes convulsive attacks. Is considered as neuroallergic reaction on nonspecific harm (for example, on mercury at treatment of a helminthic invasion by santonin, use of mercury-containing ointments and powders). Skin test on mercury happens positive. Availability of mercury manages to be established in urine. The name "Acrodynia" - extremity pains.


Acrodynia symptoms:


The acrodynia is observed, as a rule, at children up to 5 years, is rare - later. Begins with emergence of tearfulness, fatigue, a depression, negativism in behavior. Sometimes deeper disturbances of mentality - nonsense, hallucinations. At a part of patients thirst, bulimia, pristupoobrazny temperature increase, tachycardia, increase in the ABP, disturbance of a respiratory rhythm, the strengthened sialosis, abdominal pains, diarrhea are noted. In the heat of a disease extremity pains, hypotonia of muscles, sometimes paresis and paralyzes of peripheral and central character, a tremor, spasms, epileptic seizures develop. Liquor normal, regarding cases increase in protein, a cytosis are observed.

Dermatological symptomatology quite various. Skin of brushes and feet is brightly hyperemic or the tsianotichna, is sometimes edematous. A characteristic sign - a severe itch and pains in affected areas of extremities therefore children constantly rub legs one about another. There can be scarlatinous or korepodobny rashes. Quite often a hyperkeratosis, pigmentation, the hypertrichosis or a poredeniye and canities of hair are found. Some patients have a trichotillomania. The ulcers which are coming to an end with mutilation are occasionally formed. In hard cases stomatitis, dedentition join.

The disease develops gradually, average duration of 3-6 months with the periods of strengthening and weakening of clinic. In most cases comes to an end with spontaneous treatment, but also lethal outcomes, generally because of accession of consecutive infection are possible.


Acrodynia reasons:


The etiology and pathogeny of an acrodynia are unknown. According to one authors, an infectious disease with the unspecified activator (local flashes in children's collectives in cold season are described); according to others - result of defeat of the central and peripheral nervous system of toxic genesis; according to the third - a consequence of hypovitaminosis of B6. The pathoanatomical picture in acute cases comes down to the diffusion inflammatory process in all nervous system which is most expressed in hypothalamic area and side horns of a spinal cord.

In chronic cases degenerative changes with the same localization prevail.


Treatment of the Acrodynia:


Symptomatic treatment: vitamins, especially Wb, Atropini sulfas (0,02 - 0,05 mg on reception), phenobarbital (0,01 - 0,05 on reception), good nutrition.
